Every diabetic dog needs insulin. The American Animal Hospital Association’s 2026 Diabetes Management Guidelines for Dogs state, “All diabetic dogs are insulin deficient and require exogenous insulin therapy,” and Section 4 on insulin treatment (published August 31, 2026) recommends starting insulin on the day of diagnosis. Which product your dog receives, the dose, and how it is timed are decisions your veterinarian makes for your individual dog. This article explains the options the section describes, what it says about meals, pens and syringes, and how it handles storage, so you can follow the conversation at your dog’s appointments.
Why insulin starts on the day of diagnosis
A dog with diabetes mellitus does not make enough of its own insulin, so the guideline treats injected insulin as the foundation of treatment rather than something to add later. For owners, the practical question is not whether insulin will be needed, but which type, dose and schedule suit your dog.
Intermediate-acting (bolus) insulin
The section groups insulins by how they act. Intermediate-acting products are given in relation to meals, and they are the first-choice starting point described by task-force members.
First-choice options and starting dose
Two products are described as good first-choice options: U-40 porcine insulin zinc suspension, sold as Vetsulin, and U-100 neutral protamine Hagedorn (NPH). The starting range is 0.25 to 0.5 units per kilogram (U/kg) every 12 hours. The section reports that most dogs are well controlled at an average of 0.5 U/kg every 12 hours, with a range of 0.2 to 1.0 U/kg every 12 hours across dogs. Those figures describe the population of dogs the guideline discusses. They are not a target for your dog.

How the dose is calculated
The dose is based on your dog’s estimated ideal body weight, not its current weight, and is rounded down to the nearest whole unit. Your veterinarian performs this calculation and writes the dose on your dog’s plan. Do not recalculate it from the ranges above.
If your dog’s diet is changing at the same time it starts insulin, the section advises judicious initial dosing, because diet composition and food intake both affect how a dog responds.
Meals, timing and the reduced-meal rule
Bolus insulin is given after the dog eats a full meal. The section sets out a reduced-meal rule: if less than 50% of the meal is eaten, give half the normal dose and offer no additional food or treats until the next scheduled mealtime. This is a dosing instruction tied to one dog’s plan, so ask your veterinarian to put in writing what to do on a day your dog eats poorly, and follow that written plan rather than this summary.

Timing is also part of the plan:
- Keep feeding and injection times consistent from day to day.
- The section allows a two-hour window on either side of the normal time.
- Occasional missed or off-schedule doses can be acceptable, according to the section, but ask your veterinarian what to do when one happens.
Basal insulin: degludec and glargine U-300
The section describes insulin degludec and glargine U-300 as “peakless” formulations with a long duration of action. They may permit once-daily dosing and may be associated with less day-to-day variation in glucose levels. Degludec may be considered a first-choice option. Some dogs, however, need dosing every 12 hours rather than once a day.

Degludec
In a recent study cited by AAHA (Mott et al., 2025), about 84% of dogs were maintained on once-daily degludec. Task-force members describe somewhat lower success in their own practice experience.
Glargine U-300
In a study cited by AAHA (Tardo et al., 2024), 60% of dogs maintained control on once-daily glargine U-300.

Timing, monitoring and high readings after meals
Basal insulin does not need to be timed with meals, which may suit dogs with inconsistent appetites. AAHA recommends continuous glucose monitoring (CGM) when starting basal insulin, because a dog may need dosing every 12 hours or every 24 hours, and monitoring helps determine the right dose and frequency. For occasional high glucose after meals, the section describes smaller, more frequent meals or an added low dose of intermediate-acting insulin, as part of the treatment plan.

Concentration, syringes and pens are not interchangeable
The strength on the label (U-40, U-100, U-200 or U-300) determines what a “unit” means. A dose is only accurate when the syringe or pen is designed for that exact concentration, so never switch syringes, pens or insulin strengths on your own. The section marks the human insulin products in its table as not approved for veterinary use, which is one reason the prescribing veterinarian should name the exact product. Before buying supplies, confirm the product, concentration and device with your veterinarian.
Storage, in-use periods and when to replace insulin
The section emphasizes careful refrigeration and handling. The manufacturer-recommended in-use duration for most insulin types is 28 to 56 days, as summarized by AAHA.

Beyond the labeled in-use period
The task force is comfortable with use beyond manufacturer in-use durations when insulin is refrigerated and handled carefully. The section explicitly calls for caution, discussion with the client, documentation, and replacement when there are concerns. It cites a study (Berg et al., 2023) in which refrigerated, preservative-containing insulin vials in use for six months were not shown to have increased bacterial contamination. That finding does not establish that the insulin still works as intended, or that every product is suitable after its labeled period. This is the task force’s position, not blanket permission to extend storage times.
The section recommends replacing insulin when:
- it is discolored or has clumping or flakes, or its consistency has changed;
- there are concerns about glycemic control or possible contamination;
- you doubt the insulin is working, or your dog’s clinical signs return.
Questions to bring to your veterinarian
- Which product, concentration and device are you prescribing, and why is it the best fit for my dog?
- Is this a meal-timed (bolus) or basal product, and how often will it be given?
- How was my dog’s dose calculated from its ideal body weight?
- What is my written plan for a day my dog eats poorly?
- Does my dog need CGM to set the dose and frequency?
- How long can this insulin stay in use once opened, and which signs mean I should replace it?
- If my dog’s diet changes, how should the dose be adjusted?
