9 Cat Behavior Changes to Worry About—and When to Call a Vet

Hiding, new aggression, changes in litter-box habits, and other behavior shifts can signal a health issue. Learn what to track and when to seek urgent veterinary care.

Is your cat sick, or is it just old age? You can’t tell from behavior alone: pain, illness, stress, reduced mobility, and sensory changes can look alike. A new or unusual pattern deserves a call to your veterinarian, even if it appears gradually in an older cat. One exception is urgent: repeated attempts to urinate that produce no urine or only drops need immediate veterinary care.

What behavior changes should you watch for?

Compare your cat with their usual routine, not with another cat or an assumed “normal” for their age. These nine groups describe observable changes, not diagnoses. Any one can have several possible explanations, so note when it began, how often it happens, and what else has changed.

1. Hiding more or withdrawing

A cat that hides more, avoids familiar people, or stops using a usual resting place may be unwell or in pain. Hiding by itself does not identify a particular condition.

2. New irritability, hissing, or aggression

A cat who suddenly resists handling, hisses, or reacts aggressively toward people or other animals may be uncomfortable. Pain or reduced mobility can change how a cat responds. Don’t assume it is a personality problem before discussing the change with a veterinarian.

3. More vocalizing, especially at night

New or increased nighttime meowing can have several explanations in older cats, including sensory decline, pain, hyperthyroidism, hypertension, or cognitive dysfunction. Meowing alone is not enough to diagnose cognitive decline.

4. Litter-box avoidance or house-soiling

Urinating or defecating outside the box is not automatically a training problem. Urinary disease, constipation, diabetes, pain, weakness, impaired balance, or difficulty reaching the box can contribute. Tell your veterinarian about a new pattern rather than punishing your cat.

5. Repeated litter-box visits, straining, or little to no urine

Repeated unproductive attempts to urinate, no urine, or only drops are an emergency: seek immediate veterinary care rather than waiting for a routine appointment. A urinary obstruction may be involved. Urinary straining can also be mistaken for constipation, so don’t try to diagnose the cause at home.

6. Eating or drinking differently

Notice whether your cat is eating more or less, or drinking more or less than usual. If you are unsure, track food and water intake rather than relying on how full the bowl looks. These details can help your veterinarian assess the change.

7. Sleeping more or becoming less active

More sleep, less activity, or abandoning play can accompany illness or mobility problems. Don’t dismiss a new pattern as an inevitable part of aging.

8. Less jumping, climbing, or reaching a favorite place

A cat who stops jumping onto a bed, climbing stairs, or reaching a familiar perch may be experiencing joint pain, muscle weakness, or another mobility problem. Pet stairs, steps, or a ramp may help provide a safer route to a favorite spot, but they do not address the cause of the change. Ask your veterinarian about the change as well.

9. A change in grooming

Reduced grooming may occur with illness, bladder pain, joint pain, or reduced mobility. Increased grooming can also be associated with skin or systemic conditions. Describe what has changed without assuming it points to one diagnosis.

What should you record before contacting the veterinarian?

A short, factual record can help your veterinarian assess what is happening. Note the date the change began, how often it occurs, and whether it is getting better, worse, or staying the same. Include other differences you have noticed, such as:

  • Appetite and water intake
  • How often your cat urinates or defecates, and whether the amount has changed
  • Grooming, jumping, climbing, and general mobility
  • Sleep, play, hiding, and interactions with people or animals

Share the record with your veterinarian and mention any changes that happened around the same time. A behavior change is a health clue, not a diagnosis; a veterinary assessment is needed to investigate its cause.

Why “just getting old” is not a safe explanation

Older cats can develop changes in health, movement, and senses, but age alone cannot explain what is happening to an individual cat. As Cornell veterinarian Richard Goldstein, DVM, puts it in “Loving Care for Older Cats”: “Growing older is not a disease.” Contact your veterinarian about new or unusual behavior, including a change that develops slowly.
