Cat Diabetes: Symptoms, Diagnosis and Treatment

Weight loss despite a good appetite, increased thirst and frequent urination can signal diabetes in cats. Learn how diagnosis, treatment and monitoring work—and when to seek emergency care.

Weight loss despite a good appetite, increased thirst and increased urination are common signs of diabetes in cats. They are reasons to arrange a veterinary assessment—not a diagnosis: stress can temporarily raise a cat’s blood glucose, so confirmation usually depends on evidence that high glucose is sustained. Treatment is tailored to the individual cat and may involve insulin and diet, or, for a limited group of cats, an oral medication.

What are the symptoms of diabetes in cats?

The signs owners most often notice are weight loss despite a good appetite and drinking and urinating more than usual. Increased appetite can also occur. Cornell University College of Veterinary Medicine describes weight loss despite a good appetite and increased thirst and urination as the two most common signs noticed at home (2024).

These changes can have other causes, so a symptom checklist cannot confirm diabetes. Contact your veterinarian if you notice them, particularly if they persist or worsen.

Who is at greater risk?

Cornell identifies obesity, older age, inactivity, male sex and glucocorticoid use as risk factors. Cornell reports that obese cats are up to four times more likely to develop diabetes than cats at an ideal weight, and estimates that diabetes is diagnosed during the lifetime of between 1 in 100 and 1 in 500 cats (2024). These are Cornell’s reported estimates, not a prediction of an individual cat’s risk.

How is diabetes diagnosed in cats?

Diagnosis requires evidence of sustained hyperglycemia—persistently elevated blood glucose—considered alongside the cat’s signs and test results. A single high reading at the clinic may reflect stress, which can raise glucose in cats. Your veterinarian may therefore repeat testing or use additional evidence rather than relying on one measurement.

Tests your veterinarian may use

  • Blood glucose: Repeated elevated results can help show that hyperglycemia is sustained. AAHA’s 2026 guidelines describe documented hyperglycemia or glucose in the urine on more than one occasion in a nonstressed or home environment as possible evidence.
  • Urinalysis: Glucose in urine can contribute to the diagnosis when interpreted with other findings and the clinical picture.
  • Fructosamine or HbA1C: An elevated result can provide evidence of sustained high glucose. AAHA says fructosamine generally reflects average glucose over the preceding 7–10 days. Its level can be affected by conditions that shorten the lifespan of albumin in the blood, including hyperthyroidism and protein-losing disease, so it is not interpreted in isolation.

Cornell also describes fructosamine as useful when a clinic glucose result is uncertain (2024). The choice and interpretation of tests depend on the cat’s health and circumstances.

What else may be checked?

For a cat with signs suggestive of diabetes, AAHA describes an appropriate baseline evaluation that can include a complete blood count (CBC), chemistry panel, urinalysis with sediment examination and total thyroxine (T4). For newly diagnosed cats, its guidance also discusses blood beta-hydroxybutyrate testing and assessment for concurrent conditions. This is not a fixed testing plan for every cat; the veterinarian selects tests based on the individual patient.

What are the treatment options?

Treatment aims to control high blood glucose and the associated signs, address weight loss and avoid treatment-related low blood sugar. The right approach depends on the cat’s health, other conditions, medication eligibility and how well the plan controls the disease. Cornell describes insulin with dietary therapy as traditional management; AAHA’s 2026 guidance also addresses oral SGLT2 inhibitors for selected cats.

Approach Who it may suit What to discuss with the veterinarian
Insulin with dietary therapy A treatment option for cats whose veterinarian determines insulin is appropriate. Insulin preparations differ in cost, duration and concentration. The prescribed insulin concentration and syringe must match: U-40 insulin requires a U-40 syringe, and U-100 insulin requires a U-100 syringe. Confirm the exact insulin and supplies with the veterinary team. Do not adjust a dose without their guidance.
Oral SGLT2 inhibitor Cornell reports that this class was approved in 2023 for a defined group: newly diagnosed cats that are otherwise healthy and have not previously received injectable insulin. AAHA’s 2026 guidelines emphasize careful patient selection. These medicines increase glucose excretion through the kidneys and are not suitable for every cat or a universal replacement for insulin. Eligibility and labeling can vary by location and change over time; the veterinarian must assess whether a specific cat qualifies under current local labeling.

Diet is part of an individualized plan. Cornell discusses dietary therapy alongside insulin, and food choices should be made with the veterinarian rather than by assuming one diet or brand suits every diabetic cat. The sources do not establish one best treatment for all cats.

How do I monitor my cat at home?

Keep a daily record of the cat’s condition as well as any glucose results. Changes in appetite, thirst, urination, weight, activity or demeanor can help the veterinary team assess how the plan is working. Record medication given, too.

  • Track daily water intake and urine production as consistently as practical.
  • Note appetite, body weight, activity and changes in behavior or demeanor.
  • Record medication as prescribed and share the log with the veterinary team.
  • Coordinate glucose checks with the veterinarian; do not change medication based on a reading without their advice.

Home glucose monitoring can help when coordinated with the veterinary team. AAHA’s 2026 guidance says a veterinary-calibrated glucometer may be the most practical and accurate way to monitor blood glucose serially. It does not recommend human-calibrated meters for cats because their readings can be inaccurate. Ask the clinic which meter and compatible strips to use. AAHA’s guideline summary also says in-hospital blood glucose curves are not recommended for cats.

Which complications need urgent veterinary care?

Suspected low blood sugar

Low blood sugar (hypoglycemia) can be serious. Cornell lists weakness, lethargy, vomiting, poor coordination, seizures and coma as possible signs (2024). Contact a veterinarian immediately if you suspect hypoglycemia.

If the cat is alert and able to swallow, offer its regular food. If it will not eat, Cornell advises oral glucose such as honey, corn syrup or dextrose gel and immediate veterinary care. Do not put fingers, food or liquid into the mouth of a cat that is convulsing or comatose; seek emergency veterinary help.

Diabetic ketoacidosis

Diabetic ketoacidosis is a medical emergency that requires hospital care. If it is suspected, seek emergency veterinary attention rather than trying to manage it at home.
