Ataxia in Cats: Causes, Symptoms, and Treatment

Ataxia is impaired coordination, not a diagnosis. Learn the signs cat owners may notice, possible causes, when to seek veterinary care, and how treatment is chosen.

Ataxia is impaired coordination—not a diagnosis. A cat may look wobbly, stumble, fall, or have trouble placing its paws because of a problem affecting balance, movement coordination, or the pathways that tell the brain where the limbs are. New or unexplained signs need prompt veterinary assessment; treatment depends on the underlying cause.

What ataxia means in cats

Ataxia is difficulty coordinating normal movement. It is different from weakness or lameness, although those problems can occur alongside it. Veterinarians broadly classify ataxia as sensory (proprioceptive), vestibular, or cerebellar, based on the nervous system pathways involved. The categories help guide an examination, but a cat’s appearance alone cannot establish the cause.

Sensory or proprioceptive ataxia

Disruption of pathways that carry information about limb and body position can affect how a cat places its paws. The cat may misplace a limb or seem unsteady, and weakness may also be present.

Vestibular ataxia

The vestibular system helps control balance. Affected cats may have a wide stance, sway, fall, roll, or hold the head tilted. Abnormal eye movements can also occur. Similar signs may result from a problem in the inner ear or from a central neurologic problem, so they do not tell an owner which one is present.

Cerebellar ataxia

The cerebellum helps coordinate movement. A cat with cerebellar dysfunction may take exaggerated, high-stepping strides (hypermetria) and may have head tremors.

These patterns can overlap in what an owner sees. A veterinarian must assess the cat to determine where the problem may be and what is causing it.

Possible causes of ataxia

Ataxia has many possible causes, and the possibilities depend partly on which part of the nervous system is affected. Examples include:

  • Spinal cord problems: trauma, inflammation, infection, tumors, structural abnormalities, or pressure and narrowing affecting the cord.
  • Vestibular problems: middle- or inner-ear infection, trauma, tumors, or idiopathic geriatric vestibular disease.
  • Brainstem or cerebellar disease: infection, inflammation, structural or degenerative disease, tumors, or metronidazole toxicity.
  • Systemic problems: low blood glucose, certain electrolyte abnormalities, abnormal red blood cell counts, or heart, circulation, or respiratory disease.

This is not a checklist for diagnosing a cat at home. A veterinarian uses the history and examination to decide which causes warrant further investigation.

When to contact a veterinarian

Arrange prompt veterinary assessment if your cat develops new or unexplained wobbliness, falling, a head tilt, abnormal eye movements, or difficulty standing. Cornell advises immediate veterinary attention for noticeable gait or balance changes. The right urgency can vary with the presentation, so do not assume that every case can safely wait or that every case has the same outlook.

  • Go to an emergency veterinary facility immediately after a head injury. Minimize your cat’s movement during transport, as Cornell advises.
  • Seek urgent veterinary help if signs follow trauma, worsen rapidly, or leave your cat unable to stand.
  • Contact a veterinarian promptly if your cat may have been exposed to a medication or toxin. Do not start, stop, or change medication without veterinary direction.

How a veterinarian evaluates ataxia

Evaluation typically starts with your cat’s history and physical and neurologic examinations. The veterinarian may ask when the signs began, how they have changed, and whether trauma or medication or toxin exposure is possible. If vestibular disease is suspected, an ear examination with an otoscope may be part of the assessment.

Further tests depend on the suspected location and cause. They may include reflex testing and laboratory tests; advanced diagnostics or referral may be considered when the initial assessment indicates they are warranted. There is no single test pathway that applies to every cat.

Treatment and care at home

Treatment is directed at the cause. Some underlying conditions can be treated; others may be progressive or not curable. Depending on the individual case, care may include pain management, supportive care such as anti-nausea medication, a safer environment, and scheduled reassessment. Only a veterinarian can recommend treatment for your cat; do not give medication or prescribe rest at home on your own.

Make the environment safer while your cat is unsteady

  • Block access to stairs and elevated places where a fall could cause injury.
  • Keep food, water, and the litter box easy to reach.
  • Follow the veterinarian’s instructions and ask what changes in walking, eating, drinking, or litter-box use should prompt a call.

Non-slip floor mats may help improve footing, but they do not treat ataxia and are not a substitute for veterinary care.

Questions to discuss with your veterinarian

Because treatment depends on the cause, useful questions include what neurologic location or cause is suspected, whether the condition is treatable or requires supportive care, and what monitoring or reassessment is needed. Also tell the veterinarian whether your cat can safely walk, eat, drink, and use the litter box; these details help frame the care plan for your cat.
