In cats, “heart disease” most often means cardiomyopathy, a disorder of the heart muscle that makes the walls abnormally thick, thin, or scarred. Many affected cats look normal in the early stages. Signs usually appear only once the heart can no longer keep up or a blood clot forms. Diagnosis depends on an echocardiogram (heart ultrasound), and treatment is shaped by what that scan shows, especially whether the left atrium is enlarged and whether congestive heart failure is present.
What “heart disease” means in cats
Cardiomyopathy is the most common group of acquired heart disorders in adult cats. Its changes can interfere with the heart filling with blood or pumping it out, and can lead to congestive heart failure, blood clots, or sudden death. The term covers more than one condition, so the type of cardiomyopathy matters for both treatment and outlook. Cornell University’s Feline Health Center describes these diseases in its cardiomyopathy guidance, last updated March 2024.
Hypertrophic cardiomyopathy (HCM)
HCM is the most common feline cardiomyopathy. It involves thickening of the muscle of the left ventricle. Cornell cites an estimate that HCM affects up to 15% of the feline population. That figure describes cats as a group, not the risk for any one cat. Some breeds are more predisposed, but Cornell notes that no breed is immune.
Restrictive cardiomyopathy (RCM)
In RCM, scar tissue limits the heart’s ability to relax and fill between beats. The walls may look stiff rather than enlarged, and the impaired filling can cause congestion even when the heart still pumps reasonably well.

Dilated cardiomyopathy (DCM)
DCM involves a dilated heart that contracts poorly. Because the three types differ in structure and prognosis, a diagnosis of one type should not be assumed to carry the outcome of another.
Causes and contributing conditions
For many cats, the underlying cause of cardiomyopathy is unknown. Some cases are presumed to have a genetic basis, and some are associated with inherited risk or another disease. Other conditions can cause or add to heart changes, most notably hyperthyroidism and high blood pressure (hypertension). Finding and managing one of these is part of the diagnostic work-up, not an afterthought.

Symptoms to watch for
Early cardiomyopathy often produces no obvious signs. When symptoms do appear, they tend to be nonspecific: a cat may breathe faster than usual, tire easily, or eat less. The table below separates the signs that need immediate action from those that still warrant a prompt call to your veterinarian.

Cornell cardiologist John Bonagura, DVM, a professor of veterinary clinical sciences at the Ohio State University Veterinary Hospital, puts the threshold plainly in Cornell’s diagnosis article: “The animal can’t breathe or it can’t walk,” he says. “If it is struggling to take in air or if it can’t move its rear legs, you should seek veterinary help right away.”

How veterinarians diagnose feline heart disease
Diagnosis starts with the history and a physical examination that includes listening to the heart and lungs. A murmur or irregular rhythm may be detected, but a normal stethoscope exam cannot rule out heart disease. The steps below build on one another, and the veterinarian may not need all of them in every case.

- History and physical exam. The veterinarian asks about breathing, activity, appetite, and any episodes of sudden weakness, then listens to the heart and lungs.
- Chest X-rays. These help assess heart size and whether congestive heart failure is present. Fluid in the lungs does not by itself prove a cardiac cause, because lung disease and other conditions must also be considered.
- ECG (electrocardiogram). This assesses the heart’s electrical rhythm.
- Echocardiogram. This ultrasound shows heart structure and function and is the gold standard for diagnosing feline cardiomyopathy.
- Blood tests and secondary-cause screening. NT-proBNP can be used as a screening aid, particularly when an echocardiogram is not immediately available. An elevated result can point to atrial enlargement, but it does not establish a definitive cardiomyopathy diagnosis. Veterinarians also check for hypertension and hyperthyroidism.

Treatment is tailored to the diagnosis and the cat’s severity. The two findings that most often shape the plan are whether the left atrium is enlarged and whether congestive heart failure is present. Merck’s treatment guidance lists the broader goals as minimizing heart muscle damage, controlling fluid accumulation, improving circulation, regulating rhythm, maintaining oxygenation, and reducing blood clot risk.

Anti-clotting medication for enlarged atria
For cats with enlarged atria, the veterinarian may prescribe an anti-clotting medication to reduce the risk of clots.
Support for congestive heart failure
When heart failure is present, treatment can include oxygen, diuretics to reduce fluid accumulation, and other cardiac medications. Which drugs are used, and at what doses, is an individual decision made by the veterinarian.

Some cats do not need medication in the early stages and may stay stable for years. Treatment decisions are also guided by any underlying condition, such as hyperthyroidism or hypertension, which may need management in its own right.
Can cats live with cardiomyopathy?
Yes, some cats live for years with cardiomyopathy, but outcomes vary. They depend on the type of cardiomyopathy and on whether heart failure or a blood clot (thromboembolism) develops. Because the disease can progress in different ways, no single survival figure applies to every cat. An individual outlook needs case-specific assessment from a veterinarian who has examined the cat and reviewed its test results.
