BOAS Surgery or Euthanasia? Vets Urged to Put Each Dog’s Welfare First

A Vet Times report urged individualized welfare decisions for dogs with BOAS—not euthanasia as a general alternative to surgery. Here is how to discuss prognosis, risks and quality of life with your veterinary team.

A recent discussion about brachycephalic obstructive airway syndrome (BOAS) urged veterinarians to consider euthanasia when an individual dog’s suffering is likely to remain unacceptable, even with surgery. It was not a recommendation to euthanize dogs with BOAS generally: corrective surgery can improve quality of life, and the right decision depends on the dog’s condition, likely outcome and welfare.

What the headline means—and what it does not

In a 29 May 2026 report, Vet Times quoted Royal Veterinary College academics Dan O’Neill and Rowena Packer discussing difficult welfare decisions for dogs with BOAS. O’Neill said that if a dog is likely to continue having a life “not worth living” despite surgery, euthanasia needs to be considered. Packer asked whether the goal is an acceptable quality of life for that individual animal, rather than simply extending life.

Those are attributed views in a reported discussion, not a blanket clinical directive or evidence that BOAS surgery is futile. The point is to assess the likely welfare outcome for the dog in front of the veterinary team—not to choose euthanasia based on breed, diagnosis or the headline alone.

What BOAS can mean for a dog’s daily life

BOAS is associated with impaired breathing and can affect exercise, sleep and tolerance of heat. Signs described by the University of Cambridge Department of Veterinary Medicine include respiratory noise, sleep-disordered breathing, heat intolerance, cyanosis (a bluish tinge associated with inadequate oxygenation) and collapse. Their presence and severity need veterinary assessment; a noisy-breathing dog should not be assumed to have a particular grade or prognosis without examination.

Cambridge describes a functional grading system from 0 to III. Grades II and III indicate clinically affected dogs that need management and/or treatment. A grade helps describe functional impact, but it does not by itself settle whether surgery or euthanasia is appropriate for an individual dog.

What surgery may—and may not—achieve

Cambridge guidance says BOAS is not curable, but corrective upper-airway surgery can improve quality of life. The expected result varies with disease severity; dogs with more advanced laryngeal collapse may have a poorer prognosis, and some severely affected dogs may need revision surgery. Surgery therefore needs an individualized discussion of likely improvement and the possibility of persistent signs or further treatment.

Cambridge identifies anaesthetic recovery problems, airway inflammation or swelling, and aspiration pneumonia among the risks. The veterinary team can explain which risks matter for this dog and how the proposed procedure, monitoring and recovery plan address them.

Questions to take to the veterinary team

Ask for a case-specific account of both the likely benefits and the burdens of each option. Cambridge describes assessment that may include veterinary examination, respiratory function testing and exercise grading, with surgery tailored to the dog.

  • What is this dog’s functional grade, and which airway sites are affected?
  • Which signs are the proposed procedure expected to improve, and which may remain?
  • What are the anaesthetic and post-operative risks for this dog, and what would recovery and follow-up involve?
  • If surgery is not undertaken, what management options are appropriate, and what welfare changes should prompt reassessment?
  • Given the dog’s current ability to breathe, sleep, move, eat and drink, and regulate temperature, what quality of life is realistically expected under each option?

These questions do not supply a universal threshold for euthanasia. They help make the decision about expected welfare, not an abstract preference for extending life or avoiding treatment.

Management and additional assessment

Cambridge recommends avoiding overheating and excessive exercise in affected dogs. It also says body-condition scoring is more useful than comparing a dog’s weight with breed averages; for obese dogs with mild to moderate BOAS signs, its guidance suggests weight loss before surgery. These are considerations to discuss with a veterinarian, not substitutes for clinical evaluation.

Cambridge describes whole-body barometric plethysmography (WBBP) as a non-invasive way to assess respiratory function and help monitor disease progression and surgical outcomes. Its Department of Veterinary Medicine reports that more than 1,800 brachycephalic dogs have been tested, with an approximately 85% tolerance rate; the page does not state a year for those figures. This describes the assessment’s reported use and tolerance, not a result that predicts whether surgery or euthanasia is right for a particular dog.

Using welfare frameworks without replacing clinical judgment

O’Neill suggested the Innate Health Assessment (IHA) as a possible aid to welfare conversations. Vet Times describes it as a 10-point checklist covering a dog’s ability to perform natural functions and behaviours. The report does not establish it as a BOAS diagnostic test or a replacement for veterinary assessment.

The RVC’s description of innate health includes breathing and oxygenating effectively, regulating body temperature, moving without effort or discomfort, eating and drinking, sleeping, communicating and self-grooming. These functions can help structure a conversation about how illness limits a dog’s life. They do not determine the outcome on their own.

The decision remains individual

The cited sources provide no comparative statistic or universal threshold showing when euthanasia is preferable to BOAS surgery. Cambridge guidance supports the possibility of quality-of-life improvement through surgery while recognizing that outcomes vary; the welfare discussion reported by Vet Times asks clinicians to consider whether an individual dog’s suffering can be acceptably relieved. The decision should follow a veterinary assessment of likely outcomes and the dog’s lived experience.
