Senior cats can develop kidney disease, thyroid disease, diabetes, high blood pressure, dental problems, joint pain, gastrointestinal disease, cancer, cognitive changes, and other age-related concerns. These are not a ranked top ten, and signs often overlap: a change in thirst, weight, appetite, behavior, or mobility calls for a veterinary assessment rather than a guess that it is “just old age.”
The 2021 AAHA/AAFP Feline Life Stage Guidelines identify several conditions that deserve increased attention as cats age, while noting that their list is not comprehensive. The concerns below group and separate those issues for clarity; they are not a prevalence ranking, and more than one problem can affect the same cat.

When is a cat considered senior?
The 2021 AAHA/AAFP life-stage guideline defines a senior cat as older than 10 years. The 2021 AAFP Senior Care Guidelines note that some cats may be considered senior from age 8, or earlier for certain breeds or genetic predispositions. Age is a useful guide, not a switch that determines an individual cat’s health.

1. Chronic kidney disease
Chronic kidney disease (CKD) is a common concern in older cats. Possible clues include changes in drinking or urination, reduced appetite, and weight loss. Those signs are not specific to kidney disease, so a veterinarian needs to assess the cat rather than infer a diagnosis from one symptom.

Blood and urine tests, considered alongside the cat’s history and examination, help evaluate kidney health. A veterinarian may recommend a renal diet for some cats with CKD, but food choices should account for the individual cat’s overall condition and any other illnesses.
2. Hyperthyroidism
Hyperthyroidism can cause weight loss despite an increased appetite, increased thirst and urination, vomiting, diarrhea, restlessness, or an unkempt coat. A 2021 AAFP guideline states that 10% of cats over 10 years of age are affected; that figure applies to the age group cited, not to cats of every age or every population.
Diagnosis generally combines clinical assessment with thyroid hormone testing. Kidney and heart health matter when evaluating the condition and planning care. Cornell Feline Health Center describes medication, radioactive iodine, surgery, and dietary therapy as possible treatments; suitability depends on the cat’s health and circumstances. These approaches differ in how they control or address the disease, and a veterinarian can discuss monitoring, access, and the cat’s other conditions before recommending one.

3. Diabetes mellitus
Diabetes can lead to increased thirst and urination, alongside changes in appetite or body weight. Because these signs can also occur with other illnesses, diagnosis and monitoring require veterinary testing. The AAFP senior-care guidance notes that many diabetic cats need insulin at diagnosis and that concurrent disease can affect how well diabetes is controlled. Do not substitute a home diet change for a treatment plan.
4. High blood pressure
Hypertension is often associated with kidney disease or hyperthyroidism, but it is also an important condition in its own right. It can damage the eyes, brain, kidneys, and heart. New blindness, imbalance, seizures, or a marked neurological change can indicate serious injury and warrants urgent veterinary care.
Clinic stress can affect a blood-pressure reading, so measurement technique matters; a veterinarian may need to assess the result carefully or repeat the measurement.

5. Dental disease and tooth resorption
Dental disease and tooth resorption can cause substantial pain and make eating difficult. Cats may conceal oral discomfort, so a change in appetite, chewing, grooming, or behavior may be more noticeable than an obvious sign of mouth pain. A veterinary oral examination is appropriate when these changes appear. The AAFP guideline includes dental radiographs as part of feline dental care.
6. Degenerative joint disease and osteoarthritis
Joint pain may show up as a cat jumping less, avoiding stairs or a favorite perch, or struggling to reach food, water, or the litter box. Limping is not required for pain to interfere with daily life. A 2021 AAHA/AAFP guideline cites estimates that 40–92% of cats may present with clinical signs associated with degenerative joint disease. The range reflects studies with different samples and diagnostic approaches, not one precise prevalence figure for all cats.
While arranging a veterinary assessment, make essential resources easier to reach. A ramp, steps, or a low-entry litter box may improve access, but none treats arthritis or replaces an evaluation of pain and mobility.

7. Chronic gastrointestinal disease
Chronic enteropathies include conditions such as inflammatory bowel disease and gastrointestinal lymphoma. Persistent vomiting or diarrhea, changes in appetite, and weight loss deserve assessment instead of being dismissed as aging. These signs do not identify a cause by themselves; a home checklist cannot reliably distinguish among gastrointestinal diseases.
8. Cancer and other neoplasia
Neoplasia is among the conditions veterinary guidance says deserves increased attention in older cats, and the Cornell Feline Health Center notes that cancer becomes more prevalent with age. A new lump, a wound that does not heal, ongoing weight loss, appetite changes, or another persistent change merits veterinary investigation. These signs do not establish that a cat has cancer.
9. Cognitive dysfunction and behavior changes
Confusion, nighttime vocalization, altered interaction, and changes to familiar routines may be associated with cognitive dysfunction. Similar changes can also result from pain, sensory loss, hypertension, hyperthyroidism, or other medical problems. The 2021 AAHA/AAFP guidelines advise that cats showing new or unusual behavior be evaluated for medical conditions.

10. Oral tumors and persistent pain or frailty
This is an additional concern, not a claim that oral tumors are the tenth most common senior-cat diagnosis. The AAFP guideline lists oral tumors among dental conditions seen in cats over age 7 and emphasizes chronic pain and frailty in older cats. New oral changes or persistent signs of pain warrant a veterinary examination.
How to tell illness from normal aging
There is no symptom checklist that can diagnose a senior cat at home. Increased urination, for example, can occur with kidney disease, diabetes, or hyperthyroidism. A change in litter-box use may also relate to pain, weakness, urinary disease, or constipation. The same visible change can have different causes, and conditions can coexist.
AAHA lists possible signs of chronic disease that include progressively lower energy, behavior changes, reduced appetite or not eating, altered thirst, weight loss over time, changes in mobility, and wounds that do not heal. Cornell cautions owners not to assume physical or behavior changes are simply untreatable aging. Keep track of your cat’s usual appetite, water intake, weight, activity, litter habits, and behavior, and tell the veterinarian what has changed and when.

Evaluation starts with the cat’s history and physical examination. Trends in weight, body condition, and muscle condition can help the veterinary team spot changes over time. The AAHA senior-pet minimum database includes a detailed blood profile with a thyroid panel and urinalysis. Further testing depends on the cat’s history, examination findings, initial results, and the body system under investigation.
No single panel rules out every disease. Ask the veterinarian which tests fit your cat’s symptoms and health history, what the results do and do not show, and whether follow-up is needed.
How care is chosen when problems overlap
There is no one diet, medication plan, or monitoring schedule for every senior cat. The AAFP Senior Care Guidelines emphasize comorbidities and chronic pain, so decisions about treatment, food, monitoring, and quality of life should be made with the veterinary team. A renal diet, diabetic nutrition plan, or iodine-restricted food is not a general-purpose senior-cat diet; each should be considered only in the context of an appropriate diagnosis and the cat’s individual needs.
