Dystocia, or difficult birth, can threaten both the queen and her kittens, and only a veterinary examination can tell whether labor is progressing normally or has stalled for a reason that needs surgery. Do not give labor drugs at home.
What dystocia means in a queen
The Merck Veterinary Manual defines dystocia as a difficult birth, meaning the fetus cannot be expelled through the birth canal without assistance (Merck Veterinary Manual, Dystocia in Small Animals). The problem can start with the mother, with the kittens, or with both. Two distinctions matter most to the veterinarian: whether the uterus is contracting weakly or too infrequently (uterine inertia), and whether something physically blocks the passage (obstruction).
Warning signs that mean call the vet now
Owner-facing Merck guidance lists the following as reasons to seek veterinary help during feline labor (Merck Veterinary Manual, Reproductive Disorders of Female Cats):
- Strong contractions lasting more than 1 to 2 hours without a kitten being born.
- A resting period between labor stages lasting more than 4 to 6 hours.
- Obvious pain or illness, such as crying, or repeatedly licking or biting at the vulva.
- Abnormal vulvar discharge before any kittens have been born, such as fresh blood or dark green discharge.
- No delivery within 24 hours after the queen’s rectal temperature falls below 100°F.
- A history of dystocia in this queen.
These are triggers for contacting a veterinarian, not a diagnosis. A separate Merck owner overview uses different markers: labor in stage I lasting more than 24 hours, or more than 1 to 4 hours between kittens in stage II. It also lists stillborn or near-death kittens and excessive maternal distress as concerning features (Merck Veterinary Manual, Management of Reproduction of Cats). Because the thresholds differ between references, do not wait for one specific timer to pass. If anything on either list is happening, call.

What to do while you arrange veterinary care
- Write down when strong contractions started, when each kitten was born, and the time of any long pause. Your veterinarian will ask for this.
- Call your veterinarian or nearest emergency clinic immediately. Say that the queen is in labor and describe what you are seeing.
- Do not give oxytocin, calcium, or any other labor medication. Those drugs can be harmful when the cause is an obstruction, and their use depends on an examination.
- Do not pull on a kitten that is partly visible. Blind traction can injure the queen or the kitten.
- Keep the queen warm and quiet, and have a secure carrier ready so she can be transported safely.
Causes: maternal, fetal, or both
Maternal causes
Maternal factors include uterine inertia and abnormalities of the pelvis or soft tissues (Merck Veterinary Manual, Dystocia in Small Animals). Secondary uterine inertia, in which uterine function declines after one or more kittens have been born, is described as a common cause in queens (Merck Veterinary Manual, Labor, Delivery, and Postpartum Care in Bitches and Queens). A stall after a kitten or two is therefore not automatically normal “interrupted labor.” Only an examination can show whether the pause is expected for this cat or a complication.
Fetal causes
Fetal factors include a kitten that is oversized, presented or positioned abnormally, or developmentally abnormal. Obstruction can come from any of these, so a kitten that is too large or stuck in the wrong position may need more than uterine stimulation.

Acute, life-threatening complications
Uterine torsion (twisting) and uterine rupture are acute possibilities that need emergency assessment (Merck Veterinary Manual, Dystocia in Small Animals). Signs such as collapse, severe pain, or a rapidly worsening queen call for immediate transport.
How veterinarians diagnose the cause
History and physical examination come first. The veterinarian then assesses the abdomen and performs an aseptic vaginal examination to look for obstruction and to feel the presentation of the kitten. Laboratory evaluation may follow. Abdominal radiographs show how many fetuses are present, where they are, and whether the maternal pelvis is adequate. Ultrasound assesses fetal viability and distress (Merck Veterinary Manual, Dystocia in Small Animals).

Fetal heart rate is one of the key measurements. The Merck professional reference gives 200 to 250 beats per minute as a normal range, and describes rates below 200 beats per minute as concerning for a compromised fetus in its general small-animal discussion. Those figures come from a clinical reference, and only a veterinarian monitoring the individual kitten can interpret them in context. Uterine contractility may also be monitored when inertia is suspected (Merck Veterinary Manual, Labor, Delivery, and Postpartum Care in Bitches and Queens).

Treatment depends on the cause and on the condition of the queen and kittens. The veterinarian uses one of two broad routes: medical management, or cesarean delivery.

The Merck professional reference gives clinical dosing for these drugs. Those details belong to a veterinarian, who chooses the drug, route, dose, and monitoring. Surgery is not a sign of failure. It is often the safest route when the kitten cannot pass or when either patient is at risk (Merck Veterinary Manual, Dystocia in Small Animals).
