Hypercalcemia means a cat’s blood calcium is elevated; it is a finding, not a diagnosis of the underlying disease. Some cats show no obvious signs, and the symptoms that do occur are nonspecific. A veterinarian needs to interpret the result and investigate its cause before deciding whether treatment is needed and what kind will help.
What a high calcium result means
The Merck Veterinary Manual defines hypercalcemia as calcium elevated outside the normal reference range. A result must be considered alongside the cat’s history, examination, and other test findings. It does not, by itself, identify a cause or determine a treatment.
Veterinary evaluation matters even when a cat seems well: some cats have no apparent symptoms, while others have signs that could fit many different conditions.
What causes hypercalcemia in cats?
Potential causes include idiopathic hypercalcemia, cancer, chronic kidney failure, hyperthyroidism, granulomatous disease, primary hyperparathyroidism, and excess vitamin D. Which possibilities are most relevant depends on the individual cat’s test results and history.

Idiopathic hypercalcemia
“Idiopathic” means that a cause has not been identified after other causes have been investigated and excluded. The Merck Veterinary Manual describes idiopathic hypercalcemia as the most common form in cats, but it is not a conclusion to draw from a high calcium result alone.
Cancer and other systemic disease
Cancer-associated hypercalcemia can occur with conditions including lymphoma, squamous cell carcinoma, and multiple myeloma. Kidney disease and other systemic disorders are also among the causes veterinarians may consider.

Vitamin D excess and other causes
Excess vitamin D is another possible cause, so a veterinarian may ask about relevant exposures. Hyperthyroidism, granulomatous disease, and primary hyperparathyroidism are additional possibilities; testing helps determine which, if any, fits the cat.

Some cats, particularly cats with idiopathic hypercalcemia, have no obvious clinical signs. When signs are present, they may include:

- Reduced appetite or anorexia
- Weight loss
- Vomiting
- Difficulty urinating or inappropriate urination
These signs do not diagnose hypercalcemia. They can occur with many other health problems, so a cat showing them needs veterinary assessment rather than an assumption about the cause.
A 2000 case series of 20 cats reported vomiting in 6, weight loss in 4, dysuria in 4, anorexia in 3, and inappropriate urination in 3. These are counts in a small group, not estimates of how common each symptom is among all cats with high calcium.

How veterinarians diagnose the cause
The work-up is guided by the cat’s clinical picture. It may include:
- Review of the cat’s history and physical examination
- Serum biochemistry and measurement of ionized calcium
- Urinalysis
- Evaluation of calcium-regulating hormones
- Diagnostic imaging
- Cytology or biopsy when findings indicate that further investigation is needed
Additional testing helps assess possible underlying disease and whether complications such as urinary stones or kidney problems are present. A veterinarian considers idiopathic hypercalcemia only after other plausible causes have been investigated and excluded.

How is hypercalcemia in cats treated?
Treatment depends on the cause, the cat’s signs and severity, and findings such as urinary stones or kidney issues. It may involve addressing an underlying condition, supportive care such as fluids, or selected medicines to lower calcium. For idiopathic cases, a veterinarian may consider monitoring, diet changes, or medication according to the individual cat’s situation.
Do not change a cat’s diet or give medication to treat high calcium without veterinary direction. There is no single food, supplement, or other product established as a universal treatment for every cat.
A 2000 study of 20 cats found that dietary modification did not resolve hypercalcemia in 11 cats, while prednisone resolved it in 4. Seven of the 20 cats (35%) had urolithiasis. These findings show that outcomes varied in that small case series; they are historical evidence, not a current universal treatment protocol or a product recommendation.
What is the outlook?
Prognosis depends on the underlying cause and the cat’s overall condition. The available evidence does not support one general survival estimate for all cats with hypercalcemia. Ask the veterinarian what the results establish, what remains under investigation, and how the cause and any complications affect the individual cat’s outlook.
