Cyproheptadine for Cats: Dosage, Safety & Side Effects

Cyproheptadine may be prescribed extra-label to stimulate appetite in cats. Learn what veterinary references say about dosing, safety and side effects—and why your veterinarian must set the plan.

Cyproheptadine is an antihistamine with serotonin-blocking effects that veterinarians sometimes prescribe extra-label to stimulate appetite in cats. Feline use is not approved on the drug’s human label, and your veterinarian must choose your cat’s dose and schedule. Published veterinary references report different intervals, so their ranges are context—not instructions for dosing your cat.

A poor appetite can signal an illness that needs diagnosis and treatment. An appetite stimulant does not address the cause, and it is not a substitute for nutritional support when a cat is seriously ill.

What is cyproheptadine used for in cats?

Cyproheptadine is an antihistamine that also blocks serotonin activity. Veterinarians may use it extra-label as an appetite stimulant in cats. “Extra-label” means a veterinarian prescribes a medicine for a use or species beyond those on its human label when they judge it clinically appropriate. The International Society of Feline Medicine (ISFM) notes that cyproheptadine is not approved for cats and describes its historical use and efficacy as anecdotal. Reduced appetite is a sign, not a diagnosis. The cause needs veterinary evaluation; in feline hepatic lipidosis, for example, treatment must address the underlying reason the cat stopped eating. Appetite stimulants may not be effective in severe disease, and Merck cautions that they should not be the primary way to provide nutrition to critically ill animals. What dose do veterinary references report?

Published references describe doses by cat, but they do not establish the right prescription for an individual animal. They also differ on dosing interval:

Do not choose a dose or interval from this table. The veterinarian’s plan depends on your cat’s medical status and the prescribed formulation. Follow the prescription label, and ask the veterinarian before changing, repeating, or stopping a dose. What side effects should I watch for?

Sedation is a major known adverse effect and may limit use in hospitalized cats. Some cats can instead show agitation, excitement, or aggressive behavior. Other possible effects reported by PetMD include dry mouth, increased heart rate, and difficulty urinating; a particular reaction is not inevitable. 2022 ISFM consensus guidelines; Merck Veterinary Manual; PetMD.

Cyproheptadine has seizure-related cautions: Merck says it may lower the seizure threshold, worsen seizures, or reduce the effectiveness of anticonvulsant medicines. Tell the veterinarian if your cat has a seizure history or takes seizure medication.

Health conditions and medicines to flag

  • Hepatic lipidosis: Cyproheptadine should be avoided in cats with this condition. The ISFM guideline cites concern about liver failure, while the MSD Veterinary Manual warns that affected cats may not metabolize appetite stimulants normally or may accumulate harmful metabolites.   • Kidney disease: Some cyproheptadine is eliminated through the kidneys, but the ISFM guideline says dose adjustments for cats with kidney disease have not been studied. The veterinarian must assess whether and how to use it.
  • Mirtazapine: Do not combine it with cyproheptadine unless the veterinarian specifically directs a change. ISFM notes that cyproheptadine is used as an antidote for mirtazapine toxicity, so the medicines should not be used concurrently.

Give the veterinarian a complete list of your cat’s medicines and supplements before treatment or any change in appetite medication.

How should I give a prescribed dose?

Use the exact formulation and instructions on your cat’s prescription label. PetMD says a prescribed dose may be given with or without food; giving it with food may help reduce digestive upset. If the label and general guidance differ, ask the prescribing veterinarian or pharmacist which directions apply to your cat. PetMD.

When should I call the veterinarian?

  • Missed dose: Contact the veterinarian for instructions. Do not give an extra or double dose to make up for it.
  • Severe side effects or worsening condition: Contact the veterinarian promptly, especially if your cat’s appetite or overall health is deteriorating.
  • Possible overdose: Seek immediate veterinary attention. Overdose can cause neurologic or cardiovascular effects; do not wait for symptoms to become severe.
