Do Dogs and Cats Need Anesthesia for Dental Care?

Comprehensive dental care for dogs and cats is performed under general anesthesia so veterinarians can examine, image, clean below the gumline, manage pain, and treat disease.

For dogs and cats receiving a comprehensive veterinary dental cleaning or treatment, yes: the procedure is performed under general anesthesia, with an endotracheal tube, appropriate monitoring, and support tailored to the animal. Anesthesia lets the veterinary team examine the whole mouth, take dental X-rays, clean below the gumline, manage pain, and treat disease found. Anesthesia-free tooth scraping is not an equivalent substitute.

What anesthesia makes possible

Dental disease can be hidden below the gumline, where a visual check of exposed teeth cannot reveal it. AAHA’s 2019 pet-owner article says that 60% of each tooth lies below the gumline and explains that dental radiographs can reveal problems such as bone loss, root resorption, and abscesses that are not visible on the crown. The same article states that most pets develop some degree of dental disease by age three; this is AAHA’s published claim, not an individual prediction for every pet.

With the pet under general anesthesia and still, the veterinary team can examine the mouth thoroughly, take intraoral radiographs, probe the gums, and clean tooth surfaces both above and below the gumline. Below-gumline work can be uncomfortable, so anesthesia also makes pain control and treatment possible. An endotracheal tube helps protect the airway from fluid and debris during the procedure.

AAHA’s dental-care guideline says nonanesthetic dentistry is not appropriate because of patient stress, injury and aspiration risks, as well as limited diagnostic capability. Visible-surface cleaning alone is cosmetic and ineffective for treating periodontal disease. The guideline also notes that peer-reviewed evidence on the safety and efficacy of nonanesthetic dentistry is very limited. The American Veterinary Dental College (AVDC) calls the procedure “anesthesia-free tooth scraping” and says it does not replace comprehensive dental procedures under anesthesia or effectively prevent disease. Oral, inflammatory, or endodontic disease can remain undetected without a complete examination and radiographs.

How veterinarians assess anesthesia risk

Anesthesia is not risk-free, and risk differs from patient to patient. AAHA characterizes risk as low for healthy or minimally compromised patients when anesthesia is performed by appropriately trained people. That qualification matters: the veterinarian should consider the pet’s medical history, physical examination, prior anesthetic responses, and any laboratory tests or imaging indicated by the findings. Abnormal results may call for additional evaluation or a delay.

Older age or a chronic condition does not automatically rule out a medically important dental procedure. AAHA advises against excluding indicated procedures solely because of chronological age or most comorbidities. Older or medically complex pets may need a more individualized protocol, and a veterinarian can consider input from a board-certified veterinary dentist or veterinary anesthesiologist if the case warrants it.

Dental nerve blocks can provide analgesia and reduce the depth of general anesthesia needed. AAHA notes that this can help support blood pressure, reduce ventilatory depression, and generally increase safety. The appropriate plan depends on the animal and the procedure.

  • What did the examination and medical history show, including any prior anesthesia reactions?
  • Why are blood tests or other evaluations recommended for this pet?
  • Who will monitor anesthesia, and which measurements will be followed?
  • Will the pet have an endotracheal tube and an IV catheter?
  • How will pain be managed, including whether dental nerve blocks are appropriate?
  • What findings could lead to an extraction or other treatment, and how will decisions be handled?
  • How will recovery be supervised, and what care will the pet need at home?

AAHA’s pet-owner guidance lists heart rate and rhythm, pulse quality, mucous-membrane color, oxygen saturation, temperature, blood pressure, expired carbon dioxide, and respiratory rate among the measures monitored during dental anesthesia. It also describes supervised recovery. Ask the clinic how its protocol applies to your pet rather than assuming every patient or practice has an identical plan.

Home dental care helps, but does not replace treatment

The AVDC recommends daily home dental care from an early age, using brushing or another effective plaque-control method, such as suitable dental diets or chew materials. Regular veterinary examinations help identify when professional care is needed; scaling under anesthesia is recommended when indicated. The Veterinary Oral Health Council (VOHC) provides information about oral-hygiene products with demonstrated effectiveness.

A pet toothbrush and pet-safe toothpaste can support daily care. They cannot diagnose or treat disease below the gumline, take dental radiographs, or replace a veterinary dental procedure when one is needed.
