Veterinary references do not treat it as a home remedy for cats. VCA Animal Hospitals describes its use in cats as controversial and calls for extreme caution, if it is used at all. Before giving any loperamide product to a cat, contact your veterinarian. Only a veterinarian who has examined the cat can decide whether the drug is appropriate for that cat and that episode of diarrhea.
What Imodium is
Imodium is a brand name for loperamide, a medicine used to reduce the signs of diarrhea. VCA says oral forms include tablets, capsules, and liquid. The brand name does not make a product suitable for a cat. Human packages are labeled for people, so a cat dose should never be worked out from the tablet strength printed on an over-the-counter box.
Dosage: what the reference tables say and why they are not home instructions
The Merck Veterinary Manual’s general small-animal antidiarrheal table lists loperamide at 0.08 mg/kg by mouth every 6 to 8 hours for dogs and cats (table content last updated September 2024). It is a clinical reference entry written for veterinary professionals. It is not a recommendation for owners to dose at home, and it does not account for the cat’s health, the cause of the diarrhea, or any other medication the cat takes.
The two main veterinary sources differ in emphasis, and the difference matters for a cat owner:

Read together, the sources mean that a dose appearing in a reference table is not evidence that the drug is routine or safe for an individual cat. The decision belongs to the veterinarian.

- Paralytic ileus, a loss of normal intestinal movement
- Toxic megacolon, a dangerous enlargement of the colon
- Pancreatitis, inflammation of the pancreas
- CNS effects, which Merck notes are especially a concern in cats, including excitement
These are reference warnings. They do not predict that a particular cat will have them. They do explain why a cat that becomes constipated, bloated, unusually sleepy, or unusually excited after any antidiarrheal needs prompt veterinary attention.

When loperamide should be avoided
- Infection- or toxin-related diarrhea. VCA says loperamide should not be used for diarrhea caused by infection or toxin.
- Infectious colitis. Merck identifies infectious colitis as a contraindication. Its explanation: slowing gut transit during infectious diarrhea can increase the absorption of bacterial toxins.
Because a cat owner usually cannot tell an infectious cause from a non-infectious one at home, this is one of the main reasons the decision should be made by a veterinarian after an examination.
Diarrhea needs a cause-directed plan
Merck describes diarrhea treatment as potentially involving fluid therapy, electrolyte replacement, maintenance of acid-base balance, and control of discomfort. Depending on the cause, a dietary change or antiparasitic treatment may also matter. Suppressing the symptom is not the same as identifying and treating what is causing it, and a medicine that slows the bowel can hide a problem that still needs treatment.

Interactions and monitoring
VCA advises owners to tell their veterinarian about every medication, vitamin, supplement, and herbal product the cat receives. Its cautions include certain drug substrates and these named medicines:
- Itraconazole
- Ketoconazole
- Sulfamethoxazole
- Trimethoprim
VCA also recommends veterinary monitoring of hydration and electrolyte status while a cat is on the drug.

If a cat has had too much or seems unwell
- Call your veterinary office immediately after a suspected overdose or any adverse reaction.
- Have the product name, the cat’s weight, and a description of the symptoms ready. VCA asks owners to provide these.
- If your clinic cannot be reached, a poison hotline can help. Cornell University’s small-animal toxin guidance lists the U.S. Pet Poison Helpline at 800-213-6680 and the ASPCA Animal Poison Control at 888-426-4435. These are U.S. numbers; outside the United States, use the emergency or poison service listed for your country.
Do not wait for symptoms to become severe before calling. Early contact gives your veterinarian more options.
