Caring for a diabetic dog at home means following an individualized routine for meals, insulin, treats, and exercise—and knowing when a change calls for veterinary help. The American Animal Hospital Association (AAHA) says caregivers should receive clear written instructions and access to a trained veterinary team. Its guidance is educational, not a substitute for your dog’s veterinarian or the instructions for the prescribed insulin and device.
Plan for an ongoing routine and veterinary support
Diabetes management takes time and commitment, but AAHA says successful management is possible with time. Ask your veterinary team to explain the routine, answer questions, and help troubleshoot changes. Keep written instructions somewhere easy to consult, including a plan for what to do when the clinic is unavailable.
Use your veterinarian’s directions for your dog’s prescription. Do not change an insulin dose or how often it is given unless your veterinarian has clearly instructed you to do so.
Coordinate meals and insulin with your veterinarian’s plan
AAHA’s client-education guidance says a full insulin dose should be given only after a dog has eaten at least 50% of its meal. For intentional fasting or when the dog eats less than half the offered meal, the guideline describes giving a half-dose. These are conditional instructions, not a replacement for the plan your veterinarian has prescribed for your dog; confirm what to do in your dog’s specific circumstances before changing a dose.

Contact your veterinarian if your dog does not eat at least half of two consecutive meals. If your dog is unwell or you are unsure what to do about an upcoming dose, call the veterinary team for instructions rather than guessing.
Keep food, treats, and exercise consistent
AAHA emphasizes consistency in food, treats, and exercise as part of diabetes care. Discuss your dog’s nutrition and weight management with the veterinarian. Include routine treats in that conversation: their nutrient content, how much your dog receives, and their potential effect on blood glucose can matter to the overall plan.

Handle the prescribed insulin and device as directed
Insulin handling depends on the product. AAHA says to shake Vetsulin until it forms a uniformly milky suspension. It says several other named insulin vials and insulin degludec should be rolled rather than shaken. Follow the manufacturer’s instructions for the specific insulin and any pen; do not assume that one product’s handling directions apply to another.
Device choice and dosing accuracy depend on the prescribed insulin and concentration. AAHA notes that 0.3 mL or 0.5 mL syringes, or insulin pens, can facilitate accurate dosing, especially for doses below 5 U. This does not mean every dog needs one of these devices. Ask your veterinarian to confirm that a device matches the prescription and its concentration. Concentrated degludec U-200 and glargine U-300 must be used with their provided pens, according to AAHA.

Recognize possible hypoglycemia and act promptly
Low blood glucose (hypoglycemia) can be serious. AAHA lists lethargy, sleepiness, unusual behavior, an abnormal gait, weakness, tremors, collapse, and seizures among possible signs. If your dog seems weak, unusually sleepy, unsteady, or otherwise abnormal after insulin, contact a veterinarian promptly for guidance.
If signs are mild and your dog is conscious
For mild signs in a conscious dog, AAHA describes offering a small high-carbohydrate meal or using corn syrup in the circumstances outlined in its guidance. Contact your veterinarian for instructions and veterinary care; do not improvise additional treatment steps or delay help if the dog worsens.

If signs are moderate to severe, or your dog has a seizure
Moderate-to-severe signs need veterinary care. A seizure requires immediate veterinary attention. Do not put your fingers in a hypoglycemic dog’s mouth. An obtunded (markedly less responsive) dog may aspirate material into the lungs, so giving food or liquid by mouth can be dangerous. Seek urgent veterinary help.

Ask the veterinary team to make the instructions practical: what to do if your dog eats less than usual, misses a meal, or shows possible signs of low blood glucose; how to handle the specific insulin and device; and how to reach help when the clinic is closed.
