Aseptic Surgical Patient Prep: Back to the Basics for Pet Owners

Pre-operative skin antisepsis lowers microorganisms at the incision site and is one layer of surgical-site infection prevention. Here is what the human guidance says and what to ask your vet.

Skin antisepsis before surgery is a short, specific step: the surgical team treats the skin around the planned incision so fewer microorganisms are present when the skin is opened. It lowers the microbial load at the surgical site, and it is one layer of surgical-site infection (SSI) prevention, not the whole of it. The guidance below comes from human surgical practice, the most detailed published source available for this topic. Veterinary teams set their own protocols for animal patients, so use this as background for asking informed questions, not as instructions for your pet’s care.

What skin preparation is meant to do

Before an incision, the skin carries two kinds of microorganisms: transient organisms picked up from the environment and resident organisms that live in the skin. Preparation aims to reduce both around the intended incision and a wider area, so that fewer organisms are available to enter the wound once the skin barrier is cut. AORN’s 2024 summary of its patient skin antisepsis guideline describes this as the goal of preoperative prep, and the World Health Organization describes surgical-site preparation the same way: treatment of intact skin in the operating room, covering the incision site and a broader area around it.

Why it is one layer, not the whole plan

An SSI is an infection in the part of the body where surgery took place. Preventing one depends on several measures working together, and skin prep is only one of them. The U.S. Centers for Disease Control and Prevention (CDC) describes SSI prevention as a bundle of practices, and its current SSI prevention guideline page refers to the 2017 guideline, reviewed April 12, 2024. A well-prepared skin site does not make other steps unnecessary, and it cannot compensate for a poorly managed procedure.

How the antiseptic is chosen

The guidance does not name one product for every operation. It sets out principles that the team applies to each patient and site. AORN’s 2024 summary advises selecting an alcohol-based surgical skin antiseptic unless it is contraindicated. Alcohol acts quickly. Chlorhexidine gluconate (CHG) or an iodophor such as povidone-iodine, combined with alcohol, can add persistent activity after the initial application. The WHO evidence review describes both CHG and iodophors in alcohol as widely used, but it also states that the best approach remained unclear in the evidence it reviewed, so neither should be treated as universally superior.

The factors the team weighs

  • Procedure and site: the same agent may not suit every area of the body.
  • Proximity to eyes, ears or mucous membranes: these areas call for particular caution with any antiseptic.
  • Allergies and contraindications: a reaction history changes the choice.
  • Skin integrity and age: damaged or very young or very old skin can react differently.
  • Drying time and fire risk: alcohol-based products must dry fully before drapes are placed or electrosurgical tools are used.

Why the same antiseptic does not work the same everywhere

Skin microbes vary from one part of the body to another, and so do skin’s responses to antiseptics. As Karen deKay, MSN, RN, CNOR, CIC, lead author of AORN’s Guideline for Preoperative Patient Skin Antisepsis, is quoted in AORN’s article:

“Skin microbiota varies depending on body site location. Consequently, each body part may have different responses to the same skin antiseptic.”

This is why a single antiseptic routine is not a reliable shortcut, and why the team’s choice for one site cannot be transferred to another.

Hair removal and pre-operative bathing

Hair is usually left in place

CDC patient guidance advises leaving hair in place unless removal is clinically indicated. It warns that razor shaving can irritate the skin and make infection easier. If hair must be removed, the method and timing are decided by the surgical team, not by the owner or patient at home.

Bathing is a related step, with limits

AORN notes that bathing with soap or an antiseptic before surgery may reduce skin flora. It also notes that bathing products can cause irritation or an allergic reaction. Bathing is therefore not a universal self-treatment, and any pre-operative wash should follow the clinic’s specific instructions.

What this means when your pet is having surgery

The published guidance is written for people, and veterinary surgery has its own protocols, products and standards. What it offers pet owners is a set of reasonable questions to bring to the clinic:

  • Should I bathe or wash my pet beforehand, and if so, with which product?

Do not apply antiseptics, shave the surgical area or bathe your pet with a medicated product before surgery unless the clinic has told you to do so.

Currency of these sources

The AORN summary is dated December 5, 2024, and the AORN guideline page states it became effective May 13, 2021. The CDC SSI guideline page was reviewed April 12, 2024 and refers to the 2017 guideline. The WHO evidence appendix is older than these and describes evidence rather than a current recommendation. Check the current version of each source before relying on it, and treat any product-specific instructions as overriding general guidance.
