What to do today
- Notice a single mild scoot and no other signs: check for visible stool or litter only if your cat is comfortable, and monitor. Contact your veterinarian if it recurs or other signs appear.
- See repeated scooting, licking, biting, or painful defecation: contact your veterinarian for an examination. These signs do not prove that the anal sacs are the cause.
- See a painful swelling, blood, pus, an open or draining sore, or difficulty defecating: seek prompt veterinary advice.
- Your cat is severely distressed or cannot pass stool: seek urgent veterinary triage.
Keep your cat comfortable and prevent licking with a cone if your veterinarian advises it and your cat tolerates it. A cone may limit licking but does not treat the underlying problem. Do not squeeze, lance, or flush a painful or swollen area at home.
What are a cat’s “anal glands”?
“Anal glands” is the common term for the anal sacs: two small pouches beside the anus. They hold a strong-smelling secretion that normally empties through small ducts during defecation. Anal-sac disease is less common in cats than in dogs, and impaction is the most common form of feline anal-sac disease, according to the Merck Veterinary Manual’s cat-owner guidance and its veterinary reference on anal-sac disease.
What problems can affect the anal sacs?
Anal sacs may become impacted, inflamed or infected, or abscessed. Less commonly, a tumor may affect a sac. A veterinarian needs to examine your cat to determine the cause; signs and discharge alone are not enough to diagnose it.

Signs to watch for

Other problems can produce overlapping signs. The Merck Veterinary Manual recommends veterinary assessment, particularly when an older cat has a lump or persistent signs that could reflect less common disease such as a tumor.
When is a cat anal-sac problem urgent?
Contact a veterinarian promptly for a painful swelling, an open or draining sore, blood or pus, or pain or difficulty when defecating. These signs can indicate an abscess or another condition needing treatment. If your cat appears severely distressed or cannot pass stool, seek urgent veterinary triage. The veterinary guidance does not specify one universal number of hours for every case, so contact your clinic for case-specific advice.
Do not squeeze, lance, or flush a painful or swollen sac at home. Treatment can be painful and may require sedation. A cone may help prevent licking if advised by your veterinarian, but it does not treat the gland problem.
Can you express a cat’s anal glands at home?
Do not try to express a painful or swollen sac at home. Diagnosis and treatment should follow veterinary examination, and some cats need sedation for safe treatment. Ask your veterinarian what to do if your cat has recurring signs rather than relying on general expression instructions.

What happens at the veterinary visit?
A veterinarian will examine the anal area and may perform a digital rectal examination; the sacs may be expressed as part of that examination. If infection or a tumor is suspected, additional tests may be needed. Depending on the findings, these can include microscopy, ultrasound, or biopsy, as described in the Merck Veterinary Manual veterinary reference.
Treatment depends on the diagnosis

Treatment varies with examination findings. Merck and VCA Animal Hospitals’ guidance on anal-sac disease in cats describe approaches including expression, flushing, medication, abscess drainage, and surgery where indicated.
Why can anal-sac problems recur?
Recurrence is not common in cats, but it can happen. VCA lists obesity, chronic diarrhea or constipation, inflammatory bowel disease, allergies, and abnormal anatomy or neurologic function among conditions associated with recurring disease. A veterinarian can assess whether an underlying problem may be contributing and discuss a management plan.
Depending on your cat’s history and findings, ask whether evaluation for gastrointestinal, skin, weight, anatomical, or neurologic factors is appropriate. A veterinarian may discuss added dietary fiber or other management, but do not start supplements or change treatment based only on a general article.

When is surgery considered?
Anal-sac removal may be considered when medical treatment fails, problems recur, or a tumor is present. Surgery is delicate because nerves involved in bowel control run near the sacs. Fecal incontinence can occur after nerve damage; VCA describes it as often temporary but rarely permanent. Ask the surgeon to explain the expected benefit, risks, and whether referral is appropriate for your cat.

Why is my cat scooting?
Scooting can fit anal-sac discomfort, but it can also result from other rectal, perianal, or skin problems. Repeated scooting needs veterinary assessment to identify the cause.
Why is my cat licking its bottom?
Licking or biting near the anus can occur with anal-sac problems, but it is not specific to them. Persistent licking or grooming that causes hair loss warrants an examination.

What does a ruptured anal gland look like?
An abscess may drain through a sore or opening beside the anus, sometimes with bloody or pus-like discharge. A draining wound needs prompt veterinary assessment; do not assume it has healed because it is less swollen.
Can I express my cat’s anal glands at home?
Do not squeeze a painful or swollen sac at home. Have your veterinarian examine the cat and advise on appropriate treatment, especially if signs recur.
