What is a candiru?
“Candiru” is a common name used for small freshwater catfishes in the family Trichomycteridae, especially Vandellia cirrhosa and related species. The name does not always identify one precise species.
Vandellia cirrhosa enters a host fish’s gill chamber and uses needle-like teeth to make an incision in an artery. The host’s blood pressure then fills the candiru. FishBase reports feeding times of roughly 30 to 145 seconds.
Calling the candiru a “blood-sucking fish” is convenient but slightly misleading. It does not need a conventional suction pump; it feeds from blood flowing out of an injured artery.

Does candiru really swim up urine?
There is no good evidence that candiru are attracted to human urine or routinely swim up a urine stream. The claim has been repeated in travel accounts and retellings, but the best-known experimental evidence does not support it.
In a 2001 study, researchers tested Vandellia cf. plazaii with ammonia, amino acids, fresh fish slime, and human urine. The fish did not respond to those chemical cues and appeared to respond visually to live fish instead. When goldfish and Amazonian cichlids were offered together, the candiru attacked the goldfish more often.
The experiment does not show that every candiru species behaves identically in every river. It does show that the claim that candiru detect and follow human urine is not supported for the fish tested.

Where did the human-invasion story come from?
Historical accounts have often relied on explorer reports, hearsay, or secondhand testimony. Over time, these accounts grew into a dramatic warning that people risk invasion whenever they urinate while swimming in the Amazon.

The most frequently cited modern case occurred in Brazil in 1997. A urologist reported removing a fish from the urethra of a 23-year-old man who said it had entered while he was urinating in a river. The circumstances remain disputed, and a 2021 review described the concern as unsupported by confirmed medical-literature cases.

- The fish was reported to be about 133–134 millimeters long, with a head approximately 11.5 millimeters wide. Those measurements raise questions about how it entered an adult male urethra.
- The case depended heavily on the patient’s account and material provided by the treating physician; it was not an independently observed attack.
- The specimen was reportedly too deteriorated for a completely secure species identification.
- The report did not establish that the fish followed urine, swam upstream through a urine stream, or entered as part of its normal feeding behavior.
The careful conclusion is that a disputed 1997 report describes a fish being removed from a human urethra. It does not objectively confirm that candiru naturally invade human bladders or routinely parasitize people.

Why the story sounds plausible
The legend combines real details into an alarming scenario. Candiru are small fish that parasitize other fish, enter narrow spaces in their hosts’ gills, and feed on blood. Those facts can make a more extreme story sound biologically plausible.

But a fish that enters another fish’s gill chamber is not automatically adapted to locate a human urethra. The host, anatomy, cues, and feeding behavior are different. The documented target is a fish gill, not a human urinary tract.

A bizarre event may be physically imaginable without being a normal behavior, a significant travel risk, or a demonstrated biological pattern. Evidence for candiru’s fish-parasitic lifestyle is much stronger than evidence for the urethra-invasion legend.

There is no reliable evidence that urinating in an Amazonian river predictably attracts candiru into the urethra. Unfamiliar freshwater can pose other hazards, so swimmers should consider currents, water contamination, parasites, sharp objects, wildlife, and local safety advice.
If someone experiences pain, bleeding, difficulty urinating, or suspects a foreign body after swimming, they should leave the water and seek urgent medical care. Trying to remove an object with force or inserting tools can cause additional injury.
