For a person with acute diarrhea, the priority is replacing lost fluids and electrolytes—not trying to “rebalance” the gut with a supplement. Drink fluids; use an oral rehydration solution (ORS) when diarrhea is severe or dehydration is a concern, and seek medical advice for warning signs or if you are at higher risk. Probiotics have not been established as a treatment for an acute episode.
What to do first: replace fluids and electrolytes
Diarrhea can cause loss of both fluid and electrolytes. For routine replacement, water and other liquids containing electrolytes are options. When diarrhea is severe, the CDC recommends ORS to help prevent dehydration. ORS is a glucose-and-electrolyte solution; packets or powder mixed with water according to the product directions can be a practical option. - Follow the product’s mixing directions rather than changing the amount of water.
- Ask a doctor before using ORS if you have diabetes, kidney disease, weakened immunity, or another health condition. Caregivers should ask before giving it to an infant.
- If you cannot keep fluids down, or dehydration signs persist despite ORS, seek urgent medical advice.
Choose a product intended as oral rehydration solution, check its age suitability and mixing instructions, and follow clinician or pharmacist advice. The CDC names commercial products as examples but does not endorse brands; the cited guidance does not establish a brand ranking.
Do probiotics help acute diarrhea?
A probiotic is a live microorganism, often a bacterium, similar to microbes found in the digestive tract. That definition does not mean a supplement treats the illness currently causing diarrhea. NIDDK says research on probiotics for diarrhea is continuing and that many doctors and professional societies do not consider the evidence sufficient to support using them to treat or prevent diarrhea. If you are considering a probiotic, ask a clinician first, particularly if you have a health condition or weakened immunity. Do not let a supplement take the place of rehydration or medical care when warning signs are present.
When should you seek medical help?
Contact a doctor promptly for mental-state changes, frequent vomiting, severe abdominal or rectal pain, symptoms of dehydration, or black, tarry stool or stool containing red blood or pus. NIDDK also advises adults to contact a doctor for diarrhea lasting more than two days, high fever, or six or more loose stools in a day. Its symptoms and causes guidance lists these warning signs.

Seek advice sooner if you are pregnant, over 65, taking antibiotics, or immunocompromised, as these groups are more likely to have health problems. Thresholds differ between public guidance: NIDDK advises adult contact after more than two days, while the NHS says diarrhea commonly stops within five to seven days and identifies separate reasons for urgent advice. Recognize dehydration that needs urgent attention
Reduced urination, dark urine, tiredness, and dizziness can signal dehydration. The NHS advises urgent help if dizziness on standing does not go away or urination is substantially reduced. Are antidiarrheal medicines appropriate?
The CDC says an antidiarrheal medicine may relieve diarrhea and cramps when there is no blood in the stool or fever. Avoid these medicines when you have high fever or blood in your stool, because they may make the illness worse.
