Alzheimer’s in Cats: Causes, Symptoms & Treatment

Feline cognitive dysfunction can cause behavior and sleep changes in older cats, but similar signs may come from treatable illness, pain, or sensory loss. Here’s what to watch for and how veterinary assessment and home support can help.

“Alzheimer’s in cats” usually refers to feline cognitive dysfunction syndrome (CDS), an age-related decline in cognitive function. It can cause changes in an older cat’s sleep, behavior, and daily habits, but those signs do not confirm dementia: pain, sensory loss, and illnesses such as kidney disease or hyperthyroidism can look similar. A veterinarian needs to assess the cat and investigate other possible causes.

What “Alzheimer’s in cats” means

Feline cognitive dysfunction syndrome is the veterinary term for age-related cognitive decline. Researchers have described similarities between feline CDS and human Alzheimer’s disease, including findings such as beta-amyloid and hyperphosphorylated tau deposits in the brain. That resemblance does not make the conditions identical, nor does it establish a single cause for behavioral changes in an individual cat. A 2021 review in Veterinary Record discusses the neuropathological and behavioral evidence.

Age can make CDS a consideration, but it cannot diagnose it. In its 2021 senior-care guidelines, the American Association of Feline Practitioners (AAFP) cites evidence that over 25% of cats 11 years or older had certain behavioral changes with no recognized medical cause. This is not a confirmed-CDS prevalence estimate.

Signs of cognitive dysfunction in cats

Possible signs include changes in orientation, sleep, social behavior, activity, vocalizing, and toileting. They vary between cats, and none is specific to CDS.

  • Possible memory or learning changes: difficulty with familiar routines or previously learned behaviors.

One clinical observation is not a diagnostic rule. Cornell University’s Feline Health Center attributes this description to Alexander de Lahunta, DVM, former emeritus professor of anatomy at Cornell: “when examining a cat, if it is acting as if it’s in a world of its own, doesn’t respond to me, paces in a circle, or gets itself in a corner and just stays there, I’ll consider that to be a prosencephalon problem.” A brain problem is one possibility among several, so such behavior warrants assessment rather than a home diagnosis.

Other conditions can look like CDS

Behavior changes may have a medical, sensory, or environmental explanation, and multiple issues can be present at once. Cornell’s Cognitive Dysfunction guidance, reviewed in 2024, and the AAFP guidelines identify possible overlapping causes and contributors:

  • Arthritis, dental pain, or other sources of discomfort
  • Hearing or vision loss
  • Neurological disease or structural problems in the brain
  • Environmental changes that affect the cat’s routine or sense of security

A sudden change, or one accompanied by changes in thirst, appetite, vision, mobility, or apparent pain, deserves particular attention. These clues can help guide an examination, but no single behavior reliably separates CDS from another diagnosis.

How veterinarians assess a cat with behavior changes

CDS is generally a diagnosis of exclusion: the veterinarian considers the history and examination, then investigates other plausible explanations. Testing depends on the cat’s signs and clinical findings. Assessment may include a physical examination, blood-pressure measurement, blood and urine tests, and further diagnostics when indicated. The AAFP recommends structured questions to identify possible cognitive changes and stresses that medical and environmental causes must be considered.

Before the appointment, keep a dated record of changes in behavior, appetite, sleep, mobility, and litter-box habits. Short videos of everyday routines can make gradual changes easier to spot; the AAFP recommends reviewing pictures and videos serially. Note when each change began and whether it is getting better, worse, or staying the same.

Treatment and support at home

There is no single cure established in the veterinary sources discussed here. Care is individualized: addressing a diagnosed medical problem is different from making the home easier to navigate. The veterinarian can help determine which changes are suitable for your cat.

Make everyday resources easier to reach

Keep food, water, resting places, and litter boxes convenient and predictable. A low-entry, low-sided litter box may help an older cat that has trouble reaching or entering its usual box; ramps may help when stairs are difficult. These adjustments support access and comfort, but they do not treat cognitive dysfunction. House-soiling or difficulty using the box should still be discussed with a veterinarian.

Keep routines familiar and offer appropriate enrichment

Reliable routines and a comfortable, low-stress environment may support an older cat. Environmental optimization and enrichment are discussed in veterinary guidance, but what is appropriate depends on the cat’s health, mobility, and response. Observe whether a change seems to help or causes distress.

Discuss diets, supplements, and medication with the veterinarian

Veterinary literature discusses specific diets, supplements, calming pheromones, and medications as possible management options. When to arrange veterinary care

Arrange an appointment when an older cat develops new or worsening disorientation, nighttime vocalizing, sleep changes, altered social behavior, or house-soiling. A prompt assessment can identify treatable conditions or pain that might otherwise be mistaken for cognitive decline. Bring your observations and videos so the veterinarian can see how the changes unfold over time.
