A typical male has one urogenital opening; a typical female has two openings, with a separate oviduct opening. Overall color, body shape and behavior can provide supporting clues in mature fish, but they should not replace this opening-by-opening check.
Find the genital papilla
Work on the ventral underside of the fish, just behind the anus. The small structure there is the genital papilla. Avoid trying to sex the fish from its color or from a quick look at the head or body profile.
Handle the tilapia gently, support its body, and keep the inspection brief. Wet hands reduce damage to the protective mucus coating. If the fish is difficult to restrain or is very small, return it to the water and obtain help from an experienced fish handler.
Count the openings
- Locate the papilla. Look immediately behind the anus on the underside.
- Examine the shape. A male papilla generally appears more elongated; a female papilla is generally wider.
- Count the openings. One opening is the typical male pattern. Two openings, including a distinct oviduct opening, are the typical female pattern.
- Use dye only if needed. A drop of methylene blue or food coloring on the genital region can make the openings easier to see. This is an optional aid, not a requirement.

The male opening carries both milt and urine. In females, urine and eggs pass through separate openings. Those anatomical differences are why counting the papilla openings is more dependable than judging body shape.

How size affects confidence
Small tilapia are harder to sex because the papilla and its openings are tiny and visibility depends on handling and lighting. Published guidance gives different practical thresholds, so none should be treated as a universal minimum:
- Mississippi State University Extension warns that fish under 3–4 inches are difficult to distinguish externally.
- Southern Regional Aquaculture Center (SRAC Publication No. 283, March 1999) describes examining a 1-ounce (25-gram) fingerling.
- A Texas A&M-hosted sex-reversal guide says external examination is not consistently reliable until fish weigh about 20 g, even with experience.
- The FAO Small-Scale Aquaponic Food Production manual (2014 edition) describes hand-sexing fish about 40 g or larger.
These figures reflect different fish sizes and viewing conditions. If the openings are not clearly visible, record the result as uncertain rather than forcing a male or female label.

Secondary clues in mature fish
Once tilapia are mature, males may develop a more pronounced head and forehead, a humped back and squarer body features. Females often look sleeker and have smaller heads. Males may also chase rivals or court females.
These observations are supporting evidence only. Individual condition, strain, age and environment can change appearance and behavior, and the available guidance does not establish color as a dependable general-purpose sexing test.

Why sex-reversal history matters
External appearance can diverge from gonadal sex during a sex-reversal program. The Texas A&M-hosted guidance cautions that a fish may develop a male-like papilla before its gonads have changed; an externally male-looking fish can therefore have female gonads in that specific context.
For untreated fish, the papilla check is the practical external method. For sex-reversed fish, breeding studies or production decisions that depend on gonadal sex require confirmation by an experienced specialist and an appropriate method rather than appearance alone.
When to seek confirmation
- The fish is under roughly 3–4 inches or otherwise too small for a clear view.
- You can see the papilla but cannot distinguish one opening from two.
- The result will determine a breeding, stocking or research decision.
- The fish came from a sex-reversal treatment or its production history is unknown.
- Several observers disagree after inspecting the same fish.
In these situations, have an experienced aquaculturist or fish-health professional recheck the fish. A clear photograph of the underside, taken without prolonged handling, can also help a qualified person review an uncertain observation.
