Changes such as persistent pain, trouble breathing, eating less, or repeated medical crises mean your cat needs veterinary attention—not that death is certain or imminent. Some causes may be treatable, and no checklist can tell you how much time a cat has left. A veterinarian must assess your cat’s symptoms, comfort, and quality of life.
11 changes that deserve a conversation with your veterinarian
These are reasons to seek an assessment, not a ranked countdown. A cat may show only some of them, and the number of signs does not predict how soon death will occur. Cats can hide pain, so pay attention to changes from your cat’s usual behavior.
1. Persistent or worsening pain
Pain can show up as hiding, eating or moving less, reduced grooming, difficulty standing, squinting, unusual sleeping positions, sensitivity to touch, aggression, or changes in litter-box use. Cats often conceal pain; unusual behavior or a cluster of small changes is worth reporting. The Feline Veterinary Medical Association (FelineVMA) puts it plainly: “Cats are good at hiding pain, so recognizing these signs will help so your cat does not suffer.”
2. Difficulty breathing or breathlessness
Breathing trouble is not something to dismiss as old age. AAHA and FelineVMA identify difficulty breathing or breathlessness as a serious change requiring veterinary attention and assessment. Contact a veterinarian promptly if you notice it.

3. Eating much less or refusing food
A marked appetite change or trouble eating can affect comfort and may have several causes. Ask your veterinarian what might be behind it and how to manage it; do not force food based on a checklist or without veterinary guidance.
4. Drinking changes or difficulty staying hydrated
Notice whether your cat is drinking more or less than usual, or seems to have trouble drinking. These changes can matter to comfort and hydration, but their cause and appropriate support need veterinary assessment.
5. Weight loss or loss of muscle
Unexplained weight loss or a thinner, less muscular body is a change to report. It can have multiple causes and does not, by itself, establish that a cat is dying.

6. Persistent digestive changes
Ongoing nausea, vomiting, constipation, or other changes in appetite and digestion deserve a veterinary conversation. These symptoms alone do not prove terminal decline; the veterinarian can assess their cause and discuss ways to relieve discomfort.
7. Difficulty standing, walking, or reaching essentials
Struggling to rise, walk, or reach food, water, or the litter box can reflect pain or reduced mobility. Tell the veterinarian what your cat can and cannot manage around the home.
8. Withdrawal, hiding, or less interest in surroundings
A cat who increasingly hides, withdraws from family, or shows less interest in familiar surroundings may be unwell or uncomfortable. Share the change with your veterinarian, especially if it is new or worsening.

9. Reduced grooming or a deteriorating coat
Less grooming or a coat that looks poorer than usual can accompany pain or illness. Report the change rather than assuming it is simply a consequence of age.
10. Changes in urination, bowel movements, or litter-box habits
Note changes in how often your cat urinates or passes stool, difficulty doing either, or a new pattern of avoiding the litter box. Describe exactly what has changed; the behavior does not identify its cause on its own.
11. Repeated crises or a worsening cluster of symptoms
Repeated medical crises or several changes progressing together are reasons to discuss the illness trajectory and next steps with your veterinarian. Ask for a plan for both everyday care and what to do if your cat worsens suddenly.

How to assess your cat’s quality of life
Quality of life includes physical and emotional well-being. AAHA recommends considering pain, eating and drinking, breathing, elimination, mobility around the home, and engagement with caregivers and surroundings. FelineVMA also advises watching for changes in pain, nausea, thirst, breathlessness, and fear or anxiety.
Keep a brief record of when each change began, how often it happens, and whether it is getting worse. Bring those observations to the veterinary visit. If a change is difficult to describe, ask your clinic whether a home video would help explain it.

- Could my cat be uncomfortable, and which symptoms might be treatable?
- Which changes should prompt an urgent call or visit?
- What should our daily-care plan and crisis plan include?
- Is palliative care or hospice appropriate for my cat?
- How will we assess quality of life as the illness changes?
AAHA recommends personalized care planning, including a discussion of the disease trajectory and signs of approaching or imminent death. No sign or combination of signs can provide a reliable estimate of how much time remains.
Palliative care, hospice, and end-of-life choices
Palliative care focuses on easing pain and other symptoms and may be provided alongside treatment. Hospice is an end-stage approach for a terminally ill animal and family that plans for decline, crises, comfort, and death. Your veterinarian can explain whether either approach fits your cat’s condition and your family’s needs.
End-of-life discussions may include euthanasia or hospice-supported natural death. Natural death should not mean leaving a cat to suffer: veterinary guidance makes effective relief of pain and distress central to hospice care. Decisions should be made with the veterinary team, taking account of symptom control, the illness, practical care needs, and your family’s goals.
Comfort adjustments you can discuss for home
Ask your veterinarian whether changes to your cat’s surroundings could make daily life easier. FelineVMA recommends making resources accessible and providing safe resting places. Depending on your cat’s needs, possible adjustments include:

- Adding nonslip flooring and comfortable bedding.
- Reviewing litter-box location and design. If entry or exit is difficult, FelineVMA suggests a box with one low side.
- Supporting hygiene and keeping the resting area at a comfortable temperature.
These adjustments may improve access or comfort; they do not treat or diagnose the cause of a change in mobility or behavior.
