13 Cat Anxiety Medications Veterinarians Discuss: What You Need to Know

Veterinary references describe 13 medications for feline anxiety and related behavior problems. Here is how they are grouped, when they are used, and what to ask your vet before any prescription.

No single medication is the best choice for every anxious cat. Veterinary references describe 13 drugs that veterinarians may discuss for feline anxiety and related behavior problems, and the right option depends on what is driving the behavior, whether the cat has other health conditions, and whether the problem is ongoing or tied to a specific event such as a car ride or a clinic visit. The list below is an inventory of drugs described in those references. It is not a ranking, an approval list, or a dosing guide, and it does not reflect how often any of these drugs are prescribed.

Start with the cause, not the prescription

Fear and anxiety in cats often look like hiding, avoidance, a crouched posture, vocalizing, escape attempts, or aggression. Some behavior changes also have medical causes. The American Association of Feline Practitioners (AAFP) feline behavior guidance, which is older and should be read alongside current references, says to rule out medical problems and complete a physical and laboratory evaluation before prescribing for a behavior problem. A sudden change in behavior, or a change in a cat that has never shown it before, is a reason to book a veterinary visit before anyone starts a medication.

The 13 medications at a glance

The table groups each drug by the role the references give it. Those groups are broad descriptions of how the drugs are discussed, not clinical rules. Where a reference gives no drug-specific caution, the table says so.

Trazodone is also discussed for veterinary visits in the AAFP/ISFM 2022 guidelines, but it is not one of the 13 names counted here. That guideline notes less evidence for trazodone in this setting than for gabapentin.

Longer-term and situational medicines serve different jobs

The difference between the two groups is mostly about timing and planning.

  • Longer-term medicines are intended for ongoing fear, anxiety, or related problems. Merck states that serotonergic medications can take 4–6 weeks to reach their full effects in the behavior context it describes, so they are not a same-day fix. Merck also states that drug treatment works best when combined with behavior modification.
  • Situational medicines are planned around a known trigger. The AAFP/ISFM 2022 guidelines describe discussing pre-visit medication when a clinic visit is expected to be difficult, with the medication and timing set with the veterinarian for that individual cat.
  • Some drugs appear in both roles. Merck describes clonazepam for ongoing anxiety management in selected cases, while the references also list it among situational options.

What the evidence does and does not establish

The veterinary references do not provide comparative response rates or head-to-head results for these 13 drugs, so none can be called the most effective. Merck notes that the evidence base for many behavior medications is limited and that cats respond differently. Merck’s owner guidance also states that safety and effectiveness information for feline use may be limited for drugs developed for people.

Two practical gaps follow from this. First, the timing guidance in the references is clinical advice, not outcome data. Second, approval status is not the same everywhere. Pregabalin is the clearest example: a statement of U.S. approval for one situational use does not transfer to other countries, and it does not mean the drug is appropriate for every cat.

How treatment is usually built

Reduce triggers and support a predictable routine

Merck recommends identifying and avoiding fear-provoking triggers where possible, providing a safe room with resources, establishing a routine, and introducing changes gradually. Desensitization and counterconditioning may be used once the cat is calm enough to take part. Merck is direct on one point: “Positive punishment should be avoided, because it increases fear, anxiety, and aggression in cats.”

Use medication to support the behavior work

Merck describes medication as a way to reduce fear, anxiety, or arousal so that a cat can learn. It also states, “Your veterinarian may, in some cases, prescribe medication to help treat a behavior problem of your cat.” The medication is most useful as one part of a plan that includes environmental changes and behavior work, not as a replacement for them.

Plan situational medication before the stressful event

The AAHA guidance “Medications for fearful dogs and cats,” published May 10, 2019, and the AAFP/ISFM 2022 guidelines both place situational medication in the planning stage, before the visit or trip. If your cat reacts strongly to appointments, ask whether a trial run at home is appropriate, so that you know how your cat responds before the day that matters.

Transport and visits: make the carrier part of the plan

A carrier does not treat anxiety, but it can reduce handling stress and makes a planned medication easier to use. The AAFP/ISFM 2022 guidelines recommend carrier training and low-stress handling.

  • Choose a carrier that opens from the top or has a removable top. The AAFP/ISFM guidelines describe exam handling with these designs.
  • Start carrier training well before the appointment, and leave the carrier out at home so it becomes an ordinary part of the room.
  • Ask your veterinarian whether a synthetic feline pheromone product is worth trying. Merck says these products might help decrease some marking or conflict between cats, but individual benefit is uncertain and they do not replace medication.

Safety: side effects and warning signs

  • General effects. Merck’s owner guidance lists gastrointestinal upset, reduced appetite, vomiting or diarrhea, and temporary lethargy among possible effects. More serious effects are reported rarely.
  • Gabapentin. The AAFP/ISFM 2022 guidelines list sedation and ataxia (unsteadiness) among its effects. Kidney disease can raise gabapentin levels in the blood, so dosing decisions require the veterinarian to know the cat’s health status.
  • Diazepam. Merck reports rare, acute, potentially fatal liver dysfunction in cats, particularly with oral administration. This is the most specific warning in the references and is a reason to ask about liver health before treatment.
  • Disinhibition. Some anxiolytics may reduce a fearful cat’s inhibition and increase confidence or aggression in certain situations, so the behavioral goal and risk should be monitored together.
  • Never substitute. Do not give a cat human medication, or another animal’s prescription, as a shortcut.

Call your veterinarian promptly about unexpected unsteadiness, persistent vomiting or diarrhea, marked lethargy, or a refusal to eat after starting any of these medications. Ask which additional symptoms to watch for with the specific drug prescribed.

Questions to bring to the appointment

  • Has a medical cause been ruled out, and does my cat need a physical exam or lab work first?
  • Is this medication for an ongoing problem, or for one predictable event?
  • If it is for a visit, when should it be given, and should we trial it before the day?
  • How long before I should expect an effect, and what should I see in the first few weeks?
  • Does my cat’s kidney or liver health affect which option is safe?
  • Which environmental changes and behavior steps will we work on alongside the medication?
