A veterinarian evaluates the dog and tests urine to distinguish infection from stones, obstruction, sterile bladder inflammation, and other problems. Straining with little or no urine is an emergency; systemic illness alongside urinary signs needs prompt veterinary advice.
What is a UTI in a dog?
Most infections of the canine urinary system are bacterial. Bacteria commonly ascend through the urethra into the bladder, causing infection and inflammation known as bacterial cystitis. In some cases, infection travels upward to the kidneys, where it is called pyelonephritis and requires more urgent evaluation.
The word cystitis describes bladder inflammation; it does not always mean a bacterial UTI. Stones, tumors or polyps, anatomical abnormalities, and sterile inflammation can cause similar signs. An antibiotic cannot treat every cause of bladder inflammation.
Signs of a possible UTI in dogs
Signs of urinary tract disease may include:
- Asking to go outside more often or urinating repeatedly.
- Painful or difficult urination, straining, or crying while urinating.
- Passing small amounts of urine at a time.
- Urinating indoors or in unusual places after being house-trained.
- Blood in the urine, sometimes more noticeable toward the end of the stream.
- Repeatedly licking the urinary opening or appearing uncomfortable around urination.
Some dogs have bacteria in their urine with few or no obvious signs, particularly when another disease increases their risk. Conversely, dogs can have convincing urinary signs without a bacterial infection. Symptoms alone cannot distinguish a UTI from stones, obstruction, sterile inflammation, or other urinary problems.

When urinary symptoms are an emergency
Repeated straining with little or no urine, inability to urinate, or a suddenly distended and painful abdomen may indicate urethral obstruction. This is a urinary emergency and should not be treated as a routine UTI symptom.
Seek emergency veterinary care immediately if your dog:
- Strains repeatedly and produces no urine or only a few drops, or cannot urinate.
- Has a suddenly swollen or painful abdomen.
- Is vomiting, markedly lethargic or weak, collapses, or worsens rapidly.
Fever, poor appetite, vomiting, or marked lethargy alongside urinary changes also warrants prompt veterinary advice because a kidney infection or other serious illness may be involved. Arrange a prompt appointment for blood in the urine, new frequent urination, accidents, or straining while urine is still passing.
How veterinarians diagnose a suspected UTI
The veterinarian may ask about drinking, urination frequency and volume, urine appearance, behavior, medications, and previous disease. A physical examination and urine testing help identify the cause. Depending on the case, the veterinarian may also recommend blood tests or imaging.

Urinalysis and urine-sediment examination can show blood, inflammatory cells, crystals, changes in urine concentration, or bacteria-like material. Findings may also point toward stones, kidney disease, diabetes, or another cause. A dipstick or home test alone does not confirm a bacterial UTI.

Urine culture and susceptibility testing
Quantitative aerobic urine culture is the gold standard for identifying bacterial cystitis. Culture can identify the organism and provide susceptibility results to help the veterinarian select an effective antibiotic when one is needed. Merck Veterinary Manual’s 2025 clinical chapter says growth of at least 1,000 CFU/mL from a cystocentesis sample is usually considered clinically relevant bacteriuria; interpretation depends on how the sample was collected and the dog’s signs, so this is not a home diagnostic threshold.
Culture is especially useful in recurrent or complicated cases, when treatment has failed, or when resistance is a concern. Bacteria in urine without urinary signs do not automatically mean antibiotics are needed; the veterinarian considers the whole clinical picture.
Other tests your dog may need
- Blood tests: to evaluate kidney function, diabetes, or systemic illness.
- Radiographs or ultrasound: to look for stones, obstruction, masses, anatomical abnormalities, or upper urinary tract disease.
- Advanced testing: such as cystoscopy in selected recurrent, unexplained, or anatomically complicated cases.
- Prostate evaluation: particularly for an older uncastrated male with recurrent urinary signs.
Urine sample collection
Ask the clinic before collecting a sample. If it requests a free-catch sample, collect fresh urine in a clean container, note the collection time, and follow the clinic’s storage and transport instructions. If asked to store it, keep it refrigerated and tell the clinic when it was collected. A home-collected sample may be contaminated and is not necessarily suitable for culture.
For bacterial culture, cystocentesis—collection of urine directly from the bladder by a veterinarian—is the preferred method. The clinic may collect a new sample even if you brought one from home. Never attempt cystocentesis or urinary catheterization at home, and do not delay emergency care to collect a sample.
How UTIs in dogs are treated
Treatment depends on what is causing the signs and whether disease affects the bladder, kidneys, prostate, or another part of the urinary tract. The veterinarian considers examination findings, urine tests and culture when indicated, recurrence, concurrent disease, and possible stones or obstruction.

