Dog Bite or Cat Bite: How Wound Risks and Treatment Differ

Cat and dog bites cause different wound patterns, but the wound’s depth, location, damage, and your health determine the risk and next steps.

A cat bite is not automatically more dangerous than a dog bite, and the animal alone cannot determine what care you need. Cat teeth can leave narrow punctures that look small but may be deep; dogs can cause punctures, crushing, tearing, and substantial tissue damage. The wound’s depth, location, contamination, your health, and possible rabies or tetanus exposure matter most. Wash a minor bite promptly. Seek urgent medical advice for a deep wound, a bite to the hand, foot, face, or head, infection signs, or uncertainty about rabies; uncontrolled bleeding or severe injury needs emergency care.

How the wounds differ—and why appearance can mislead

Cat bites

A cat’s narrow teeth can make a small surface mark while penetrating deeply. A puncture can be difficult to assess by appearance alone, so consider the wound’s location and depth, not just its size.

Dog bites

Dog bites may puncture the skin, but they can also crush or tear tissue and cause significant damage. A visibly larger wound is not the only concern: a puncture, contamination, or injury to an important structure can also make a bite serious.

Guidelines assess the wound itself—whether skin was broken and blood drawn, how deep it is, whether it is punctured or crushed, the amount of tissue damage, and whether bone, a joint, tendon, or blood vessel may be involved. These factors are more useful for judging risk than a simple cat-versus-dog label. NICE bite-wound recommendations

When to seek medical care

Get urgent medical advice if a bite is on the hand, foot, face, or head; is deep, punctured, or contaminated; or is becoming hot or swollen. Fever or chills, fluid or pus, and an unpleasant smell can indicate infection. Seek prompt assessment if you have diabetes, a weakened immune system, no spleen, or serious liver disease. A clinician can judge whether the wound needs cleaning, antibiotics, or other care. NHS guidance on animal and human bites identifies high-risk sites and infection signs; CDC dog-bite guidance lists uncontrolled bleeding, extreme pain, and loss of function among signs of a serious wound.

  • Emergency care: A large or deep wound, bleeding that will not stop, severe injury, or loss of function needs emergency attention.
  • Prompt medical advice: Contact a clinician for a bite to a high-risk site, a puncture or crush injury, possible deep penetration, infection signs, or a bite when you have a relevant health condition.
  • Also ask promptly: If you are unsure whether the animal could have exposed you to rabies, contact a clinician or public-health authority rather than waiting for symptoms.

Do you need antibiotics after a bite?

Not every bite needs preventive antibiotics. Recommendations differ by country and by wound details. NICE guideline NG184, published in the UK on 4 November 2020, gives these recommendations:

Wound under consideration NICE recommendation
Cat bite that breaks skin and draws blood Offer antibiotic prophylaxis. NICE recommendation 1.1.8 states: “Offer antibiotic prophylaxis (see the recommendations on choice of antibiotic) to people with a cat bite that has broken the skin and drawn blood.”
Cat bite that breaks skin but does not draw blood Consider prophylaxis if the wound could be deep.
Dog or other traditional-pet bite that does not break skin, or breaks skin without drawing blood Do not offer prophylaxis.
Dog bite that breaks skin and draws blood Offer prophylaxis if it penetrates bone, a joint, tendon, or a blood vessel; is deep, punctured, or crushed; causes significant tissue damage; or is visibly contaminated.
Bloody dog bite involving a high-risk site or a person with a relevant comorbidity Consider prophylaxis. NICE high-risk sites include hands, feet, face, genitals, skin over cartilage, or areas with poor circulation; relevant conditions include diabetes, immunosuppression, asplenia, and decompensated liver disease.

These are UK recommendations, not a universal prescription. In the United States, the Infectious Diseases Society of America’s 2014 guideline recommends 3–5 days of preventive antimicrobial therapy for bite-wound patients who are immunocompromised, asplenic, have advanced liver disease or swelling in the affected area, have a moderate-to-severe injury (especially to the hand or face), or may have penetration of the periosteum or joint capsule. It does not say every dog bite needs antibiotics. IDSA skin and soft-tissue infection guideline

If a bite is already infected—for example, with increasing pain or inflammation, fever, discharge, or an unpleasant smell—NICE recommends antibiotics. It recommends a wound swab when discharge is present. A clinician should decide whether antibiotics are appropriate and which treatment to use; do not take leftover antibiotics or choose a prescription based only on the animal species.

What to do immediately after a bite

For a minor wound

  1. Wash the wound thoroughly with soap and water.
  2. Cover it with a clean bandage.
  3. Watch for increasing pain, inflammation, heat, swelling, fever, discharge, or an unpleasant smell, and seek medical advice if these occur.

This first aid is for minor wounds. Mayo Clinic’s animal-bite first-aid guidance recommends thorough cleaning and a clean dressing for a minor bite.

For a deep or serious wound

  1. Apply pressure with a clean, dry cloth to help control bleeding.
  2. Seek medical attention. Get emergency help for bleeding that will not stop, a severe injury, or loss of function.
  3. Let a clinician assess whether the wound needs irrigation or removal of damaged tissue, whether it should be closed, and whether infection, tetanus, or rabies precautions are needed.

Do not rely on home first aid to treat a serious bite. NICE recommends assessing the need for wound irrigation and debridement and evaluating tetanus and rabies risk.

How tetanus and rabies are assessed

Tetanus

Animal bites are considered dirty or major wounds for tetanus management because saliva can contaminate them. Whether you need a tetanus vaccine depends on your vaccination history and the wound; ask a clinician to check your record. Antibiotics are not a way to prevent tetanus after a wound. Rabies

Rabies risk depends on the animal and the circumstances of the exposure, including local risk and whether the animal can be assessed. If the exposure may be a risk, contact a clinician or public-health authority promptly; do not wait for symptoms. For people not previously vaccinated, CDC describes post-exposure prophylaxis (PEP) as wound washing, human rabies immune globulin, and rabies vaccine. Its clinical guidance describes vaccine doses at the initial visit and on days 3, 7, and 14; a clinician or public-health authority should determine what is appropriate for an individual exposure. What infection figures can—and cannot—tell you

CDC says that “Nearly 1 in 5 people bitten by a dog requires medical attention.” The page does not state a publication year, and this figure describes medical attention after dog bites—not infection rates or a comparison with cat bites. CDC dog-bite guidance

IDSA’s 2014 guideline reports a 16% cumulative incidence of infection in a meta-analysis of eight randomized dog-bite trials, and a relative risk of 0.56 among patients treated with antibiotics compared with controls. Those are historical pooled dog-bite findings, not a current universal infection rate or a cat-versus-dog comparison. The cited guidance notes that preventive antibiotics may have only marginal benefit for low-risk dog bites seen within 12–24 hours. IDSA guideline

The cited guidance does not establish a reliable head-to-head infection percentage for all cat bites versus all dog bites. Rates can vary with the wounds included, how infection is defined, wound location, patient factors, and care received, so the dog-bite trial estimate cannot be used as a cat-bite statistic.
