Sanitation training reduces disease risk when it matches the work: handling sewage requires different protections from cleaning a school or a healthcare facility. Workers need practical instruction in hygiene, task-appropriate protective equipment, safe waste or chemical handling, and the procedures that apply at their site. There is no single sanitation curriculum or set of precautions that fits every setting.
How sanitation training helps control disease
Training helps workers recognize exposure routes and follow safer routines consistently. For people handling human waste or sewage, the Centers for Disease Control and Prevention (CDC) says disease-prevention training should cover hygiene, personal protective equipment (PPE), and proper waste handling. Environmental cleaners need instruction suited to the surfaces, products, and rules in their facility.
The public-health stakes are substantial, but the figures below are dated estimates, not current-year counts. The World Health Organization (WHO) reported in its 2024 WASH and health compendium that its 2016 estimate attributed 432,000 diarrhoeal disease deaths globally each year to sanitation deficiencies and 165,000 diarrhoeal deaths to inadequate hygiene behaviours. The same compendium reported a WHO estimate that approximately 2.3 billion people lacked functioning handwashing facilities with water and soap in 2020.
Training for workers handling human waste or sewage
CDC guidance dated April 22, 2024, says: “All workers who handle human waste or sewage should receive training on disease prevention.” The training should turn that principle into safe work routines rather than rely on general reminders.

Hygiene and exposure prevention
- Wash hands with soap and water after handling waste and after removing PPE.
- Avoid touching the face, mouth, eyes, or open wounds while handling waste; cover cuts before work.
- Know how work clothing and boots are handled after a task so contamination is not carried into clean areas.
- Seek prompt medical assessment if symptoms such as vomiting, stomach cramps, or watery diarrhoea develop.
PPE and waste handling
CDC identifies waterproof gloves, rubber boots, goggles, face protection, and liquid-repellent coveralls as protective equipment relevant to this work. Selection and use should follow a site-specific hazard assessment; the list is not a universal specification for every task. Training should include safe donning, use, removal, and disposal of PPE, as well as the site’s waste-handling procedures.
CDC advises that local risks vary and that site-specific plans be developed with a trained health and safety professional. It also advises consulting local health authorities about vaccination recommendations: tetanus should be up to date, while other vaccines depend on local guidance and risk. This is not a blanket vaccination schedule.

Training for environmental cleaning in healthcare
Healthcare environmental cleaning operates within facility infection-prevention and control (IPC) and water, sanitation, and hygiene (WASH) procedures. WHO’s environmental-cleaning training package is designed for healthcare-facility cleaners and includes a trainer’s guide, learner modules, and supporting resources. WHO says, “Those who clean are the first line of defense against health care-associated infections (HAIs), and support efforts to reduce antimicrobial resistance (AMR).” The package can inform training design, but it is not automatically a curriculum for sewage workers or community cleaners.
WHO’s 2024 operational guide for infectious diarrhoea in healthcare settings addresses WASH infrastructure and waste management, food safety, and infection-prevention measures for suspected infectious diarrhoea and healthcare-associated cases. Healthcare facilities should use applicable facility guidance and protocols rather than substitute general community-cleaning advice.

Training for community and education facilities
CDC’s community-facility cleaning guidance is not intended for healthcare settings or other regulated settings with specific requirements. For most community settings, CDC says regular cleaning is sufficient; disinfection is advised when someone has been sick or is at higher risk. Workers should follow product-label directions, understand chemical hazards, ensure adequate ventilation, store products safely, and use PPE appropriate to the Safety Data Sheet. Cleaning chemicals should never be mixed.
For K–12 schools and early-childhood settings, CDC offers the free “Clean Hands and Spaces” interactive training in English and Spanish. Its four modules cover handwashing, hygiene planning, selection, use and storage of cleaning products and disinfectants, and working with parents and caregivers. CDC lists an estimated completion time of two to three hours, depending on role. This is a setting-specific training, not a general course for sanitation workers.

How to choose or adapt a training program
Before adopting a resource, check that it prepares workers for their actual tasks and workplace requirements. A package designed for healthcare cleaners, for example, may not cover the exposure controls needed by a sewage worker.
- Setting and rules: Identify whether the work is sewage handling, healthcare cleaning, community cleaning, or education-facility cleaning, and determine which local requirements apply.
- Tasks and exposure routes: Match instruction to the waste, surfaces, chemicals, and likely ways workers could be exposed.
- Who the resource trains: Check whether it is a trainer’s guide, learner course, or both, and whether learners need additional site-specific instruction.
- Language and delivery: Confirm that workers can access and understand the material in its available language and format.
- Practical skills: Ensure workers can practise relevant PPE procedures and chemical-safety routines, not just review written information.
- Local adaptation: Have procedures checked against local public-health and occupational-safety guidance and the facility’s own plans.
What training cannot replace
Training is one part of disease prevention. It cannot compensate for missing handwashing facilities, unsuitable PPE, unsafe work processes, or unclear responsibilities. The appropriate equipment, procedures, chemical products, and vaccination advice depend on the task, workplace, and local authorities; facility-specific IPC procedures govern healthcare work.
