Cleft Palate in Cats: Causes, Symptoms and Treatment

A cleft palate is a birth defect that can connect a kitten's mouth to its nasal passages. Learn the causes, warning signs, feeding safety, surgical timing, and realistic prognosis.

A cleft palate is a birth defect that leaves an opening in the roof of a kitten’s mouth, and sometimes in the lip, that can connect the mouth to the nasal passages. Many affected kittens can be helped, but the outcome depends on where the opening is, how large it is, and whether the kitten can feed and breathe safely while it grows. Repair is possible in appropriate cases, usually once the kitten is older and bigger, and only after a veterinarian has assessed it.

What a cleft palate is and where it occurs

The palate is the roof of the mouth. It has a front part with bone (the hard palate) and a rear, flexible part (the soft palate). Clefts are often described by location, and location strongly affects how much a kitten struggles with feeding and breathing:

  • Cleft lip or primary palate: the opening is at the front of the mouth, in the lip or the front of the roof of the mouth.
  • Hard (secondary) palate: the opening is in the bony roof behind the front of the mouth.
  • Soft palate: the opening is in the flexible tissue at the back of the roof of the mouth.

Size and extent matter as much as location. A small opening may cause few problems, while a wide defect that runs through several areas can make feeding difficult and is harder to close.

What causes cleft palate in cats

A cleft palate is a developmental defect. The Merck Veterinary Manual’s cat-owner guidance describes it as a failure of the jaw and face to form correctly during embryonic development, which can leave an opening between the roof of the mouth and the nasal cavity. The broader Merck reference lists genetic causes, teratogenic causes (exposure to certain medications or toxic substances during pregnancy), and nutritional imbalance as contributors in animals generally. Merck also notes that Siamese cats have a higher incidence of cleft lip than other breeds.

In most individual kittens, the exact cause cannot be identified. Veterinary references do not give a reliable prevalence figure for cats, so this article does not offer one, and it is not possible to say how common the condition is in a given litter or breed.

Symptoms to watch for in kittens

Signs often appear soon after birth or during nursing. The severity depends on the size and location of the cleft. Common signs include:

  • A visible gap in the lip or roof of the mouth
  • Difficulty latching on to nurse, or difficulty swallowing
  • Milk coming out of the nostrils
  • Nasal discharge, congestion, or sneezing
  • Coughing or gagging
  • Signs of a respiratory infection
  • Poor weight gain or slow growth

A kitten showing feeding trouble, nasal discharge, or breathing signs should be examined by a veterinarian promptly rather than observed at home for a long period.

How a veterinarian confirms the diagnosis

Diagnosis usually starts with a careful examination of the mouth. Because a cleft can be seen directly, the defect is often recognized at this stage. The veterinarian will also assess the kitten’s general condition, its feeding, and its breathing.

If pneumonia or another complication is suspected, the veterinarian may recommend further tests, which can include chest imaging. Whether further testing is needed depends on the individual kitten’s presentation.

Feeding a kitten with a cleft palate

Some kittens need intensive feeding support while they grow and until a treatment plan is set. This may involve hand feeding or tube feeding, with close monitoring. The feeding method, the diet, and the amount are decisions for the veterinarian, and they should not be copied from general advice.

PetMD lists milk replacer or an appropriate recovery diet among possible options for interim feeding. No single food or device is appropriate for every kitten, and no brand is endorsed here. Feeding a kitten with a cleft in the wrong way can cause liquid or food to enter the airways (aspiration), which can lead to pneumonia. Tube placement should never be attempted at home.

Surgical repair and timing

Surgery is an option for appropriate cases, but not every cleft can be repaired. Whether a kitten is a candidate depends on its age and size, the location and extent of the cleft, and its general health. A veterinary surgeon with relevant experience should assess a secondary palate defect.

These are general clinical timing ranges, not a fixed deadline. The surgeon decides the right time for each kitten. The American College of Veterinary Surgeons states plainly why repair matters for some cats: “Secondary cleft palates require surgical treatment to prevent long-term nasal and lung infections and to help your pet receive proper nutrition.”

How the repair is done and what recovery involves

The operation may use tissue from the palate to close the defect. The technique depends on the cleft’s anatomy, so the surgeon chooses the approach for that kitten. After surgery, the care team may prescribe soft food or tube feeding during healing, and may restrict hard food and toys. These are instructions for the veterinary team and the owner to follow exactly as given, not a protocol to adapt at home.

Possible complications

  • Breakdown of the repair
  • Continued nasal discharge or sneezing
  • Coughing or gagging
  • Breathing problems caused by swelling

Any of these signs after surgery should prompt a call to the veterinary team.

Prognosis: what affects the outcome

Outcomes are assessed case by case. The factors that matter most are the cleft’s location, its size and extent, any respiratory complications, the kitten’s nutrition and growth, and whether the repair is feasible and heals well. Small clefts generally have a better prognosis. Extensive defects, and very large defects or extensive involvement of the soft palate, can be difficult to repair and have poorer outcomes.

No general success rate for feline cleft palate repair is established in veterinary references, so no cure or typical result can be promised. A 2022 report in the Journal of Feline Medicine and Surgery Open Reports describes one kitten repaired at four months of age. At 12 months after surgery, the cat was in good general condition with no clinical signs, although it had two oronasal fistulae. That is a single case, and it cannot show how most kittens will do. Read it as an example of what is possible, not a measure of success.

Breeding and prevention

Not every case has a known or preventable cause, so prevention cannot be guaranteed. Because genetic factors may contribute, veterinary references advise against breeding affected cats. Merck specifically recommends spaying or neutering them. A pregnant cat should receive no medication or supplement changes unless a veterinarian directs them, since some medications and toxic substances during pregnancy are linked to birth defects.

If you are planning a litter, discuss the cat’s history with your veterinarian before breeding. If a kitten is born with a cleft, a veterinarian should assess it as early as possible.
