Vestibular disease is not one diagnosis. It is a group of signs that point to a problem in the balance system, and the same signs can come from an ear infection, a growth, a congenital defect, or, less often, inflammation or infection in the brain. A cat with a head tilt, sudden loss of balance, or abnormal eye movements needs a veterinary examination soon, because the outward signs cannot tell you which cause is involved, and treatment and outlook depend entirely on that answer.
What vestibular disease means in cats
The vestibular system is the set of structures that tells the brain where the head is and how the body is moving. It includes the balance organs of the inner ear and the nerves that carry their signals to the brain. When any part of it malfunctions, the cat’s eyes, head posture, and coordination all fall out of step, which is why affected cats look disoriented even though their strength and awareness are usually intact.
Veterinary references describe two broad locations. Peripheral vestibular disease involves the middle or inner ear or the nerves leaving them. Central vestibular disease involves the brain. The distinction matters because it changes which tests are needed and how serious the situation may be.
Signs to recognize
The signs commonly described in cats include:
- Head tilt, often to one side
- Loss of balance, leaning, or circling
- Falling or rolling
- Abnormal, rapid eye movements (nystagmus)
- Uncoordinated, wobbly movement when walking
Cornell’s Feline Health Center notes that a more serious problem can sometimes underlie these signs, so a cat should not be assumed to have a harmless episode simply because it looks like a familiar balance problem.

When to call the veterinarian right away
Contact your veterinarian promptly if signs begin suddenly, if balance or eye movement changes, or if the cat is not improving rather than gradually settling. Do not start home remedies or leftover medications first. Tell the clinic exactly when the signs started, whether the cat has had recent ear problems, sneezing, or head shaking, and whether any eye or ear discharge is present. Those details help the team decide what to examine first.
Causes of vestibular signs
Otitis media and otitis interna
Infection and inflammation of the middle ear (otitis media) and inner ear (otitis interna) are among the most common causes of peripheral vestibular signs. The Merck Veterinary Manual’s review of otitis media and interna in animals (full review noted December 2025) describes these conditions as ear disease that can affect balance as well as hearing.
Aural polyps
Polyps are benign growths in the ear canal or middle ear. They can press on or irritate balance structures and produce the same outward signs as infection, which is why a physical and ear examination matters before a diagnosis is assumed.

Idiopathic vestibular syndrome
“Idiopathic” means no underlying cause is identified. In idiopathic vestibular syndrome, the signs appear without an ear infection, polyp, or other finding that explains them. This is a diagnosis made after other causes have been considered, not a starting assumption.

Neoplasia
Tumors in the middle ear and nearby structures are a less common but important cause. They are one reason imaging is used when signs persist or when the examination does not explain them.
Congenital disease
Some cats are born with a vestibular abnormality. Congenital signs may be present from early life, although the signs in any single cat should still be evaluated rather than attributed to birth status alone.

Central inflammatory or infectious disease
When the brain is involved, the cause may be inflammatory or infectious. These are the situations in which the signs are less likely to be explained by the ear and more likely to need a neurologic workup. Veterinary references list them among the differential diagnoses precisely because their treatment and urgency differ from ear disease.
What a 2025 referral study shows about the causes
A 2025 retrospective study by Jordina Caldero Carrete and colleagues, published in Veterinary Record (online May 20, 2025), reviewed 196 cats with peripheral vestibular syndrome at four referral hospitals. Every cat underwent CT or MRI as part of its investigation. The diagnoses were:

These figures describe a selected group of cats with peripheral signs who were referred and had advanced imaging. They are not the prevalence of each cause among all cats, and they say nothing about how often central disease occurs in the wider feline population. Read them as a picture of what specialist referral cases looked like in that series, not as a forecast for your cat. The full article is available through PubMed.

How veterinarians work out the cause
The workup usually starts with the history and a physical examination, followed by neurologic and ear examinations. Together these help establish whether the problem is likely peripheral or central and point toward the next step. An otoscope exam can check the ear canal and eardrum for infection, inflammation, or tumors.
Cornell and VCA Animal Hospitals both describe imaging as something used when deeper ear or skull structures need to be assessed. CT or MRI can show the middle and inner ear and the brain in ways a standard ear exam cannot. Whether a cat needs advanced imaging, or a referral to a veterinary neurology or specialty service, depends on the findings and the clinic’s capabilities. Ask your veterinarian what each test is expected to clarify and what result would change the plan.

There is no single treatment for every cat with vestibular signs. The plan follows the diagnosis:

- Note: Be sure to measure the size of the pet when choosing carrier size
- Identified middle- or inner-ear disease (such as infection or polyps) needs cause-directed management from the veterinarian.
- Idiopathic vestibular disease has no specific treatment. Care focuses on supportive needs and symptom control.
- Neoplasia, congenital disease, or central inflammatory or infectious disease each require a plan designed around the specific finding.
Medication choices, dosing, and any further tests should be decided by your veterinarian. Do not give a cat medication intended for people or for another animal.
Prognosis is cause-dependent
Outlook depends on what is causing the signs and what the examination and imaging show. The veterinary references consulted for this article do not establish a single recovery timeline that applies to every cat or every cause, so ask your veterinarian for the expected course for your cat’s specific diagnosis rather than relying on general time frames online.
Keeping an unsteady cat safe at home
Cornell’s Feline Health Center gives the core safety rule directly: “Animals must be kept confined in a safe place where they will not injure themselves.” In practice:
- Pick a quiet room with a non-slip floor, and close off stairs, balconies, and open windows.
- Move furniture that the cat could fall from or bump into, and remove objects that could cause injury.
- Place food, water, and the litter box within easy reach on the same level, so the cat does not need to climb or jump.
- Check on the cat often, especially if it is falling or unable to stand.
- Follow your veterinarian’s instructions on monitoring, fluids, or feeding support, and report any worsening promptly.
Practical accessibility aids
PetMD suggests several home modifications that make daily life easier for a cat that is wobbly: a low-sided litter box that the cat can step into without climbing, a non-slip rubber mat at feeding and resting spots, and shallow, wide bowls for food and water that are easier to reach without leaning. These are accessibility measures. They do not treat the underlying condition and they do not replace an examination.

Where this leaves a cat with vestibular signs
Vestibular disease is a set of signs with several possible causes, from ear infection and polyps to idiopathic cases and, less commonly, neoplasia, congenital disease, or brain inflammation or infection. The right first step is a veterinary examination, because the cause decides the treatment and the outlook. While the diagnosis is being worked out, keep the cat confined to a safe space and make food, water, and the litter box easy to reach.
