Fat Metabolism in Cats: How It Works and What Changes in Fatty Liver Disease

Cats normally use dietary fat for energy and essential nutrients. Hepatic lipidosis is a different process that can follow prolonged appetite loss, when mobilized body fat overwhelms the liver’s capacity to process it.

Cats use dietary fat as an energy source and to obtain essential fatty acids and fat-soluble vitamins. In a cat that is eating adequately, the liver helps process and distribute that fat. Hepatic lipidosis—often called fatty liver disease—is different: it commonly develops after a cat eats very little or stops eating, when mobilized body fat can reach the liver faster than it can be handled. Dietary fat is not, by itself, the cause of the disease.

How does fat metabolism work in a healthy cat?

Fat is part of normal feline nutrition, not simply excess energy to be avoided. Cats use it for energy and need it to obtain essential fatty acids and absorb fat-soluble vitamins. The liver contributes to this process by producing bile, which helps the body absorb fat, and by processing nutrients, synthesizing some fats, and managing energy stores.

Sharon Center, DVM, of Cornell University College of Veterinary Medicine describes the normal process this way: “In a healthy, properly eating cat, consumed fat is efficiently broken down in the liver and exported throughout the body to supply the nutrients required for all of an animal’s life processes.” Cornell’s general feeding guidance describes cats’ broad dietary pattern as high in protein, moderate in fat, and low in carbohydrate; commercial foods are formulated with nutritional requirements in mind.

What happens to fat during hepatic lipidosis?

Hepatic lipidosis is a syndrome in which triglycerides accumulate in liver cells (hepatocytes). It often accompanies or follows a period of near-total food avoidance. When a cat takes in too little food, fat stored elsewhere in the body is mobilized and sent toward the liver. If the influx outpaces the liver’s ability to use or redistribute the fat, triglycerides build up in liver cells.

Merck Veterinary Manual characterizes feline hepatic lipidosis as multifactorial: peripheral fat mobilization can exceed the liver’s capacity to redistribute fat or use it through beta-oxidation. A 2017 review by Valtolina and colleagues likewise describes an imbalance among fatty-acid influx, synthesis, oxidation, and triglyceride dispersal. These accounts explain the broad mechanism; they do not establish one single molecular cause for every case.

Why does a cat stop eating—and why does that matter?

Loss of appetite can be a sign of another health problem, and hepatic lipidosis may develop secondarily rather than as an isolated consequence of a cat’s usual diet. Cornell reports that more than 90 percent of hepatic lipidosis cases are secondary to another underlying condition, attributing the estimate to Sharon Center, DVM. Cornell’s page does not identify the original study year, so this is best understood as a Cornell-reported clinical estimate, not a newly measured statistic.

The sequence matters: inadequate intake can trigger fat mobilization, while the illness that caused the appetite loss may also need attention. A cat that refuses food while losing weight should be assessed by a veterinarian; waiting for appetite to return or trying a diet change alone can delay care.

Does dietary fat cause fatty liver, or should it be restricted?

No general rule says that dietary fat causes hepatic lipidosis or that every cat with liver disease should eat a low-fat diet. Merck Veterinary Manual’s nutrition guidance says most animals with hepatobiliary disease do not need dietary fat restriction, because fat provides essential fatty acids and fat-soluble vitamins. It notes exceptions where reduced bile entry into the intestine or impaired fat assimilation changes what the animal can use. Those exceptions call for veterinary guidance, not a one-size-fits-all restriction.

For a cat without a diagnosed condition, choose food based on whether it is nutritionally complete for the cat’s life stage, its nutrient profile, food format and moisture, and how it fits the cat’s weight or medical plan. Cornell advises involving a veterinarian to determine ideal weight. Neither a particular food format nor a commercial product is established here as treatment for hepatic lipidosis.

What should an owner do if a cat is eating very little?

Contact a veterinarian promptly if a cat stops eating, eats markedly less than usual, or is losing weight. Suspected hepatic lipidosis is not a do-it-yourself diet or supplement problem. Veterinary management involves addressing underlying and metabolic problems and restoring adequate nutrition; the appropriate plan depends on the individual cat and clinical assessment.

A 2003 study of 24 adult cats found that carnitine improved fatty-acid oxidation during weight loss under the study conditions, but did not correct hepatic lipidosis in cats fed a diet low in n-3 fatty acids. That limited experimental finding is not evidence that owners should give carnitine or use it to treat fatty liver disease.
