Sudden bloody diarrhea in a dog—especially with vomiting, weakness, or collapse—can be an emergency. Contact a veterinarian immediately rather than treating it like an ordinary stomach upset. Dogs with acute hemorrhagic diarrhea can lose fluid rapidly and develop shock before dehydration is obvious.
“Hemorrhagic gastroenteritis” (HGE) is the older term for what veterinary references now generally call acute hemorrhagic diarrhea syndrome (AHDS). AHDS is a clinical syndrome, not a single proven infection. A veterinarian must assess the dog and consider other dangerous causes of bloody diarrhea.
What is HGE, or acute hemorrhagic diarrhea syndrome?
HGE is the familiar owner-facing name for a condition now more often called acute hemorrhagic diarrhea syndrome. The defining pattern is sudden vomiting and profuse bloody diarrhea, often accompanied by poor appetite, lethargy, or abdominal pain.
AHDS is a diagnosis of exclusion. Signs and bloodwork may support the diagnosis, but bloody diarrhea alone does not prove that a dog has AHDS. Intestinal dysbiosis and toxins produced by some strains of Clostridium perfringens, including netF toxins, are possible contributors; the precise cause is not known.

Signs of HGE in dogs
Common signs may include:
- Sudden, profuse bloody diarrhea
- Vomiting, which may happen before diarrhea
- Lethargy, weakness, or reluctance to move
- Loss of appetite
- Abdominal discomfort or pain
- Rapid fluid loss
Severe illness may cause poor circulation or shock, including:
- Collapse or inability to stand
- Marked weakness or worsening responsiveness
- Abnormally fast heart rate or weak pulses
- Pale or otherwise abnormal gums
- Cold extremities or abnormal body temperature
- Reduced urination
Do not rely on the skin-tent test alone. Fluid loss can cause shock before dehydration is readily recognizable.
When bloody diarrhea is an emergency
Call a veterinarian immediately for severe bloody diarrhea, especially when it occurs with vomiting, weakness, or collapse. Other concerning signs include abdominal pain, repeated vomiting or diarrhea, inability to retain fluids, pale or abnormal gums, or reduced responsiveness. VCA Animal Hospitals describes emergencies as requiring immediate veterinary attention.
A puppy that is unvaccinated or incompletely vaccinated needs particular urgency. Canine parvovirus can cause similar signs. Call the clinic before arriving so staff can advise you about infection-control precautions.
While arranging care:
- Follow the veterinary clinic’s instructions about food and water.
- Do not force food or water into a dog that is repeatedly vomiting, very weak, or less responsive.
- Do not give human anti-diarrheal drugs, pain relievers, antibiotics, or electrolyte products unless a veterinarian specifically tells you to.
- Do not delay care while trying to identify a food trigger or waiting for severe bloody diarrhea to resolve.
When you call, report when signs began, how often the dog has vomited or passed stool, vaccination status, possible toxin or medication exposure, access to foreign objects, and known medical conditions.
What causes HGE in dogs?
The exact cause of AHDS remains unclear. Possible associations include intestinal microbiome disruption and toxin-producing bacteria such as some strains of Clostridium perfringens. These are hypotheses or associations, not proof that a particular bacterium, food, or event caused a specific dog’s illness.

Young dogs and small or toy breeds are overrepresented, but any dog with these signs needs appropriate veterinary assessment. Do not assume that a recent treat, table scrap, boarding stay, or stressful event explains away bloody diarrhea.
Conditions that can look like HGE
Veterinarians consider other conditions that can cause vomiting, bloody diarrhea, abdominal pain, or collapse, including:
- Canine parvovirus, especially in puppies and inadequately vaccinated dogs
- Bacterial, viral, or parasitic gastroenteritis
- Pancreatitis
- Foreign body, intestinal obstruction, or intussusception
- Rodenticide toxicity or other causes of abnormal clotting
- Severe gastrointestinal ulceration or perforation
- Neoplasia
- Systemic illness such as hypoadrenocorticism
Blood in the stool does not distinguish these conditions reliably. Some require very different treatment from AHDS.
How veterinarians diagnose AHDS
The veterinarian assesses how sick the dog is and uses the history, examination, and tests as needed. The work-up depends on the dog’s age, vaccination status, severity, findings, possible exposures, and response to initial stabilization.
Physical examination and perfusion assessment
The examination may assess hydration and circulation, heart rate and pulse quality, gum color, body temperature, mental status, abdominal pain or distension, and urine production. This helps determine whether immediate shock treatment and hospital monitoring are needed.
Blood tests
A complete blood count and packed cell volume (PCV, also called hematocrit) are commonly used. A characteristic finding is hemoconcentration: the PCV is often above 60%, while total plasma protein may be normal or slightly decreased. The MSD Veterinary Manual reports this pattern in its 2025 review. It supports AHDS but does not prove it by itself.

