Diabetic ketoacidosis (DKA) is a life-threatening complication of diabetes. If a diabetic dog is vomiting, not eating, weak, markedly lethargic, dehydrated, or otherwise unwell, contact a veterinarian or emergency hospital now—do not wait for a home ketone result. DKA is diagnosed by assessing a sick dog and confirming both ketosis and acidosis; ketones alone do not establish it.
Why is DKA dangerous in dogs?
DKA develops when a dog has too little effective insulin to use glucose normally. The body increasingly uses fat for energy, producing ketones that can build up as acids in the blood. This can cause profound metabolic disturbances and become life-threatening. Merck Veterinary Manual describes DKA as decompensated diabetes involving insulin deficiency and ketoacid accumulation.
Other illness can trigger or complicate the crisis. Examples include pancreatitis, urinary tract or other infections, hypercortisolism, and kidney disease, according to the 2026 AAHA Diabetes Management Guidelines for Dogs.
What are the signs of diabetic ketoacidosis in dogs?
Signs can include vomiting, loss of appetite, lethargy, weakness, depression, and dehydration. Severe illness may cause altered mentation or coma. These signs are not unique to DKA, and no single symptom confirms it.

Veterinary findings may include marked hyperglycemia, dehydration, azotemia, and low bicarbonate or total CO2 associated with metabolic acidosis. Urinalysis may show glucose and ketones, as well as findings such as protein, bacteria, or casts. The combination of signs and test results matters; a home observation or one abnormal result cannot diagnose DKA. See the AAHA DKA guideline.
Can a dog with diabetes and ketones wait until morning?
A diabetic dog that is systemically ill needs prompt veterinary assessment, whether or not ketones have been detected at home. Call your veterinarian or an emergency hospital for guidance rather than waiting to repeat a test.

The AAHA guideline recommends 24-hour referral for dogs with severe dehydration, weakness or inability to stand, obtundation, low blood pressure or reduced circulating blood volume, severe acidosis, significant electrolyte abnormalities, hyperosmolality, or signs affecting the central nervous system. A veterinarian can determine the appropriate level of care from the dog’s condition and examination.
How do veterinarians diagnose DKA?
Diagnosis requires a compatible clinical picture in a sick diabetic dog and confirmation of both ketosis and acidosis. The AAHA guideline states: “Diagnosis of DKA relies on identifying compatible clinical signs in sick diabetic patients and confirming ketosis and acidosis.” Veterinarians evaluate the dog’s history and examination alongside blood and urine testing; ketones by themselves are not enough.

Blood glucose helps assess diabetes, but a glucose value alone does not define DKA. In the AAHA/Project ALIVE diagnostic context for diabetes mellitus, blood glucose at or above 200 mg/dL together with glucosuria and characteristic signs or a hyperglycemic crisis supports diabetes diagnosis; it is not, by itself, a DKA definition. AAHA gives the average canine renal glucose threshold as 160–200 mg/dL. These figures are diagnostic context, not home decision thresholds. See AAHA’s section on recognizing and diagnosing diabetes.
Do urine ketones mean my dog has DKA?
No. Dogs with DKA usually have ketones in their urine, but ketonuria is not synonymous with DKA. The veterinarian must assess whether the dog is sick and confirm acidosis as well as ketosis. A positive urine result should be discussed with the veterinary team, especially if the dog is unwell; it is not a reason to manage the situation at home. AAHA’s diagnostic guideline makes this distinction explicitly.

What does a blood ketone meter tell you?
Urine ketone strips and blood beta-hydroxybutyrate (BHB) meters measure ketones differently. BHB is the earliest and most predominant ketone body in canine DKA. AAHA says blood BHB meters identify earlier DKA stages more accurately than urine testing, but neither kind of result replaces assessment of a sick dog.

Comparison and availability details are from the 2026 AAHA DKA guideline. If you use urine strips as part of a veterinarian-directed monitoring plan, follow that plan; a reassuring or abnormal home result should not delay care for a sick dog.

How do vets treat DKA in dogs?
DKA treatment requires hospital-level monitoring and is tailored to the dog’s test results and concurrent illnesses. At a high level, veterinary care may include:
- Fluid therapy to address dehydration and circulatory problems.
- Careful management of electrolytes, including potassium and sometimes phosphate.
- Insulin therapy with frequent checks of blood glucose and ketones.
- Investigation and treatment of an underlying or concurrent illness.
- Nursing care and repeated reassessment of the dog’s condition.
AAHA’s primary-care protocol recommends attempting to initiate insulin within 4–6 hours of presentation, after fluids and potassium assessment. During the described regular-insulin phase, it calls for blood glucose checks every 1–2 hours and BHB monitoring every 8–12 hours until stable. These are clinician-directed protocol details, not instructions for owners to give or adjust insulin. Do not change a dog’s insulin dose or attempt DKA treatment without veterinary direction.
Transition to routine injections under the skin occurs only after hydration and ketones improve and the dog is eating consistently. The immediate goal is stabilization; regulating glucose for discharge is not the goal of acute DKA management. These care details are from the AAHA guideline.
What should owners expect during recovery?
There is no single general survival estimate established by the cited guidance. A dog’s outlook depends on how ill the dog is and on factors such as concurrent disease and response to treatment. Ask the treating veterinarian what the dog’s test results show, which stabilization milestones have been reached, and what monitoring and insulin plan is appropriate after discharge.
