Clinicians use them as wound grafts or matrices for selected burns and other difficult wounds. They are specially processed medical materials—not raw fish skin, a consumer dressing, or a routine transplant of living fish tissue.
What does “compatible” mean in this context?
Here, compatibility means that a prepared fish-skin material can be applied to a human wound as a scaffold, with clinical handling and healing outcomes studied in particular indications. It does not mean that a person’s body receives a whole, living, vascularized piece of fish skin that becomes ordinary human skin.
Preparation matters: processing, disinfection, decellularization, sterilization, and storage affect the material. A 2026 review identifies variability in these steps, possible immunogenicity, and regulatory approval as continuing challenges. Findings about one product or preparation should not be generalized to raw skin or every fish-skin product.
Which fish species have human clinical evidence?

A 2023 systematic review covering 10 studies reported overall improved or quicker healing, fewer dressing changes, less pain, and lower costs for acellular fish skin compared with several alternative dressings. It also noted that there is no single gold standard for wound healing; the review does not establish a universal result for every wound, product, or patient.

What wounds are fish-skin grafts used for?
Studies and clinical use described in the reviews include burns, diabetic foot ulcers, venous leg ulcers, traumatic and surgical wounds, and other chronic or complex wounds. These are clinician-directed adjuncts to wound care, not stand-alone substitutes for diagnosing and treating the cause of a wound.
A 2023 expert panel issued 43 consensus statements for intact fish-skin grafts in acute and chronic lower-extremity wounds. Its primary recommendation is to determine the wound’s cause and establish standard care before adding a fish-skin graft. For example, standard care includes compression for venous ulcers and off-loading for diabetic foot ulcers.

Are fish-skin grafts xenografts, and how are they regulated?
In clinical language, fish-derived grafts may be described as xenografts because their source is a nonhuman animal. The regulatory terminology requires care: the U.S. Food and Drug Administration defines xenotransplantation as a procedure involving implantation or transplantation of live cells, tissues, or organs from a nonhuman animal source into a human recipient. That definition does not by itself determine the regulatory classification of a particular processed, acellular fish-skin product.
The FDA separately says that human cells or tissue intended for implantation, transplantation, infusion, or transfer into a human recipient are regulated as human cell, tissue, and cellular and tissue-based products (HCT/Ps). That statement concerns human-derived material; it should not be taken as a classification of fish-skin grafts. Product-specific regulatory status depends on the particular material and product, and the available evidence here does not establish a single status for every fish-skin graft.

Why not use fresh or edible fish skin?
Clinical grafts are prepared medical materials. Fish bought as food or raw skin has not thereby undergone the product-specific processing, disinfection, sterilization, quality control, and storage needed for clinical use. Do not apply raw fish skin to a wound or try to preserve it at home as a graft.
A 2026 ex-vivo study examined iodine-treated fresh tilapia skin kept at 4 °C on days 0, 1, 3, and 7. It illustrates that disinfection and storage are technical variables; it is not a validated consumer preservation method and does not authorize home wound treatment.

What should guide the clinical choice?
There is no direct head-to-head clinical study in the reviewed evidence showing that cod outperforms tilapia, or vice versa, for efficacy or cost-effectiveness. A clinician should select any graft in the context of the wound and the specific product rather than choosing by fish species alone.
- Confirm the wound’s cause and follow indicated standard care, such as compression or off-loading.
- Consider the evidence for the specific wound type and product, not just the fish species.
- Account for how the material is processed, disinfected or sterilized, and stored.
- Use the graft as part of clinician-directed wound care, with infection control and follow-up appropriate to the wound.
A 2024 case series reported preparation and application of Nile tilapia grafts to four contaminated full-thickness wounds in zoological patients, with contraction and epithelialization in all four. Those were animal patients, so the report is translational evidence—not proof of efficacy in human patients.
