Sometimes, but only when a veterinarian picks the drug and the dose. Antihistamines are used in veterinary medicine, yet a human over-the-counter (OTC) allergy pill is not automatically safe for a dog or cat. The right choice depends on the species, the specific ingredient, your pet’s health, and everything else listed on the package. The Merck Veterinary Manual’s guidance for pet owners is to discuss the risks of OTC medications with your veterinarian before you intentionally give them to your pet.
Why “just an antihistamine” is not a safe instruction
Antihistamines block some effects of histamine, which is why they can ease itching and hives. The Merck Veterinary Manual notes that responses vary considerably between animals and that several trials may be needed to find one that works. So even when a vet approves a drug, benefit is not guaranteed, and a pill that helped a neighbor’s dog may do nothing for yours.
Veterinary references list a number of antihistamines, including diphenhydramine, chlorpheniramine, cetirizine and fexofenadine. Their use regimens differ between dogs and cats and from drug to drug. Those tables are written for clinicians. They are not a dose you can copy for your own animal, and this article does not give one.
What can go wrong
Side effects at ordinary use
First-generation antihistamines can cause sedation or stomach upset. A drowsy pet that is also on other sedating medicine, or one with a health condition, is a reason to ask your vet first.

Overdose
Too much of an antihistamine can cause central nervous system hyperexcitability and may be fatal. Merck’s poisoning guidance lists these possible signs:
- Depression or lethargy, or the opposite: hyperactivity and agitation
- Drooling
- Rapid breathing or a fast heartbeat
- Vomiting or other gastrointestinal upset
- Lack of coordination
- Tremors
- Fever
- Seizures
Combination products
The label’s “allergy,” “cold” or “sinus” name tells you little. Many products pair an antihistamine with a decongestant such as pseudoephedrine, or with a pain reliever. Merck says pseudoephedrine combinations can lead to more serious signs. Read the full active ingredient list, and treat any multi-ingredient product as off limits unless a veterinarian or poison service says otherwise.

If you want to use one on purpose
- Call your veterinarian before buying or giving anything. Describe the symptoms, since itching, vomiting or facial swelling may need a different approach.
- Ask which specific ingredient and formulation suit your pet, and have them confirm the dose, frequency and how long to continue.
- Mention your pet’s other medicines, supplements and health conditions.
- Check the package against the vet’s instructions. The product must contain only the ingredient they named.
- Ask what to watch for and when to call back if your pet does not improve.
If your pet already swallowed an allergy pill
Call a veterinarian or an animal poison service promptly. Do not wait for symptoms if the amount might be significant. Merck stresses that toxicity depends on the dose and on how long ago the pet swallowed the product. Keep the package in hand and be ready to give:
- Product name and strength, including every active ingredient
- Estimated amount swallowed
- When it happened
- Your pet’s species and weight
- Any symptoms you have noticed
When it is an emergency
Difficulty breathing, collapse, or signs of a severe allergic reaction need emergency veterinary care immediately. Merck describes epinephrine as the initial treatment for anaphylaxis in animals, and an antihistamine at home is not a substitute. Do not spend time trying one first.