Bacterial cystitis
Merck Veterinary Manual’s 2025 clinical guidance recommends a relatively short course, usually 3–5 days, for sporadic bacterial cystitis. It identifies amoxicillin or trimethoprim-sulfonamide as reasonable empirical choices in many settings. These are clinician-level recommendations, not a prescription: the veterinarian selects the drug, dose, and duration for the individual dog.
That guidance does not automatically apply to kidney infection, prostatitis, stones, resistant bacteria, recurrent infection, or significant underlying disease. Those cases may need a different plan and follow-up testing.
Kidney infection
Pyelonephritis is more serious than an infection limited to the bladder. Signs can include lethargy, poor appetite, vomiting, and other systemic illness; some chronic cases may be subtle. Evaluation and treatment may involve urine and blood testing, imaging, fluids, medication, and monitoring. A dog that appears systemically ill should not be managed as though it has a simple bladder infection.
Obstruction, stones, and underlying disease
Antibiotics alone will not resolve a mechanical obstruction or every type of stone. Treatment depends on the cause and may include relieving an obstruction, managing stones, veterinary-prescribed pain control, and investigating why the problem developed. A veterinarian-directed urinary dissolution diet applies only to selected confirmed stone cases; it is not a routine diet for every dog with urinary signs.
When a predisposing condition is found, the plan may also need to address diabetes, kidney disease, Cushing disease, incomplete bladder emptying, immune suppression, anatomical abnormalities, or prostate disease.

Why you should not treat a dog’s UTI at home
- Leftover or human antibiotics: the problem may not be bacterial, and the medication may be ineffective or inappropriate.
- Changing or stopping prescribed treatment: follow the veterinarian’s directions and contact the clinic about missed doses or side effects.
- Cranberry products and urinary supplements: they are not substitutes for diagnosis and prescribed treatment.
- Extra water or “flushing”: fresh water is supportive but does not remove an obstruction or reliably cure infection.
- Urinary diets: they are not interchangeable or a general treatment for undiagnosed urinary signs.
- Human pain relievers: do not give them unless a veterinarian specifically directs you to do so.
Recurrent UTIs: why repeated antibiotics are not enough
Repeated bacterial cystitis should prompt evaluation for an underlying cause rather than repeated empirical antibiotic courses. A relapse may mean the original infection was not eliminated; a reinfection is a new infection later. Culture findings and episode timing help the veterinarian investigate.

This is a clinical classification, not a reason to wait for a particular number of episodes before seeking veterinary advice.
Possible contributors include stones, urine retention or incomplete emptying, anatomical problems, incontinence, kidney or prostate infection, immune suppression, and underlying disease. The veterinarian may recommend culture or other follow-up testing based on the case.
Risk factors for urinary infections
Female dogs are generally more prone to bladder infection. Older uncastrated males are prone to bacterial prostate infection. Other risks include diabetes, kidney disease, Cushing disease, dilute urine, impaired bladder emptying, urinary stones, long-term steroid use, and weakened immunity. Risk factors do not prove a dog has a UTI, but they can make careful evaluation especially important.
What to do before the appointment
Write down when the changes began, how often the dog tries to urinate, whether a normal amount comes out, and any blood, accidents, straining, or vocalization. Note changes in thirst, appetite, energy, vomiting, and abdominal comfort, as well as medications, supplements, prior cultures, antibiotics, stones, or relevant diseases.

If the clinic requests a sample, ask how it should be collected and stored. Do not delay emergency care while trying to collect urine.
Can UTIs in dogs be prevented?
Prevention depends on the cause; there is no single guaranteed plan. Keep fresh water available, provide regular opportunities to urinate, follow prescribed treatment, attend recommended rechecks, and investigate recurrent signs rather than reusing old medication. Dogs with underlying disease or repeated infections may need individualized monitoring.