Other tests may include blood chemistry, electrolytes, urinalysis, and coagulation testing. Repeated measurements can help assess circulation and ongoing losses.
Fecal and infectious testing
Fecal testing may look for parasites. Parvovirus testing may be especially important in puppies or dogs with incomplete vaccination histories. Test results must be interpreted in context.
Imaging
Abdominal radiographs or ultrasound may be recommended when the history or examination raises concern about a foreign body, obstruction, intussusception, pancreatitis, perforation, or another structural problem.
How HGE is treated in dogs
Prompt intravenous crystalloid fluid therapy is the mainstay of treatment. The veterinarian selects the fluid type and rate based on circulation, dehydration, ongoing losses, and other findings. For many dogs, drinking or routine subcutaneous fluids are not enough to address the fluid requirements.

Veterinary treatment may involve:
- Intravenous fluids and monitoring to restore circulation and replace ongoing losses
- Antiemetic medication when vomiting is present
- Dietary management once the veterinarian determines feeding is appropriate
- Correction of electrolyte or other abnormalities
- Plasma or colloid therapy in selected dogs with shock or marked protein loss
- Additional treatment if testing identifies sepsis, organ dysfunction, or another diagnosis
Treatment is individualized. A stable dog and a dog in shock need different levels of care.
Are antibiotics used for HGE?
Antibiotics are not routine treatment for mild to moderate AHDS. The MSD Veterinary Manual advises against antimicrobials in these cases. Unnecessary antibiotics can cause adverse effects and contribute to resistance.

A veterinarian may consider parenteral antimicrobials in severe disease, such as persistent systemic illness despite fluid treatment, severe neutrophilia, neutropenia, or a degenerative left shift. The decision and treatment must be based on the dog’s examination and test results.
Can HGE be treated at home?
Severe or sudden bloody diarrhea should not be managed at home without veterinary guidance. Home remedies cannot rule out parvovirus, obstruction, pancreatitis, toxin exposure, or shock. Human medicines may be dangerous for dogs, and forcing fluids can cause aspiration.

Discharge instructions are individualized. Once vomiting is controlled and the dog is stable, the veterinarian may recommend appropriate meals, medication, and follow-up examination or testing. Do not substitute a diet, probiotic, or home hydration product for emergency assessment or prescribed treatment.
When to return to the veterinarian
Contact the veterinarian for reassessment if:
- Bloody diarrhea returns or does not improve as expected
- Vomiting resumes
- The dog refuses food or water
- Weakness or lethargy worsens
- The abdomen becomes painful or distended
- Gums become pale or otherwise abnormal
- Urination decreases
- The dog collapses or becomes less responsive
A dog may look better after initial fluids and still need monitoring. Improvement does not rule out another underlying disease or guarantee that fluid, protein, or electrolyte abnormalities have resolved.
Prognosis and possible complications
With appropriate treatment, the prognosis for AHDS is generally good. The MSD Veterinary Manual reports mortality of less than 10% in hospitalized dogs; this figure applies to hospitalized cases and should not minimize the danger of untreated illness.

Possible complications include:
- Hypovolemic shock from severe fluid loss
- Marked loss of blood proteins, or hypoproteinemia
- Sepsis
- Disseminated intravascular coagulation
- Organ dysfunction or death
Prognosis depends on the degree of circulatory compromise, protein loss, systemic illness, concurrent disease, and whether testing identifies another cause.
Does HGE spread to other dogs?
AHDS is not considered contagious, according to the MSD Veterinary Manual. However, some diseases that resemble AHDS—including parvovirus—can spread between dogs. Until a veterinarian has considered infectious causes, keep a sick dog away from other dogs as practical and follow the clinic’s instructions. This is especially important for puppies and incompletely vaccinated dogs.
