Some diabetic cats eventually maintain normal glucose without ongoing insulin, a state called remission. This usually follows veterinary-directed treatment and monitoring; it is not a reason to skip prescribed insulin or stop it without your veterinarian. Remission is not guaranteed, and diabetes can return.
What remission means in a diabetic cat
Remission means that diabetes is controlled without glucose-lowering treatment; it does not mean the disease is cured. Definitions used by veterinary sources are similar but not identical. VCA describes remission as normal glucose for more than four weeks without insulin injections or oral glucose-regulating medication. The FelineVMA toolkit relays the ISFM guideline wording that a cat has likely achieved remission if negative glucosuria and/or euglycaemia are maintained for 2–4 weeks without insulin. Your veterinary team determines how to assess your cat.
Because diabetes may return, a cat in remission still needs follow-up and attention to possible symptoms. Remission is a period of control, not a guarantee that treatment will never be needed again.
Does remission mean a cat can skip insulin?
No. “Without insulin” describes the remission state, not a safe approach to newly diagnosed diabetes. Early insulin and dietary therapy may help bring glucose under control, and Merck Veterinary Manual notes that some newly diagnosed cats enter remission after diet changes and insulin treatment. Do not withhold prescribed insulin or alter its dose, timing, or your cat’s diet without veterinary advice.

Insulin overdose can cause life-threatening hypoglycemia. If your cat eats less than usual, behaves differently, has changed glucose readings, or seems to need a different insulin dose, contact your veterinary team for instructions rather than making an adjustment yourself.
How likely is remission?
Remission is possible, but no reliable general percentage can predict an individual cat’s chance. The FelineVMA toolkit says no single factor consistently predicts remission and characterizes the evidence as moderate to poor. Earlier treatment and prompt glucose control are associated with better chances, but they cannot guarantee remission.

A 2014 systematic review assessed 22 studies of pharmaceutical interventions, diet, diagnostic tests, and patient characteristics as predictors of remission; FelineVMA characterizes the evidence it summarizes as moderate to poor. Different insulin types and protocols have been associated with remission, and reducing dietary carbohydrate might help, but further study is needed. No one insulin, diet, or protocol is established as a guaranteed route.
When remission may happen—and why it can end
Merck Veterinary Manual says remission commonly occurs within three to four months of starting diet changes and insulin treatment in some newly diagnosed cats. VCA says remission may last months or years, with duration varying. These are broad descriptions, not a forecast for an individual cat.

The FelineVMA toolkit FAQ reports that at least 25% of cats that achieve remission subsequently resume insulin dependence. That estimate describes the toolkit’s reported experience, not the odds for a particular cat. A veterinarian can advise what follow-up is appropriate for your cat.

Monitoring is part of diabetes care, and the plan should be tailored to the cat and owner. A home glucose meter may be useful when your veterinarian recommends one, but a reading or device alone does not diagnose remission or determine a safe insulin dose. Results need to be interpreted alongside symptoms and veterinary assessment.

Ask your veterinary team which monitoring approach is appropriate, how to use it consistently, what readings or symptoms warrant a call, and how often follow-up is needed. If glucose readings or your cat’s appetite or behavior change, contact the team for guidance.
