Urine on the bedding does not always mean an elderly dog has “forgotten” its toilet training. Incontinence is involuntary leakage: urine may pool beneath a sleeping dog, dribble while it walks, or appear without the dog seeming aware of it. Other accidents may result from infection, pain, increased urine production, mobility problems, cognitive decline, diarrhea, or difficulty emptying the bladder.
New leakage deserves a veterinary examination rather than being dismissed as a normal part of aging. In the meantime, you can protect your dog’s skin, reduce mess, and make toileting easier and safer.
1. Check what kind of accident is happening
Note exactly when and how the accident occurs:
- Leakage during sleep or relaxation: a wet patch under the dog or urine dripping without awareness is consistent with urinary incontinence.
- Dribbling while walking: this can occur with sphincter weakness, nerve problems, or an abnormal urinary tract.
- Frequent attempts to urinate: infection, bladder inflammation, stones, pain, or an obstruction may be involved.
- Large volumes of urine and increased thirst: kidney disease, diabetes, certain medications, or other conditions may be increasing urine production.
- Straining with little output: the dog may not be emptying its bladder properly. Leakage does not prove that the bladder is empty.
- Stool loss: fecal incontinence has different causes, including diarrhea, intestinal disease, anal or rectal problems, and nerve damage.
Do not punish the dog. If the loss is involuntary, it cannot control the accident, and punishment can add fear without solving the medical problem.
2. Arrange a veterinary appointment
Contact your veterinarian about any new or worsening incontinence in an older dog. Common causes of urinary leakage include:

A veterinary workup may include a history, physical and neurologic examination, observation of urination, urinalysis, and possibly a urine culture. Depending on the findings, the veterinarian may recommend blood tests, X-rays, ultrasound, catheterization, cystoscopy, or specialized bladder testing.
What to record before the appointment
- Write down when the leakage occurs: asleep, lying down, walking, after drinking, or during excitement.
- Record urine volume, color, odor, and whether there is blood.
- Note thirst, appetite, weight changes, straining, pain, licking, and the frequency of toilet breaks.
- Watch for changes in gait, hind-leg strength, tail carriage, or bowel control.
- List every medication and supplement the dog receives.
- Take a short video of an attempt to urinate if it is safe to do so. This may help distinguish leakage from difficulty emptying the bladder.
3. Know when it is an emergency
Seek emergency veterinary care if your dog:
- Cannot urinate
- Repeatedly strains but produces little or no urine
- Cryies, shows severe pain, or becomes distressed while urinating
- Has significant blood in the urine
- Has a swollen or painful abdomen
- Becomes weak, collapses, or vomits repeatedly
- Develops sudden hind-limb weakness, paralysis, or loss of tail or anal tone
A complete urinary obstruction can become life-threatening. A dog can still leak urine when its bladder is not emptying normally, so apparent dribbling should not reassure you if the dog is straining or producing very little urine.
Blood in the urine, painful urination, frequent attempts, or a major change in urinary habits also warrants prompt veterinary attention, even if the dog otherwise seems well. Some urinary infections cause few outward signs, especially in dogs with diabetes, kidney disease, hyperadrenocorticism, or relevant medication histories.

4. Follow treatment for the underlying cause
Treatment depends on whether the dog has sphincter weakness, infection, stones, metabolic disease, neurologic disease, prostate disease, an obstruction, or another problem.
For urethral sphincter mechanism incompetence, a veterinarian may prescribe phenylpropanolamine or, for appropriate female dogs, an estrogen medication such as estriol. Some dogs need a combination. Dogs that do not respond adequately to medication may be evaluated for urethral bulking injections or surgical placement of an artificial urethral sphincter.
Phenylpropanolamine is a prescription veterinary drug in the United States. The FDA approved the first generic phenylpropanolamine hydrochloride chewable tablets for canine urinary incontinence on June 11, 2024, in 25-, 50-, and 75-mg strengths. Its approved indication is urinary incontinence caused by urethral sphincter hypotonus; it is not a general treatment for unexplained urine leakage.

Phenylpropanolamine can cause excitability, restlessness, reduced appetite, or high blood pressure. Estrogen medications can cause hormonal adverse effects and require appropriate veterinary selection and monitoring. Never give a human decongestant, leftover prescription, or compounded medication unless your veterinarian has specifically prescribed and instructed you to use it.
5. Protect the dog while the cause is being investigated
- Increase toilet breaks. Take the dog out more frequently, particularly after sleeping, eating, drinking, and exercising. Keep the route short and easy to reach.
- Improve footing. Clear obstacles and use nonslip rugs or flooring if arthritis, weakness, or poor balance makes walking difficult.
- Protect bedding. Use washable pads or waterproof covers beneath the bed. Keep spare bedding available so a wet bed can be replaced immediately.
- Clean the skin. Gently clean urine from the hindquarters, vulva or prepuce, and inner thighs, then dry the area thoroughly. Urine left against the skin can cause urine scalding and dermatitis.
- Manage the coat. If hair is heavily soiled, ask a veterinarian or experienced groomer about a safe trim. Do not force grooming if the dog is painful or difficult to handle.
- Monitor for skin problems. Redness, sores, hair loss, odor, discharge, persistent licking, or apparent pain needs veterinary attention.
Using a dog diaper safely
A diaper can contain urine temporarily, but it does not diagnose or treat incontinence. Choose one that fits snugly without chafing, restricting movement, or pressing tightly on the abdomen. Check it frequently and change it whenever it is wet or soiled. Remove it every few hours to let the skin dry and take it off before the dog goes outside.

Common misconceptions to avoid
- “It is just old age.” New leakage can be caused by treatable disease.
- “A urinary infection would make my dog obviously sick.” Some dogs have minimal outward signs.
- “Only spayed females become incontinent.” Sphincter weakness is common in spayed females, but males, intact females, and dogs with infectious, neurologic, anatomic, metabolic, or obstructive disease can also leak urine.
- “A diaper fixes the problem.” It only contains waste and can cause skin injury if left wet or soiled.
- “If urine is leaking, the bladder must be emptying.” Overflow leakage can happen when urine cannot pass normally.

Is incontinence in an elderly dog always caused by aging?
No. Urine leakage may result from sphincter weakness, infection, stones, obstruction, kidney disease, diabetes, prostate disease, neurologic problems, or an anatomic abnormality. A new problem should be assessed by a veterinarian.
Should I put my incontinent dog in a diaper?
A diaper can be used as a temporary sanitary aid. It must fit without chafing, be checked and changed frequently, and be removed every few hours so the skin can dry. It should not replace diagnosis or treatment.

What medications can treat canine urinary incontinence?
Depending on the cause, a veterinarian may prescribe phenylpropanolamine or an estrogen medication such as estriol. The correct treatment depends on the diagnosis, and human decongestants or leftover prescriptions should not be given.
