Apoquel (Oclacitinib) is a prescription veterinary medicine for dogs that controls itching (pruritus) associated with allergic dermatitis and signs of atopic dermatitis. The FDA-approved canine use is for dogs at least 12 months old; veterinarians prescribe an initial twice-daily regimen for up to 14 days followed by once-daily maintenance, not a self-adjusted dose.
Oclacitinib works by inhibiting Janus kinase signaling involved in itch and inflammation. The medicine can provide rapid symptom control, but scratching is not a diagnosis: fleas, mites, food reactions, infection, ear disease, and other conditions may still require separate investigation and treatment.
Apoquel is not a cure for canine atopic dermatitis. Safe use requires attention to the dog’s age, infection and cancer history, reproductive status, concurrent medicines, and the risk of accidental ingestion of flavored chewable tablets.

Apoquel can reduce scratching, licking, chewing, and other itch-related behavior, but itch is a symptom rather than a diagnosis. Apoquel does not by itself determine whether the trigger is fleas, mites, food, environmental allergens, bacterial or yeast overgrowth, contact dermatitis, autoimmune disease, or endocrine disease.
A veterinarian may use oclacitinib while investigating and treating the cause of the skin problem. A dog that improves on Apoquel can still have an untreated flea infestation, food reaction, ear infection, bacterial infection, yeast infection, or another condition that needs separate treatment.
How does Apoquel work, and how fast does it work?
Apoquel works as a Janus kinase, or JAK, inhibitor. Merck Veterinary Manual explains that oclacitinib inhibits signaling involving itch-associated cytokines such as IL-31 and inflammatory cytokines that depend on JAK1 or JAK3 activity.
This mechanism helps explain why oclacitinib can act quickly against allergic itch. The same immune-signaling effects also explain why infection risk, demodicosis, and other safety considerations matter. Apoquel is not simply an antihistamine, and its immune effects should be considered when a veterinarian reviews a dog’s medical history.

What is the FDA-approved Apoquel dosage schedule?
The FDA-approved Apoquel Chewable regimen is 0.4–0.6 mg/kg, or 0.18–0.27 mg/lb, by mouth twice daily for up to 14 days and then once daily for maintenance. The FDA corrected FOI summary provides the labeled schedule; the prescribing veterinarian’s instructions take priority for an individual dog.

The labeled schedule means that a veterinarian may prescribe daily Apoquel maintenance after the initial twice-daily period. Owners should not extend twice-daily dosing, stop treatment, increase the amount, decrease the amount, or change the schedule without veterinary direction.

What Apoquel strengths and formulations are available?
Apoquel is available in oral tablet formulations, including the Apoquel Chewable product. The FDA’s June 9, 2023 approval summary identifies Apoquel Chewable strengths of 3.6 mg, 5.4 mg, and 16 mg. Tablet strength should not be selected from a general weight chart without a veterinarian’s prescription.
Zoetis owner information says the tablet may be given with or without food. Give the prescribed product and amount as directed, and ask the prescribing clinic what to do if a dose is vomited or missed rather than automatically giving an extra dose.
In an FDA-reviewed client-owned-dog palatability study, the U.S. Food and Drug Administration reported in 2023 that more than 90% of offered chewable doses were consumed within five minutes. Mild drug-related gastrointestinal signs occurred in some dogs and resolved without treatment. Palatability can make administration easier, but it can also make an unattended bottle attractive to pets.
Is Apoquel safe for long-term use?
Apoquel may be used as once-daily maintenance when a veterinarian decides that the benefits outweigh the risks for a particular dog, but there is no universal long-term safety guarantee. The decision depends on age, diagnosis, infection history, reproductive status, cancer history, other medicines, and the dog’s response over time.
The FDA safety update states, APOQUEL modulates the immune system.
That statement is important: a medicine that reduces inflammatory and itch signaling can also affect pathways involved in host defense and hematopoiesis.

The FDA’s 2021 Apoquel labeling update also says that risks and benefits should be considered before treatment in dogs with recurrent serious infections, recurrent demodicosis, or neoplasms.
What are the side effects of Apoquel?
Reported Apoquel adverse events include vomiting, lethargy, reduced appetite, diarrhea, elevated liver enzymes, dermatitis, crusts, pododermatitis, pyoderma, seizures, increased thirst, and demodicosis. These are post-approval reports listed by the FDA in decreasing reporting frequency, not reliable estimates of how often each event occurs.
The same FDA update includes reports of benign, malignant, and unclassified neoplasms, dermal masses, lymphoma, other cancers, and death, including euthanasia. A post-approval report does not establish that Apoquel caused the event, nor does the report count establish the event’s true frequency.

