If your cat insurance claim is denied, start with the denial notice: identify the exact policy term the insurer relied on, then compare it with your policy and the dates and records for your cat’s care. Ask for a written explanation if the reason is unclear, submit a documented reconsideration through the policy’s stated process, and contact the insurance department for the state regulating the policy if the dispute remains unresolved. Pet insurance is governed by the specific contract and applicable state law, so appeal procedures and regulator authority vary.
First, find out exactly why the claim was denied
Save the denial notice and note the claim number. If the insurer has not explained its decision clearly, ask in writing for the specific policy wording and facts supporting it. Confirm whether the claim was denied in full or paid only in part.
Collect the policy and any endorsements, the claim form you submitted, the itemized veterinary invoice, relevant medical records, and messages from the insurer or claim portal. The NAIC identifies coverage terms, exclusions, waiting periods, claim procedures, deductibles, limits, and reimbursement methods as important details to understand in pet insurance policies (NAIC pet insurance guidance; NAIC, A Regulator’s Guide to Pet Insurance).
Check which issue the notice cites, then compare it with the actual policy and the timeline of symptoms, diagnosis, and treatment:

- Exclusion or preexisting condition: Find the policy’s definition and identify the records the insurer says support the exclusion.
- Waiting period: Compare the policy’s effective date and waiting-period terms with the dates symptoms appeared and care was provided.
- Documentation or filing issue: Check what information the insurer says is missing and how the policy says to provide it.
- Deductible, limit, or reimbursement calculation: Review the deductible remaining, applicable benefit limits, and the policy’s reimbursement formula against the insurer’s calculation.
Do not assume that a familiar term has the same meaning in every policy. The contract and state law that apply to your policy matter.
Submit a written reconsideration with evidence
Use the appeal or review method and deadline stated in your denial notice or policy. There is no universal internal appeal deadline established for pet insurance, so check both documents promptly and ask the insurer to confirm the deadline and submission method if either is unclear. Do not use health-insurance appeal deadlines as a substitute.

Make the request specific and factual. Identify the decision you are challenging, quote the relevant policy wording, explain why the cited exclusion or limit does not fit the facts, and attach records that support your account. Depending on the issue, useful documents may include:
- The itemized invoice and proof of payment, if relevant.
- Your cat’s veterinary chart, test results, and relevant earlier medical records.
- Records establishing when symptoms began, when the diagnosis was made, or when treatment occurred.
- A veterinarian’s explanation of the diagnosis and medical timeline.
A veterinarian can help clarify medical facts, but cannot determine what the insurance contract covers or guarantee that an appeal will succeed. Keep a complete copy of your submission and a dated log of calls, emails, letters, and portal messages. The NAIC recommends detailed factual accounts and preserving supporting documents and communications when pursuing an insurance complaint (NAIC complaint guidance).

If the insurer cites a preexisting condition
Read the exact definition in your policy and check the medical records the insurer relied on. The NAIC’s 2022 Pet Insurance Model Law defines a preexisting condition in terms of veterinary advice, prior treatment, or directly related signs or symptoms before the policy’s effective date or during a waiting period. Section 5(A) says: “The pet insurer has the burden of proving that the preexisting condition exclusion applies to the condition for which a claim is being made.” The NAIC’s 2025 examination guidance also addresses that burden (NAIC Pet Insurance Model Law; NAIC, Conducting the Pet Insurance Exam).
These NAIC materials are model and examination guidance, not proof that every state has adopted identical rules. Check the law applicable to your policy and use the policy’s own definition when preparing your reconsideration.

Contact your state insurance department if the dispute remains unresolved
If you have tried to resolve the issue with the insurer and are still dissatisfied, you can file a complaint with the insurance department for the state that regulates your policy. The NAIC recommends addressing the problem with the insurer first and directs consumers to their state insurance department for unresolved complaints (NAIC complaint guidance; NAIC, “Need Help with Insurance? Your Insurance Department is Here for You”).
Follow your department’s instructions; filing options and procedures differ by state. Be ready to provide your policy number, policy, denial notice, bills, veterinary records, correspondence, a dated call log, a concise timeline, the policy language at issue, and the resolution you are requesting. A department may send the complaint to the insurer and review its explanation under the policy, but its authority and available remedies depend on the jurisdiction. It is not a substitute for personal legal counsel if you are considering court action.

When to consider legal advice
A state insurance department complaint is one possible escalation route, not a guarantee of payment or a court appeal. If the amount at stake is significant, the dispute involves complex policy language, or you are considering litigation, a lawyer qualified in your state can advise you about your options. Not every denied claim requires an attorney.
