What Is White Spot on Tropical Fish? How to Identify and Treat Ich

White spot is usually ich, a contagious parasite—not dirt or fungus. Learn the life cycle, treatment schedule, salt and heat precautions, and prevention steps.

White spot on tropical freshwater fish is usually ich ( Ichthyophthirius multifiliis), a contagious ciliated protozoan parasite. It appears as tiny salt-like dots or blisters, but serious gill infections can cause rapid breathing before any spots are visible. Ich is not dirt or a harmless cosmetic mark: untreated outbreaks can kill fish quickly.

How to recognize ich

Typical lesions look like scattered grains of salt on the skin, fins or gill covers. Fish may also flash against objects, produce excess mucus, become weak or inactive, and stop eating. Gill-only disease can be severe without obvious white dots, so appearance alone cannot confirm the diagnosis.

Ich versus fungus and other white marks

Ich spots are usually discrete, round white dots embedded in the skin or fins. Fungal growth more often looks cottony or fuzzy, while mucus, injury, mineral deposits and parasites can create other pale marks. These differences are not reliable enough for treatment decisions: examination of skin, fin and gill samples under a microscope is the confirmation method described by Merck Veterinary Manual and VCA.

Freshwater and marine white spot are different diseases

Freshwater tropical fish are usually affected by I. multifiliis. Marine fish can develop a similar-looking disease caused by Cryptocaryon irritans. The treatments and tolerances are not interchangeable, so identify whether the aquarium is freshwater or marine before choosing a product.

Why one dose rarely works

Ich has three life stages:

  1. Trophont: the feeding parasite under the fish’s skin or in its gills.
  2. Tomont: a protected cyst that leaves the fish and reproduces on the substrate or other surfaces.
  3. Theront: the free-swimming infective stage that must find a live fish.

The trophont and tomont are protected from many chemicals. Treatments mainly kill free-swimming theronts, which is why a single dose can leave an aquarium infected. At about 23°C/74°F, one tomont can produce as many as 1,024 daughter parasites, according to UF/IFAS Extension.

What to do when you suspect white spot

  1. Check the system. Test temperature and water quality, list every fish species, and note whether the aquarium contains scaleless fish, sensitive plants or invertebrates. Read the medication label for those exact inhabitants.
  2. Increase oxygen. Heat and many medications reduce the available oxygen margin. Add vigorous aeration or an air stone, keep the surface moving, and watch fish for gasping.
  3. Treat every connected tank. Fish sharing the same water can carry different life stages even when only one shows spots. Follow a current, species-appropriate label and do not stop when the visible dots disappear.
  4. Remove organic debris. Siphon the substrate and remove waste because tomont cysts can attach to debris. Perform only the water changes required by the product directions so you do not dilute a dose unexpectedly.
  5. Continue on schedule. At 75–79°F, UF/IFAS reports a life cycle of roughly three to six days and describes daily chemical treatments for at least three to five treatments. Cooler water slows development, so the interval must be longer and must follow the product label or veterinary direction.

Treatment choices and their limits

Approach What it reaches Important cautions
Chemical medication Primarily free-swimming theronts; repeated doses expose later waves Use only a label approved or legally marketed for your location and species. Check compatibility with scaleless fish, plants, invertebrates and filtration media.
Aquarium salt Can support treatment of compatible freshwater fish over a prolonged exposure Not suitable for every species, plant or invertebrate. Use aquarium salt rather than table salt; remove it later through repeated water changes.
Heat Speeds the parasite’s life cycle, allowing timed treatments; many strains cannot survive above 86°F Not universal. Some fish cannot tolerate that temperature, and warm water holds less oxygen. Use strong aeration and increase temperature gradually only when species and label guidance allow.
Transfer or hospital tank Separates fish and can reduce environmental contamination when managed correctly Moving fish does not automatically remove cysts from the original system. Equipment must be dedicated or disinfected, and both systems need a coherent treatment plan.

Using aquarium salt safely

UF/IFAS describes 4–5 g/L (4–5 parts per thousand) for a prolonged seven- to 10-day bath for compatible scaled freshwater fish. That is a conditional protocol, not a universal dose. Confirm that every fish, plant and invertebrate can tolerate salt, measure the actual water volume, and follow the product or veterinarian’s instructions. Salt does not evaporate or disappear when water evaporates; replace only the amount removed during water changes, and plan repeated fresh-water changes to remove it after treatment.

API Liquid Super ICK Cure Freshwater and Saltwater Fish Medication 16-Ounce Bottle
  • Contains one (1) API LIQUID SUPER ICK CURE Freshwater and Saltwater Fish Medication 16-Ounce Bottle
  • Treats "ich" white spot disease in fish and secondary bacterial fish infections
  • Quickly kills ich parasite, typically within 24 hours of first dose
  • Corals and invertebrates may be sensitive to this medication. We do not recommend usage in reef aquariums or any freshwater or saltwater aquarium with invertebrates.
  • Use at the first sign of “Ich” (white spots). Repeat dose after 48 hours. See Directions for more details.

Using heat without creating a second emergency

VCA notes that many ich strains cannot survive above 86°F, but also warns that not all fish tolerate that temperature. Raising heat accelerates the parasite’s cycle and can sharply increase oxygen demand. Use a reliable thermometer, raise temperature gradually, add aeration, and stop or lower the temperature if fish show distress. Heat should not replace repeated, species-appropriate treatment when the diagnosis is uncertain or the fish are already compromised.

Medication and regulatory notes

In the United States, the FDA announced on April 8, 2025, that Faunamor was added to the Index of Legally Marketed Unapproved New Animal Drugs for Minor Species as a treatment for ornamental-finfish ich. The listed active ingredients are methylthionine chloride, malachite green oxalate and acriflavine chloride immersion solution. Availability and legal status differ by country; use the current label supplied in your jurisdiction and never combine products unless a veterinarian or label specifically permits it.

When to call an aquatic veterinarian

  • Fish are gasping, breathing rapidly, lying on the bottom or deteriorating quickly.
  • Several fish die suddenly, or gill signs occur without visible skin spots.
  • The aquarium contains species, plants or invertebrates for which salt, heat or the proposed medication may be unsafe.
  • Fish fail to improve after a complete, label-directed treatment course.
  • You cannot distinguish ich from another disease.

A veterinarian can examine skin, fin and gill samples by microscopy and select a treatment that fits the species and system. UF/IFAS describes an untreated ich outbreak as a true emergency; mortality can reach 100%.

Preventing another outbreak

  • Quarantine new fish for at least 30 days, following local veterinary advice for the exact protocol.
  • Quarantine new plants and disinfect nets, decorations, substrate and other shared equipment before moving them between aquariums.
  • Avoid transferring water, wet equipment or untreated animals from a shop or another tank.
  • Maintain stable water quality and reduce crowding and other stressors that make disease more likely to spread.

Because the parasite can persist in protected environmental stages, prevention depends on breaking the route of introduction rather than merely removing visible spots.

The Bottom Line

White spot is usually contagious freshwater ich, and it needs prompt, repeated treatment of the whole connected system. Confirm compatibility before using medication, salt or heat; maintain oxygenation, remove debris, and seek aquatic-veterinary help for gill signs, rapid decline or an uncertain diagnosis.
