It is not insulin, and it is not appropriate for every diabetic cat. Its most serious risk is diabetic ketoacidosis (DKA), including euglycemic DKA, which can be fatal and may occur even if blood glucose does not look high. Screening and ongoing veterinary monitoring are essential.
What is Bexacat for cats?
Bexacat is the brand name for bexagliflozin, an SGLT2 inhibitor. It works in the kidneys: by reducing glucose reabsorption, it allows more glucose to leave the body in urine. It is an oral diabetes medicine, not insulin.
In the United States, the FDA-labeled use is to improve glycemic control in otherwise healthy cats with diabetes mellitus that have not previously been treated with insulin. A veterinarian must determine whether a cat fits that narrow profile; a diabetes diagnosis alone does not establish that Bexacat is suitable. FDA reported that Bexacat improved glycemic control in over 80 percent of cats in two six-month field studies and an extended-use field study. What is the Bexacat dosage for cats?
The U.S. label, updated May 19, 2026, states a dose of one 15 mg tablet by mouth once daily for cats weighing at least 3.0 kg (6.6 lb). The label says to give it at approximately the same time each day, with or without food, regardless of the cat’s blood glucose level. This is label information, not an individualized treatment direction. Which cats should not receive Bexacat?
Bexacat is contraindicated in cats that have previously been treated with insulin, are currently receiving insulin, or have insulin-dependent diabetes mellitus. Do not start it in a cat that is anorexic, dehydrated, or lethargic when diabetes is diagnosed. These restrictions matter because a cat who needs insulin or is already unwell may face serious complications with this treatment.
Before prescribing, the veterinarian should screen for conditions including kidney, liver, and pancreatic disease, as well as ketoacidosis. The FDA also advises regular examinations and blood tests after treatment begins. What are Bexacat’s side effects?
Reported effects include vomiting, diarrhea or loose stools, reduced appetite or not eating, low energy, dehydration, weight loss, increased drinking or urination, and ketones in the urine (ketonuria). Serious complications include DKA, euglycemic DKA, and pancreatitis. A cat may develop a serious problem even after appearing stable, so new symptoms during treatment warrant attention rather than watchful waiting.
Recognize possible DKA urgently
The current label says that sudden appetite loss or refusal to eat, lethargy, dehydration, or weight loss calls for immediate discontinuation of Bexacat and assessment for DKA, regardless of blood glucose. Euglycemic DKA can occur without an apparently high glucose reading, so a reassuring glucose result does not rule it out.

Contact a veterinarian urgently if your cat has appetite loss, low energy or a behavior change, vomiting or diarrhea, weakness, difficulty walking or standing, dehydration, or weight loss while taking Bexacat. How safe is Bexacat?
Bexacat has important safety concerns, and safe use depends on selecting an appropriate cat, checking for underlying problems before treatment, and continuing veterinary monitoring for as long as the cat takes it. It should not be treated as a simpler or interchangeable substitute for insulin. In its December 8, 2022 approval announcement, the FDA said safety concerns can be mitigated through careful screening, diligent monitoring regardless of treatment duration or response, and prompt recognition and appropriate treatment of serious reactions. How does Bexacat differ from Senvelgo?
Bexacat and Senvelgo are distinct FDA-approved oral diabetes medicines for cats, not interchangeable products. Bexacat contains bexagliflozin and comes as a tablet; Senvelgo contains velagliflozin and is an oral solution. Eligibility depends on the individual cat’s medical history and test results, and each medicine has its own administration and safety requirements.
