Follow the discharge instructions from your dog’s operating veterinary team first. Pyometra is a potentially life-threatening infection, and its surgery and recovery can be more involved than a routine spay. Keep your dog quiet, protect the incision, give only prescribed medicines as directed, and contact the clinic promptly if she worsens or the incision looks abnormal.
What to expect after pyometra surgery
Pyometra is a serious infection of the uterus. The usual definitive treatment is surgery to remove the uterus and ovaries, but a dog’s needs afterward depend on how ill she was and whether complications occurred. Some dogs need intensive care and monitoring for problems such as sepsis, dehydration, shock, or anemia. Your veterinary team’s plan—not a routine-spay timetable—should guide your dog’s recovery. What to do on the first day home
Use the clinic’s discharge sheet for food, water, medicines, and activity; instructions can differ depending on your dog’s condition and procedure. In general, keep her indoors, warm, quiet, and supervised, and let her rest. If you are unsure how to give a medicine, your dog cannot keep it down, or her recovery seems to be going backward, call the operating clinic rather than changing the plan yourself. Keep activity controlled
Movement that stretches or jolts the abdomen can interfere with healing. General canine surgery guidance commonly recommends restricted activity for 7–14 days; the surgeon may prescribe a different or longer period after pyometra surgery. Take short, slow leash walks for toileting only, and avoid running, jumping, rough play, and off-leash exercise. Limit stairs where possible. A crate or small quiet room can help if your dog is comfortable there and can be safely confined. VCA’s incision-care guidance gives the general interval; follow your surgeon if the individual plan differs.

Protect and check the incision
Look at the incision regularly without pulling at it. A clean incision with its edges together may show mild redness or bruising early in healing; worsening changes are not expected. Keep the site dry, and do not bathe your dog until the clinic says it is safe. Do not apply ointments, disinfectants, hydrogen peroxide, or alcohol unless the veterinarian specifically instructs you to.
Prevent licking and chewing, which can damage the incision, remove sutures, and introduce infection. An Elizabethan collar (recovery cone) or a recovery suit may help, but the option must fit safely and actually keep your dog from reaching the abdominal incision. Give medicines exactly as prescribed
Your dog may go home with pain medicine, antibiotics, or other oral medicines. VCA’s pyometra guidance says antibiotics are usually continued for two weeks after surgery, but the prescription for your dog is the one to follow. Read the label and give each medicine as directed. Do not stop or change a course, add supplements, or give human pain medicine without veterinary advice. If you cannot administer a dose or have a concern about a medicine, call the clinic. VCA’s pyometra guidance describes the typical antibiotic course.
Know when to call the veterinary clinic
Contact the operating clinic promptly if you see continuous bleeding or fluid seepage, seepage lasting more than 24 hours, excessive redness or swelling, foul odor, discharge, or missing sutures. Also call if your dog’s condition changes or suddenly worsens. Do not wait for a scheduled recheck if she is deteriorating.

Because pyometra can cause serious illness, seek prompt veterinary advice for new weakness, collapse, fever, vomiting, marked lethargy, poor appetite, pale gums, or a painful or distended abdomen. The need for a recheck or suture removal depends on how the incision was closed. General VCA guidance says visible non-dissolving sutures or staples are often removed 10–14 days after surgery; buried absorbable sutures may not need removal. The surgeon will set your dog’s appointment and confirm when normal activity can resume. Do not remove sutures yourself. VCA’s incision-care instructions.
