Persistent or recurring bad breath in a cat deserves a veterinary check. Periodontal disease is the most common cause described by veterinary sources, but odor alone cannot identify the problem: dental disease, something lodged in the mouth, an injury, or an illness elsewhere in the body can all contribute. A brief smell after a strongly scented meal may pass; an odor that keeps returning should not be ignored.

What causes bad breath in cats?

A healthy cat’s breath should not be offensive, according to Cornell Feline Health Center veterinary expert Eric Davis, DVM. The most common problem associated with halitosis is periodontal disease. Plaque builds up on teeth and can harden into tartar, inflaming the gums and contributing to more advanced disease. Cornell reports that studies have found some form of dental disease in 50–90% of cats older than four years; this is an estimate of dental disease in that age group, not the share of bad breath cases caused by dental problems.

Other possible causes include:

  • Food, hair, or string caught between teeth or under the gums, where it can decay and infect nearby tissue.
  • Mouth ulcers, sores, trauma, or oral tumors.
  • Systemic illness, including diabetes, kidney disease, or liver disease.

Cornell notes that sweet or fruity breath can accompany diabetes, ammonia- or urine-like breath can accompany kidney disease, and foul breath with vomiting may occur with liver disease or an intestinal blockage. These smells are clues, not diagnostic tests. Odor cannot establish which condition a cat has; a veterinarian needs to assess the cat and any other symptoms. Cornell’s guide to bad breath discusses these possible associations.

What symptoms should you watch for?

Look for changes in the mouth and in how your cat eats or behaves. Signs can include:

  • Red, swollen, or bleeding gums.
  • Drooling, pawing at the mouth, or apparent discomfort around the face.
  • Difficulty or reluctance eating, turning the head while chewing, or reduced appetite.
  • Loose or missing teeth.

Cats often conceal discomfort, so a cat that still eats—or shows no obvious mouth symptoms—may still have dental disease. Persistent or recurring bad breath warrants a veterinary appointment. Contact a veterinarian promptly if odor comes with difficulty eating, reduced appetite, drooling, mouth bleeding, visibly inflamed gums, suspected trauma, or a possible object lodged in the mouth. If sweet breath occurs alongside increased thirst or urination or weight loss, contact a veterinarian about possible diabetes rather than trying to diagnose from smell.

How does a veterinarian diagnose the cause?

The veterinarian evaluates the mouth and the cat’s general health to determine whether the odor is dental, caused by another oral problem, or associated with systemic illness. A brief look while a cat is awake cannot reliably show what is happening below the gumline. Cornell University Hospital for Animals says definitive dental diagnosis requires general anesthesia, dental probing and charting, and full-mouth radiographs. These allow the veterinary team to assess and record disease that an awake examination may not reveal. Cornell’s dentistry and oral surgery service describes this approach.

What treatment may help?

Treatment depends on the diagnosis. For dental disease, a professional dental procedure under anesthesia may include scaling and polishing above and below the gumline. Depending on the findings, the veterinarian may recommend further periodontal treatment or extracting diseased teeth. If the odor stems from a lodged object, injury, ulcer, tumor, or systemic illness, care is directed at that problem instead.

Antibiotics are not a universal treatment for bad breath or gingivitis. Cornell notes there is little or no evidence that antibiotics alone effectively treat gingivitis; when gingivitis is connected to a systemic or infectious disease, that underlying condition also needs attention. Cornell’s overview of feline dental disease explains the role of veterinary treatment.

Can you prevent dental problems at home?

Regular brushing helps disrupt plaque at the tooth–gum margin. Cornell recommends brushing ideally every day, using toothpaste or tooth gel made for cats. Human toothpaste is not appropriate. Introduce mouth handling gradually, keep sessions calm, and stop if your cat becomes distressed. Do not force brushing, and ask a veterinarian before brushing a cat with severe gingivitis because it may be painful.

Brushing is preventive care, not a substitute for an examination when breath persistently smells bad or the mouth may hurt. If your cat will not tolerate brushing, ask your veterinarian about alternatives and whether they suit your cat. VCA describes the Veterinary Oral Health Council’s role in accepting products shown to be safe and to reduce plaque or tartar; check the current feline accepted-product list before choosing a dental diet, treat, gel, or water additive. VCA’s article on halitosis in cats provides additional context on oral care.
