6 Antihistamines for Cats: Uses, Reference Dosages, and Side Effects

Antihistamines may help some cats with histamine-related symptoms, but results vary. See six veterinary-reference options, dose figures, cautions, and poisoning signs.

Antihistamines may ease histamine-related signs such as itchiness or hives in some cats, but their benefit is variable and may be limited. A veterinarian should identify the likely cause, select the medicine and formulation, and confirm the dose for your cat. The six drugs below appear in veterinary references; they are not interchangeable home-treatment options.

What antihistamines can—and cannot—do for cats

Antihistamines block histamine, a substance involved in signs such as itching and hives. They may help selected cats, but they do not diagnose or treat every cause of skin irritation. Fleas, infections, food reactions, and other conditions can look similar and need different care.

For feline atopic skin syndrome, the Merck Veterinary Manual describes antihistamine benefit as small and limited in some cats. Responses vary, and several trials may be needed to find an option that helps. Merck states: “Responses to antihistamines vary considerably, and several trials may be necessary to find one effective for an animal.” Merck Veterinary Manual, Antihistamines for Integumentary Disease in Animals.

Chlorpheniramine is specifically identified as a first-line antihistamine option in Merck’s guidance for feline atopic skin syndrome. That designation applies to that condition; it does not establish that chlorpheniramine is the best choice for every cat or that any of the six drugs has a proven overall efficacy ranking.

Six antihistamines listed in veterinary references

The figures below reproduce the feline entries in the Merck Veterinary Manual dosage table, accessed in 2026. They are reference information, not a prescription or individualized dosing directions. The table does not state a publication year. Your veterinarian must determine whether a drug is appropriate and resolve the dose, interval, route, and formulation for your cat and the condition being treated.

Fexofenadine 10–15 mg per cat by mouth every 12–24 hours as needed Do not assume a human product or formulation is suitable for a cat.

All six dose entries are from Merck Veterinary Manual’s Antihistamine Dosages for Integumentary Disease in Animals. The additional chlorpheniramine values are from its First-Generation Antihistamines and Feline Atopic Skin Syndrome entries. Because the tables give different chlorpheniramine schedules, do not select one based on the table alone; ask the prescribing veterinarian which schedule applies.

Possible side effects and health cautions

First-generation antihistamines can cause drowsiness and gastrointestinal effects such as vomiting or diarrhea. Merck also describes anticholinergic effects, including dry mouth and urinary retention, and cautions that some health conditions may affect whether a medicine is suitable. Sedation was the most commonly reported adverse effect in the antihistamine studies cited in Merck’s feline atopic skin guidance.

Second-generation H1 blockers are less likely to cross the blood-brain barrier, but Merck’s pharmacology review says they have not proved useful to date for controlling pruritus in small animals. Some second-generation drugs also raise cardiotoxicity concerns at high doses. “Newer” does not mean reliably effective or risk-free for cats. A veterinarian needs to consider your cat’s other illnesses and medicines as well as the intended treatment.

Overdose can cause central nervous system hyperexcitability and may be fatal. Do not increase a dose, repeat a dose early, or combine antihistamines unless your veterinarian directs you to do so.

Human allergy and cold medicines can be dangerous

Human over-the-counter products sold for allergies, colds, or sinus symptoms may combine an antihistamine with other active ingredients. A product that contains an antihistamine is not automatically safe for a cat. Check the exact product and ingredients with a veterinarian before giving anything.

Possible poisoning signs described by Merck include depression or hyperactivity, drooling, rapid breathing, rapid heartbeat, gastrointestinal upset, poor coordination, tremors, fever, and seizures. If your cat may have swallowed a human medicine or develops a concerning reaction, contact a veterinarian promptly. Do not wait for symptoms and do not induce vomiting unless a veterinary professional directs you to.