Contact the prescribing veterinarian promptly about persistent vomiting or diarrhea, marked lethargy, loss of appetite, worsening skin or ear disease, new lumps, seizures, or other concerning changes. A serious reaction, breathing difficulty, collapse, or suspected overdose warrants emergency veterinary care.
Do not add a human antihistamine, steroid, supplement, or another allergy medicine to Apoquel without veterinary direction. Different products can have different active ingredients, contraindications, and interactions.
Can I give my dog Apoquel every day?
A dog may receive Apoquel every day during once-daily maintenance if a veterinarian prescribed that plan. The FDA-labeled schedule starts with twice-daily dosing for up to 14 days and then changes to once-daily maintenance; daily use does not mean that owners should continue the initial frequency indefinitely.
Daily treatment should be reviewed in the context of the dog’s response and safety risks. Ongoing itching, recurrent skin infections, ear infections, or a need for repeated dose changes can indicate that the underlying allergy or flare factor needs further investigation.
Can Apoquel be used for a puppy or a cat?
Apoquel’s FDA-approved canine indication is for dogs at least 12 months old, so Apoquel should not be treated as an approved puppy allergy medicine. A veterinarian must evaluate any younger dog rather than an owner adapting the adult label.
The FDA approval described here is for dogs, not a general cat allergy indication. Cats have been involved in accidental Apoquel Chewable ingestion reports, but that does not make Apoquel appropriate for routine use in cats. Never give a cat a dog’s Apoquel prescription unless a veterinarian specifically directs it.
What happens if a dog eats a whole bottle of Apoquel?
Call a veterinarian immediately after a suspected Apoquel overdose, even if the dog looks normal. If the regular veterinarian is unavailable, contact an emergency animal hospital or an animal poison-control center, and do not wait for symptoms to appear.
The pork-liver flavoring in Apoquel Chewable can attract dogs and cats, and the FDA says many reported incidents involved a full or nearly full bottle. Reported overdose signs include vomiting, diarrhea, lethargy, eye redness or swelling, and effects involving the heart, brain, kidneys, or liver; signs usually appear within 24 hours.

As of April 2, 2026, the FDA reported 259 overdose reports in dogs and 64 in cats involving Apoquel Chewable since the chewable was approved in June 2023. These are reports, not a prediction of what will happen after a particular ingestion. The FDA overdose safety communication recommends keeping the medicine in its original packaging and in a secure location inaccessible to pets.
Do not induce vomiting or give a home remedy unless a veterinarian or poison-control professional instructs you to do so. Have the bottle, strength, estimated amount eaten, time of ingestion, and the dog’s weight available when calling.
Does Apoquel cure allergies or only stop the itching?
Apoquel controls allergic itch and inflammatory signs; it does not cure the allergy or remove every trigger. Merck Veterinary Manual states, Atopic dermatitis in dogs cannot be cured.
The condition can usually be managed to improve quality of life.
Diagnosis of canine atopic dermatitis is based on the dog’s signalment, clinical signs, history, and exclusion of other itchy skin diseases rather than on one definitive laboratory test. A flare or incomplete response should prompt a veterinary review of fleas, food or environmental allergens, bacterial infection, yeast infection, and otitis externa.

Long-term management may combine several measures:
- Reliable flea control when flea allergy or flea exposure is possible.
- A veterinarian-supervised diet trial when a food reaction is a consideration.
- Diagnosis and treatment of bacterial pyoderma, Malassezia or yeast dermatitis, and ear disease.
- Bathing to reduce allergen load and remove irritants from the coat and skin.
- Itch control with a medicine selected for the dog’s age, health, diagnosis, and treatment goals.
- Allergen-specific immunotherapy when appropriate.
Merck identifies allergen-specific immunotherapy as the only treatment for canine atopic dermatitis that can change the immune response to allergens and induce remission of clinical signs. Immunotherapy is slow and is not an acute-flare treatment. Merck Veterinary Manual summarizes a 2022 retrospective study in which approximately 60% of 664 dogs treated with subcutaneous allergen-specific immunotherapy for at least nine months had more than a 50% decrease in pruritus.

The best alternative depends on whether the dog needs rapid itch relief, long-term control, treatment of an infection, or investigation of a trigger. No alternative should be substituted or dosed without veterinary guidance.

For dogs with a veterinarian-diagnosed superficial bacterial skin infection, a chlorhexidine dog shampoo may be recommended as part of the treatment plan. The active ingredient, concentration, contact time, and bathing schedule should follow veterinary guidance. Chlorhexidine shampoo does not replace Apoquel when prescription itch control is needed, and it does not replace flea control, antifungal treatment, antibiotics, or investigation of the underlying allergy.
High-quality fish oil containing EPA and DHA may be considered as an adjunct for some dogs, but Merck Veterinary Manual says essential fatty acids are not useful for acute flares, may take up to two months to take effect, and lack evidence that one particular combination, dose, ratio, or formulation is superior. Fish oil should therefore not be presented as a rapid substitute for prescription itch control.

How should owners decide whether Apoquel is appropriate?
Apoquel is most appropriately considered after a veterinarian evaluates the dog’s age, itch pattern, skin and ears, infection status, parasite control, medical history, reproductive status, and current medicines. The following checklist helps identify information to bring to the appointment:
- When did the itching start, and is it seasonal, year-round, or suddenly worse?
- Where does the dog scratch, lick, chew, or lose hair?
- Is there odor, discharge, redness, crusting, moist skin, or recurrent ear disease?
- Is flea prevention current, and have mites or other parasites been considered?
- Has the dog had recurrent infections, demodicosis, seizures, cancer, or other serious illness?
- Is the dog younger than 12 months, pregnant, nursing, or intended for breeding?
- What prescription medicines, injections, supplements, and over-the-counter products is the dog receiving?
Do not use another dog’s Apoquel, estimate a dose from tablet strength, or treat unexplained itching indefinitely without an examination. A short-term reduction in scratching can hide a continuing infection or allergen exposure.
How should Apoquel be stored?
Keep Apoquel in its original packaging in a secure location that pets cannot access. The flavored chewable formulation deserves extra care because its pork-liver flavoring can make the tablets appealing to dogs and cats. Count remaining tablets after a suspected incident and call for veterinary advice immediately rather than waiting for vomiting or other signs.
