The 2024 AAHA Community Care Guidelines offer small-animal veterinary teams a flexible framework for improving access by considering both the pet’s clinical needs and the family’s circumstances. The approach combines appropriate care options, respectful conversations, and coordination with local organizations; it is guidance, not a single protocol or a solution to every structural barrier.

What the AAHA Community Care Guidelines are

Published in 2024, the guidelines were developed to help veterinary practices increase access to care in their clinics and communities. They describe community care as mobilizing resources across a community, rather than expecting each clinic to address access challenges alone. The framework rests on four principles: recognizing the urgency of access to care, collaborating within community networks, providing family-centered health care, and redefining the gold standard of care. (AAHA guideline launch; journal abstract)

AAHA developed the guidelines with an expert task force and formal peer review. They are an initial framework for local problem-solving, not an exhaustive answer to access-to-care challenges.

How the framework can shape a veterinary visit

Begin with the pet and family

Ask about the pet’s needs alongside the family’s goals, concerns, and constraints. A respectful conversation makes it easier to identify what is feasible without assuming why a family has delayed care or declined a recommendation. AAHA’s family-centered care guidance includes preventive care, contextualized care, coordination, education and support, community engagement, and relationships built on trust and collaboration. If cost may affect a decision, raise it early enough to inform the discussion. Families may need time to consider options, so clear written or digital information and follow-up can help them make an informed choice.

Discuss clinically appropriate options

Spectrum of care means considering a range of evidence-based options suited to the patient’s likely outcomes and the family’s realities. It does not mean lowering care indiscriminately, nor does it assume that the most expensive or technologically advanced option is automatically the right choice for every patient.

Explain the likely benefits, limitations, risks, and trade-offs of appropriate options. Consider patient safety and expected outcome, available clinical evidence, the family’s goals and constraints, access barriers, and the resources of the practice. AAHA’s course identifies tiered spectrum-of-care recommendations as a learning objective. (AAHA Community Care Guidelines course)

Look beyond price

Financial constraints are only one possible obstacle. Transportation gaps, language barriers, mobility limitations, poverty, discrimination, and other social determinants can also affect whether a family can obtain or continue care. Ask what is getting in the way rather than assuming the answer. (AAHA health-equity material)

Where possible, identify practical supports: accessible appointment arrangements, language assistance, educational resources, or a referral to a local organization. The availability and eligibility of these supports vary by community.

Build a local community-care network

Community care connects services and expertise that already exist. Depending on the area, useful partners may include animal shelters, nonprofit and government providers, spay/neuter programs, animal-welfare organizations, and other veterinary practices. A network can help clinics and families find options that one practice cannot provide on its own. (AAHA guideline launch; journal abstract)

  • Map nearby organizations and the services, eligibility rules, and referral processes they publish.
  • Agree on how to share relevant information and coordinate follow-up, while respecting client privacy and each organization’s policies.
  • Keep referral details current and revisit partnerships as community needs and available services change.

AAHA presents community care as an ongoing process. Its resource center includes community-care tools and materials on inclusion, mental health, and community partners. (AAHA resource center)

Use the guidelines as guidance, not a rigid standard

AAHA explicitly states that the guidelines are not an AAHA standard of care and do not dictate an exclusive protocol, treatment course, or procedure. A suitable approach can vary with the individual patient, available resources, and the limitations of a practice setting. The journal abstract likewise describes the guidelines as an initial framework rather than an exhaustive solution. (AAHA health-equity material; journal abstract)

That flexibility does not remove the need for sound clinical judgment or informed discussion. Nor can a family-centered approach by itself eliminate poverty, transportation gaps, discrimination, or other systemic barriers. Financing and assistance programs may be worth discussing where available, but families should check eligibility and terms directly.
