Zyrtec is a brand name for cetirizine. Giving it to a cat is off label, which means it is a veterinarian-directed decision and not a home remedy. The reference regimen in Merck Veterinary Manual’s dosage table is 1 mg/kg or 5 mg per cat by mouth every 12 to 24 hours as needed. That figure is a veterinary reference point, not a dose to measure out at home. Before giving any dose, confirm the exact product and the dose with your cat’s veterinarian, because the right choice depends on the cat’s health, other medications, and the formulation on the shelf.
What “off label” means for Zyrtec in cats
VCA Animal Hospitals states on its cetirizine page that use in cats and dogs is off label (extra label). In plain terms, the U.S. FDA and Health Canada have not reviewed or approved cetirizine for this veterinary use. Off-label use is common in veterinary medicine, but it shifts responsibility toward the veterinarian’s judgment. VCA’s guidance is to follow the veterinarian’s directions and cautions closely.
Cornell University’s Feline Health Center makes the same general point in its “Common Cat Hazards” guidance: do not give a cat medication without first consulting a veterinarian.
Cat dosage: what the reference value means
Merck Veterinary Manual’s table “Antihistamine Dosages for Integumentary Disease in Animals” lists feline cetirizine at 1 mg/kg or 5 mg per cat, by mouth, every 12 to 24 hours as needed. Merck’s companion review, “Antihistamines for Integumentary Disease in Animals” (last updated June 2026), repeats the same range. Both are professional references written for clinicians, so the range is a starting framework that a vet adjusts for one animal.

Do not derive a dose from the body weight on the label or from a human package. Human products vary in strength and ingredients, and the reference range is not meant to be converted into a measured amount without a veterinarian’s assessment.
What Zyrtec may help with, and how certain the benefit is
VCA says cetirizine is used in cats and dogs to treat or prevent itching associated with atopic dermatitis, hives, and insect-bite reactions. VCA also notes it may be used for respiratory allergy symptoms. These are the situations a veterinarian may consider; they do not establish that the drug will work for a given cat.

Merck’s review is more cautious. It says responses to antihistamines vary considerably, and several trials may be needed to find one that works for a particular animal. It also states that second-generation H1 blockers, including cetirizine, have not proved useful to date for controlling pruritus (itching) in small animals. Owners should expect a trial-and-review process with the vet, not guaranteed relief.
Side effects and cautions
VCA lists the following for cetirizine in pets:
- Common-to-rare effects: vomiting and increased salivation.
- Sleepiness: reported in small dogs and cats at high doses.
- Report promptly: severe or worsening side effects and any unusual signs should go to a veterinarian.
VCA advises against use in pets known to be hypersensitive to cetirizine or hydroxyzine. It recommends caution, and a veterinarian’s assessment first, for cats with:

Medicines that depress the central nervous system need particular caution. VCA names diazepam and alprazolam as examples. Tell the veterinarian about every prescription, supplement, and over-the-counter product the cat receives.
Products to avoid and what to check on the label
The most important product-level warning concerns combination products. VCA states that cetirizine products containing pseudoephedrine should not be used in dogs or cats. Many human cold and allergy combinations include that decongestant, so a box that says “allergy” is not automatically safe for a cat.

Before any purchase or dose, ask the veterinarian to confirm:
- the active ingredients are cetirizine alone, with no pseudoephedrine or other added decongestant
- the formulation (tablet or oral liquid) and the strength per unit
- how the dose will be measured, since the reference range is expressed per kilogram of body weight
Plain, single-ingredient cetirizine is the only form that matches the veterinary reference. Any choice of formulation is for the veterinarian to confirm.

Suspected overdose or adverse reaction
If you think your cat has had too much cetirizine or is reacting badly, act immediately:
- Call your veterinary office right away.
- If the office is closed or unreachable, follow its after-hours instructions or contact the nearest emergency veterinary facility.
- Have the product packaging, active ingredients, your cat’s weight, the approximate amount given, the time of the last dose, and a list of current symptoms ready.
Merck’s pet-owner guidance on poisoning from human over-the-counter drugs (last updated May 2026) lists general antihistamine toxicity signs: depression or hyperactivity, drooling, rapid breathing, rapid heartbeat, gastrointestinal upset, poor coordination, tremors, fever, and seizures. These signs vary with dose and the time since ingestion, and they are not a cetirizine-specific symptom checklist. Do not wait for symptoms to become severe before calling.
