In neurocysticercosis, larvae of the pork tapeworm Taenia solium form cysts, and scans may show a cyst, inflammation and swelling, or a calcified lesion depending on its stage and location.
Symptoms and scan findings vary, and some cysts cause no symptoms for a time. Diagnosis and treatment require clinical evaluation: inflammation around a cyst can be dangerous, and antiparasitic treatment may temporarily worsen it.
What happens in neurocysticercosis
Neurocysticercosis is caused by the larval form of Taenia solium. After a person swallows the tapeworm’s eggs, the larvae can enter tissues and form cysts, including in the brain and other parts of the central nervous system. Symptoms may not appear until months or years later.
The appearance and effects depend on the cysts’ number, size, location, stage, and the body’s inflammatory response. A viable cyst may cause little reaction. As a cyst degenerates, the immune response can produce inflammation and surrounding edema (swelling). A dead lesion may be resorbed or calcify, sometimes leaving a focus associated with seizures. The striking part is often the tissue reaction and where the lesion is—not a larva visibly burrowing through the brain.

What brain scans may show
There is no single scan appearance for neurocysticercosis. Imaging may show cysts, edema, contrast enhancement around a degenerating cyst, or calcification. Findings depend on the lesion’s stage and location and need to be interpreted alongside a patient’s history and examination.

Neither modality is always better overall. The choice depends on the clinical question. A scan finding alone does not establish the diagnosis, and other conditions can produce overlapping abnormalities.
Symptoms depend on the cysts and inflammation
Some people with cysticercosis have no symptoms at first. When neurocysticercosis causes symptoms, seizures are common; headaches, confusion, inattention, and difficulty with balance can also occur. Depending on location and inflammation, complications may include hydrocephalus, increased pressure inside the skull, chronic meningitis, or cranial nerve abnormalities.

The U.S. Centers for Disease Control and Prevention (CDC) reports that epilepsy is the most common symptom and occurs in 70% to 90% of symptomatic patients in published case series. This figure describes symptomatic patients in those case series, not the general population.

Clinicians combine the patient’s history and examination with imaging and, when appropriate, laboratory tests. No single result should be used in isolation to diagnose or exclude the infection.
- Review symptoms and history. A clinician considers neurological symptoms and possible exposure, including travel and circumstances in which food, water, hands, or surfaces could have been contaminated.
- Choose brain imaging. CT and MRI have different strengths. A clinician selects the imaging needed to assess the suspected lesion and its location.
- Consider antibody testing. CDC identifies enzyme-linked immunoelectrotransfer blot (EITB) as its preferred antibody test among available tests. Serology can be negative in patients with only one or a few cysts, and antibody tests do not distinguish active from inactive infection or track treatment outcomes.
- Assess complications and plan care. Clinicians determine whether seizures, edema, increased intracranial pressure, or hydrocephalus need attention and whether treatment directed at the parasite is appropriate.
Why treatment must be individualized
Treatment depends on the cysts’ number, location, size, and stage, as well as symptoms and complications. Care may first focus on seizures, swelling, increased pressure inside the skull, or hydrocephalus. Antiparasitic drugs may be used for selected symptomatic patients with multiple live cysticerci, while treatment does not benefit dead, calcified cysts. Some ventricular cysts may require surgical removal.

Killing viable cysts can trigger inflammation and temporarily worsen symptoms. The CDC notes that clinicians often use corticosteroids to suppress this response, and some presentations require delaying antiparasitic treatment. The right plan depends on specialist assessment; do not self-treat with human or veterinary dewormers or online protocols.
How people acquire cysticercosis
People develop cysticercosis by swallowing T. solium eggs shed in the feces of a person carrying an intestinal tapeworm. Eggs can contaminate food, water, hands, or surfaces. As the CDC explains, eating pork cannot give a person cysticercosis.
Eating pork containing larval cysts can cause intestinal taeniasis, a different infection. A person with an intestinal tapeworm can then shed eggs and spread them through fecal contamination. The distinction matters: pork is not the direct source of cysticercosis.

Practical prevention
- Wash hands after using the toilet or changing diapers and before handling food.
- Wash and peel fruits and vegetables.
- Avoid food or water that may be contaminated.
- Use safe sanitation and prevent fecal contamination of food, water, hands, and surfaces.
- If neurological symptoms develop, tell a healthcare professional about relevant travel or possible exposure and seek medical advice.
When to seek medical care
Seizures or new or worsening neurological symptoms require medical assessment. A seizure, confusion, difficulty with balance, or severe headache can have many causes; these symptoms do not by themselves prove a parasitic infection. Do not try to diagnose a brain lesion from online images or treat it with a dewormer.

Can a brain scan show a worm moving through the brain?
Not typically. In neurocysticercosis, scans may show cysts, calcification, edema, or changes around a degenerating lesion. These appearances vary by stage and location; they are not a video of a larva crawling through brain tissue.

Can a negative blood test rule out neurocysticercosis?
Not in every case. Serology can be negative when a person has only one or a few cysts. Antibody tests also do not distinguish active from inactive infection, so clinicians interpret results with imaging and the clinical picture.
