Cerebellar hypoplasia is incomplete development of the cerebellum, the part of the brain that coordinates movement and balance. It usually becomes apparent when a puppy starts walking and is generally nonprogressive; although there is no treatment that restores the missing tissue, many affected dogs adapt and have a good quality of life.
What cerebellar hypoplasia means
The cerebellum helps coordinate posture, balance, and purposeful movement. In a dog with cerebellar hypoplasia, it did not develop completely. The resulting movement problems reflect a developmental difference in the brain, rather than a condition in which the cerebellum is steadily degenerating.
Signs are commonly apparent from the beginning of walking. How much they affect a dog varies: some can move around and eat independently, while others need more help with everyday activities.
Symptoms to look for
The main sign is cerebellar ataxia: poor coordination that can make a dog look wobbly or unsteady even when it has strength in its legs. Signs may include:

- A wide-based stance or swaying while standing.
- High-stepping, exaggerated, or choppy limb movements.
- Poor balance and difficulty coordinating steps.
- Intention tremors, often affecting the head or neck and becoming more noticeable during purposeful movement, then easing at rest. Tremors can also affect the limbs.
- Nystagmus, or involuntary eye movements.
One sign alone cannot establish the cause. Similar coordination problems can occur with other neurologic conditions, including some that worsen or need prompt treatment.
Why it happens
The cerebellum may be affected by events before birth or during early development. Viral infection is a classic suspected cause: Sarah A. Moore, DVM, a neurology and neurosurgery professor at The Ohio State University College of Veterinary Medicine, told the American Kennel Club that infections affecting an animal in the womb or shortly after birth are a common way veterinarians think about cerebellar hypoplasia. Canine parvovirus and canine herpesvirus are among the infectious risks discussed in that context. A veterinary neuropathology review has also reported canine parvoviral DNA detected in cases of canine cerebellar hypoplasia.

Infection is not the explanation for every dog. Other possible contributors include hereditary conditions, toxic exposure, brain injury, and other developmental events. Specific genetic associations have been described in some breeds: an Eurasier genetic cause is linked to underdevelopment or absence of the cerebellar vermis as part of Dandy-Walker syndrome. Toy Fox Terriers are predisposed to Dandy-Walker syndrome, but the causal variant has not been established. In an individual dog, the cause may remain unknown.
How veterinarians distinguish it from other problems
A veterinarian considers when the signs began, whether they are changing, the dog’s developmental and birth history, and findings on physical and neurologic examination. The timing and course help distinguish cerebellar hypoplasia from cerebellar abiotrophy, an important look-alike:

These patterns are useful clues, not a diagnosis by themselves. Other neurologic diseases can also cause ataxia, and the work-up may include blood and urine tests to investigate infectious, inflammatory, metabolic, toxic, traumatic, or other causes. MRI can confirm the cerebellar malformation and may reveal concurrent structural abnormalities. The veterinarian chooses tests based on the dog’s signs and history.

A puppy that has been uncoordinated from its first attempts to walk still needs a veterinary assessment; appearance alone cannot confirm cerebellar hypoplasia. Seek prompt veterinary care if coordination problems begin suddenly, are newly worsening, seem painful, or occur alongside signs of systemic illness. Those features are not the typical stable pattern and may point to another condition.

Treatment and everyday care
There is no curative treatment that restores cerebellar tissue. Care is tailored to the dog’s abilities and focuses on safety, daily function, and any separate condition found during evaluation.
- Reduce fall and injury risks in the places the dog walks, rests, and eats.
- Adapt the home and routine to the dog’s balance and mobility rather than assuming every affected dog needs the same equipment or assistance.
- Keep track of whether the dog can move around safely and eat and drink adequately; discuss difficulties with the veterinarian.
- Follow veterinary advice for any concurrent disorder. A medication, supplement, or device should not be treated as universally appropriate for cerebellar hypoplasia.
Outlook and quality of life
Because cerebellar hypoplasia is generally nonprogressive, the movement signs do not usually worsen simply because of the hypoplasia itself. Sarah A. Moore, DVM, has described it as not curable but nonprogressive, and noted that many dogs compensate well and live a good-quality life. A veterinary review likewise characterizes the signs as usually nonprogressive and the prognosis as fair to good, depending on severity.
An individual dog’s outlook depends on how much incoordination affects safe movement and eating, and on whether hydrocephalus or another concurrent disorder is present. The veterinarian can help assess those factors and adjust the care plan as the dog grows.
