Trazodone for Cats: Overview, Dosage & Side Effects

Trazodone is used off-label in cats for situational anxiety such as travel and vet visits. Here is what the dose references say, what the small studies show, and which side effects need prompt veterinary attention.

Trazodone can be used in cats before a stressful event such as a car ride or a veterinary visit, but only on a veterinarian’s plan. It is prescribed off-label for cats, which means the dose, timing, and whether it is appropriate at all have to be set for the individual animal. Veterinary references publish pre-event dose ranges, and those figures are useful for understanding a prescription. They are not a home dosing guide.

What trazodone is used for in cats

Trazodone is a serotonin antagonist and reuptake inhibitor (SARI). In cats it is used off-label, meaning it is not approved for feline behavioral use in every jurisdiction, and veterinarians most often reach for it for situational anxiety: travel in a carrier, handling during an examination, or a clinic visit that reliably upsets a particular cat. It is usually one piece of a broader plan that also reduces the stressors themselves, such as carrier training, shorter waiting-room time, and gentler handling.

The 2022 AAFP/ISFM Cat Friendly Veterinary Interaction Guidelines list pre-visit trazodone as an option to reduce fear and anxiety and to improve how well cats tolerate examination. The same guidelines describe the supporting evidence as less extensive than for many other medications and based on small studies, and they recommend pairing medication, when it is indicated, with measures that lower distress during feline care.

Dosage and timing: what veterinary references publish

Published references give a pre-event range, but none of them is a prescription. The table below puts the figures side by side so you can see where they agree and where they differ.

The cat’s weight, health status, other medications, past response to sedatives, and the nature of the planned event all shape the plan. Your veterinarian may choose a dose below the reference range, may decide against trazodone entirely, or may want a trial run at a quiet visit before the day that matters.

Do not give a dose from an article, a human prescription, or a leftover bottle. Do not combine trazodone with another medication, repeat a dose, or change the timing without asking the prescribing veterinarian. The sources reviewed here do not establish a general long-term or repeat-dose schedule for all cats, so any ongoing use is a separate decision that needs its own veterinary review. Ask in advance what to do if the cat spits out a dose, vomits soon after taking it, appears unusually sedated, or if the appointment moves.

What the studies show, and what they do not

The feline evidence is limited. Four studies or appraisals are commonly cited, and each answers a narrower question than the headlines suggest.

The 2016 crossover trial in 10 cats

Stevens and colleagues, writing in the Journal of the American Veterinary Medical Association in 2016, ran a double-blind, placebo-controlled randomized crossover study in 10 healthy client-owned cats aged 2 to 12 years. Each cat had transport or examination anxiety and received a single 50 mg oral dose of trazodone or placebo. Compared with placebo, trazodone reduced transport-related anxiety signs and improved handling scores, and sleepiness was the most common adverse event. The authors concluded that a single pre-visit dose produced fewer signs of anxiety than placebo and was generally well tolerated by most cats.

That result is encouraging for a single, specific use. Ten cats is a very small group, and the trial tested one dose before one kind of visit. It cannot show how well trazodone works for every cat, for chronic anxiety, or for stress that begins at home.

The laboratory pilot in six cats

A separate pilot study gave single 50, 75, and 100 mg doses to six laboratory cats. It detected no adverse effects and no changes in physical examination or laboratory values at those doses. Because six animals is far too few to estimate how often a problem might occur, the study is best read as early safety information for single doses in healthy cats, not as proof of benefit.

The 2023 pharmacokinetic study

A 2023 pharmacokinetic study involved six healthy male cats and compared oral trazodone at 5 mg/kg alone with oral trazodone combined with gabapentin at 10 mg/kg. It adds information about how trazodone concentrations and sedation behave in cats. It does not answer long-term safety questions and does not offer a general dosing plan for the diverse population of pet cats.

The 2025 Veterinary Evidence appraisal

Stephanie Shone’s critical appraisal in Veterinary Evidence, published in 2025, reviewed two of these studies and rated the overall evidence as weak. The 10-cat trial showed promising findings, but the other small study did not show a statistically significant improvement in overall stress scores. The appraisal supports a cautious statement: trazodone may help some cats with a defined, short-term situation, but the evidence is not strong enough to say it reliably relieves feline stress across all clinical settings.

Side effects and safety

Sedation is the effect reported most consistently. The AAFP/ISFM guideline table lists sedation as an expected effect, and the 2016 trial recorded sleepiness as the most common adverse event. Veterinary medication references also describe possible incoordination (ataxia), gastrointestinal signs, and behavioral changes. Responses vary from cat to cat, and small trials cannot estimate how often rare reactions occur.

Interactions with other medicines

Trazodone acts on serotonin. Combining it with other serotonergic drugs can cause serotonin syndrome, which veterinary sources identify as a serious interaction risk. The VCA medication resource also lists monoamine oxidase inhibitors among its precautions. Tell your veterinarian about every prescription drug, over-the-counter product, supplement, and herbal remedy the cat receives, and do not start another medicine without instructions.

The VCA resource also advises caution in certain health conditions. Veterinary review matters most for cats with liver or kidney impairment, heart disease, pregnancy, or other significant illness. A general reference list cannot replace a veterinarian’s assessment of one cat’s history.

Signs that need prompt veterinary attention

Veterinary references describe the following as possible signs of serotonin syndrome or overdose. Contact a veterinarian or an emergency veterinary service promptly if you see any of them:

Act the same way if the cat gets into a human medication or receives more than prescribed. Bring the medication packaging and the time of the dose to the clinic, because that information changes how the veterinarian responds.

Who should be assessed most carefully

Trazodone is not a routine add-on for every cat with a fear of carriers. Your veterinarian will weigh the benefits against the risks in cats with liver or kidney disease, heart disease, significant illness, or pregnancy, and in any cat already taking medicines that affect serotonin or that interact with it. Because sedation is common, cats that must be monitored closely after a dose, or that will be transported alone, need that practical plan discussed in advance.

Non-drug and other options

Merck lists synthetic feline pheromone products as possible standalone or adjunctive calming options. A pheromone diffuser in the carrier or examination room can be a reasonable environmental addition, but it is not proven to be equivalent to trazodone, and it does not replace a veterinary assessment. Many cats do best with a combination of approaches chosen for the situation.

When comparing options with your veterinarian, consider the following:

  • The goal: a single trip or visit, or an ongoing behavior problem
  • How strong the cat-specific evidence is for each option
  • Whether the option must be given before the stressor, and how far ahead
  • Whether you want calming, or whether sedation would be a problem for the cat
  • The cat’s other health conditions and medicines
  • Whether the plan has been tried at a low-stakes visit before the event that matters

Questions to bring to your veterinarian

  • Is trazodone appropriate for this cat, and for which situation?
  • What dose and timing would you use, and how was that chosen for this cat?
  • Should we do a trial run before the appointment?
  • What should I do if the cat spits out the dose, vomits, or becomes very sleepy?
  • Which of the cat’s current medicines or supplements need to be reviewed?
  • What non-drug steps should we combine with the medication?

Trazodone can be a useful tool for a defined, stressful event in some cats. Its benefit is established only in a small amount of evidence, and its safe use depends on a veterinarian setting the dose and timing for the individual animal.
