Cat Hyperthyroidism: Symptoms, Diagnosis, and Treatment Options

Weight loss despite a strong appetite can signal feline hyperthyroidism. Learn how vets diagnose it and compare the main treatment options.

Weight loss despite a strong appetite is a common warning sign of hyperthyroidism in cats, but it is not enough to diagnose the condition. A veterinarian can confirm it with an examination and thyroid testing, then help choose among medication, radioiodine, surgery, or a prescription diet. Each option has different practical demands, and thyroid treatment can make previously less apparent kidney disease easier to detect.

What are the signs of hyperthyroidism in cats?

Hyperthyroidism occurs when an overactive thyroid produces too much thyroid hormone. Common signs include weight loss despite increased appetite, increased thirst and urination, vomiting, diarrhea, and unusual activity or restlessness. A cat’s coat may become unkempt, matted, or greasy. Signs vary, and other illnesses can cause similar changes, so symptoms alone cannot establish the diagnosis.

If you notice these changes, arrange a veterinary assessment rather than trying to diagnose or treat the problem at home. Cornell’s guide to hyperthyroidism in cats describes the signs and diagnostic approach.

How do veterinarians diagnose it?

The veterinarian may examine your cat, feel the neck for an enlarged thyroid gland, check heart rate and blood pressure, and order blood chemistry, urinalysis, and thyroid hormone tests. Most affected cats have an elevated total T4, a thyroid hormone measured in blood, but a result within the reference interval does not always rule out hyperthyroidism when signs strongly suggest it.

Merck Veterinary Manual estimates that approximately 5%–10% of affected cats may have a normal total T4 result, according to its review dated May 2024. If suspicion remains, the veterinarian may recommend additional testing, such as free T4 or repeat testing, interpreted alongside the examination and any other illness. What are the treatment options?

The four main approaches are radioiodine, antithyroid medication, surgery, and an iodine-restricted therapeutic diet. The right choice depends on your cat’s overall health and other conditions, access to treatment facilities, and whether you can give medication or maintain a strictly exclusive diet. Discuss the options with your veterinarian before starting, changing, or stopping treatment.

Radioiodine

Radioactive iodine (I-131) is considered the treatment of choice in the American Animal Hospital Association’s 2023 guidance, which reports a cure in more than 95% of cases. It avoids surgery, ongoing medication, and a restrictive diet, but is available only through appropriately licensed facilities, which may limit access. Treatment-induced hypothyroidism is a risk, so follow-up is important. Cornell University Hospital for Animals reports an approximate 95%–98% cure rate for its own service; that is an institutional figure, not a guarantee for an individual cat or a universal outcome.

Methimazole medication

Methimazole reduces thyroid hormone production and can control the disease, but it is not a cure and usually requires ongoing treatment. Oral forms are available, and transdermal formulations are used in some settings. AAHA recommends starting at a low dose and adjusting it under veterinary supervision; a veterinarian must determine the appropriate plan for an individual cat.

Possible adverse effects include vomiting, reduced appetite, blood-cell abnormalities, lethargy, and skin reactions. Periodic blood tests help the veterinarian assess thyroid response, kidney status, and potential adverse effects. AAHA’s 2023 feline hyperthyroidism therapy guidance discusses this approach and its monitoring needs.

Surgery

Thyroidectomy can cure many cats long term or permanently, but it requires general anesthesia. Cardiac or kidney conditions, which may be more relevant in older cats, can affect anesthesia risk. The nearby parathyroid glands help regulate calcium; damage to them during surgery can disrupt that regulation. Ectopic thyroid tissue can also complicate the procedure. The veterinarian should assess and stabilize your cat before surgery is considered.

Iodine-restricted therapeutic diet

An iodine-restricted prescription diet may control hyperthyroidism in some cats. Cornell names Hill’s Prescription Diet y/d as an example. The diet must be the cat’s exclusive food: treats, other pet food, human food, and prey can undermine the restriction. Some cats take months to normalize, and some do not normalize on the diet, according to AAHA’s therapy guidance.

Ask your veterinarian whether this diet fits your cat’s other nutritional needs and your household routine. Exclusivity can be difficult in a multi-cat home or for a cat that goes outdoors and may eat prey.

Why do kidney checks matter during treatment?

Excess thyroid activity can make reduced kidney function less apparent. When thyroid hormone levels return toward normal, kidney disease that was previously harder to detect may become evident. A normal kidney marker before treatment does not guarantee that kidney disease will not show up afterward.

For that reason, treatment planning and follow-up should consider both thyroid control and kidney function. The veterinarian can use the results to guide ongoing care and assess whether hypothyroidism has developed after treatment.
