A diaphragmatic hernia is an opening or tear in the diaphragm that lets abdominal organs move into a cat’s chest. It can follow trauma, such as being struck by a vehicle, or be present from birth. Because the displaced organs can interfere with breathing, a cat with labored breathing or suspected serious trauma needs immediate veterinary assessment. Imaging confirms the diagnosis, and surgery is usually the definitive treatment once the cat is stable enough.
What is a diaphragmatic hernia in a cat?
The diaphragm is the muscular barrier between the chest and abdomen and plays an important role in breathing. A defect in it can allow organs such as the stomach, liver, or intestines to move into the chest. This can limit lung function and, depending on the defect and the organs involved, affect the heart or circulation.
Not every cat with this condition has a known injury. Some defects are congenital and may be discovered incidentally, including in cats with few or no obvious symptoms.
What causes it, and what types occur?
Traumatic diaphragmatic hernia
A forceful injury can tear the diaphragm. Vehicle-related trauma is a common acquired cause in cats. Breathing problems may appear soon after an accident, but signs can vary and other injuries may also be present.

Congenital hernia and PPDH
A congenital defect develops before birth. One form, peritoneopericardial diaphragmatic hernia (PPDH), allows abdominal contents to communicate with the pericardial sac around the heart. A cat with congenital PPDH may have no obvious injury history; some cases are found incidentally.
What symptoms should you look for?
Signs depend on the size and location of the defect, which organs have moved, and whether the condition is acute or longstanding. Some cats have mild signs or none at all. Acute cases can worsen quickly.

- Labored, rapid, or otherwise difficult breathing
- Reduced activity, weakness, or collapse
- Reduced appetite or vomiting
- A swollen abdomen, particularly if the stomach is trapped or distended
A veterinarian may find muffled or diminished breath sounds; occasionally, digestive sounds can be heard in the chest. These findings do not confirm a hernia on their own.
When is it an emergency?
Breathing difficulty is an emergency. If your cat is struggling to breathe, or has suffered a serious accident, contact a veterinarian or emergency clinic immediately and follow their directions. Do not try to diagnose the problem, give medication, provide oxygen, or handle or transport a severely distressed cat without veterinary instructions. A trapped, distended stomach can contribute to rapid deterioration.

How do veterinarians diagnose it?
Diagnosis draws on the injury history, physical examination, and imaging. Merck Veterinary Manual states that “Thoracic radiographs are usually diagnostic” in its Diaphragmatic Hernia in Animals reference (full review May 2022; last updated September 2024). X-rays may show abdominal organs in the chest, displacement of organs, or loss of the normal outline of the diaphragm.
When radiographs do not make the diagnosis clear, a veterinarian may use ultrasound or consider other imaging, such as contrast studies, CT, or MRI. Fluid in the chest can make images harder to interpret. Blood work, blood gas analysis, and an ECG may help assess how the cat is coping, but do not independently prove the hernia.

How is a diaphragmatic hernia treated?
Stabilization comes first
A breathless or unstable cat may need stabilization before surgery. Depending on the cat’s condition, veterinary care may include oxygen, pain relief, cardiovascular support, or drainage if air or fluid in the chest is impairing breathing. The team decides what is needed and when the cat is stable enough for the next step.
Surgical repair
For most cases, surgery is the definitive treatment: the surgeon returns displaced abdominal contents to the abdomen and repairs the diaphragm. Timing and suitability depend on the defect, the cat’s respiratory and cardiovascular stability, which organs are involved, any stomach distention, concurrent injuries, and whether chronic changes or adhesions are present.

When surgery may not be chosen
Some cats with asymptomatic PPDH or cats that are poor surgical candidates may be managed medically. That decision requires case-specific veterinary judgment; it is not a home treatment or a substitute for assessment when signs are present.
What happens after surgery?
Hospital monitoring and pain management are typical after repair. Re-expansion pulmonary edema, a potentially life-threatening lung complication, is a particular concern discussed in chronic cases. The early postoperative period therefore warrants close monitoring; VCA describes the initial prognosis after surgery as guarded. Recovery time varies with the cat’s condition and complications, so there is no single timeline that applies to every patient.
What affects the prognosis?
Prognosis depends on whether the hernia is traumatic or congenital, the cat’s condition before treatment, the organs involved, and any concurrent injuries. The American College of Veterinary Surgeons says the prognosis for traumatic hernia is variable depending on other injuries incurred. It describes outcomes for surgically treated congenital PPDH as generally favorable, with low mortality and excellent return to normal function. Those broad descriptions do not predict an individual cat’s outcome; the treating veterinarian can explain the relevant risks in that case.
There is no broadly applicable prevalence or survival figure established by the owner and clinical guidance cited here. A 2022 retrospective hospital case series reported trauma as the most common cause among its enrolled cats, but selected hospital cases are not a population estimate. A separate retrospective series covered 34 cats treated during 1991–2001; its PubMed abstract does not provide outcome figures, so its sample size should not be read as a survival rate.
