Hyperthyroidism in Cats: Symptoms, Treatment & Causes

Feline hyperthyroidism is excess thyroid hormone, usually from benign thyroid enlargement. Learn the signs, how vets diagnose it, the four treatment options, and why kidney checks matter.

Feline hyperthyroidism means a cat’s thyroid glands produce more thyroid hormone than the body needs. It is most often seen in middle-aged and older cats, and in most cases the cause is a benign enlargement of thyroid tissue. Weight loss despite a strong appetite is the classic sign. Four standard treatments exist: radioiodine, antithyroid medication, thyroid surgery, and a prescription iodine-restricted diet. The right choice depends on the individual cat, the owner’s ability to carry out treatment, and access and cost, so it is a decision to make with the cat’s veterinarian.

What hyperthyroidism does in a cat

The thyroid glands sit on either side of the windpipe in the neck. They release hormones, mainly thyroxine (T4), that set the body’s metabolic pace. When the glands overproduce, the body runs fast: energy use climbs, the heart works harder, and the digestive system and coat are affected. Veterinarians call the state of excess hormone thyrotoxicosis, but most owners will hear it described simply as an overactive thyroid.

Causes and who is affected

Feline hyperthyroidism is a disease of older cats, and it is most commonly diagnosed in middle-aged and senior animals. Cornell University College of Veterinary Medicine’s feline hospital page reports that functional benign thyroid enlargement (an adenoma) accounts for about 98% of cases, while thyroid carcinoma accounts for about 2%. That page does not state the year those figures were compiled, so treat them as the figures Cornell publishes rather than newly measured rates. The same page reports that about 70% of affected cats have both thyroid lobes involved.

The underlying cause is not known. Cornell lists possible contributing factors, including dietary components and chronic exposure to thyroid-disrupting chemicals in food or the environment. These are considered possible contributors only; no single cause has been established, and owners cannot prevent the disease by changing one factor alone.

Symptoms to watch for

Signs often develop gradually, and early changes can be easy to miss, especially in a cat that still eats well. The most recognizable pattern is weight loss combined with a voracious appetite. Other possible signs include:

  • Weight loss while eating normally or more than usual
  • Increased thirst
  • Rapid heart rate
  • Poor or unkempt coat
  • Nervousness, restlessness, or hyperactivity
  • Vomiting or diarrhea
  • Increased vocalizing

A cat may show only one or two of these. Because the same signs can accompany other illnesses, a single symptom does not establish a diagnosis, and a new pattern of weight change in an older cat is a reason for a veterinary visit.

How veterinarians diagnose the disease

Diagnosis combines a physical examination with blood testing. Often the veterinarian can feel an enlarged thyroid gland during the exam. Confirmation generally rests on thyroid hormone blood tests, usually measuring total T4.

  1. Examination. The veterinarian checks body condition, heart rate, coat, and the neck for an enlarged thyroid.
  2. Total T4 blood test. An elevated result supports the diagnosis.
  3. Interpreting a normal or borderline result. If the cat still looks suspicious for hyperthyroidism but total T4 is not elevated, concurrent illness may be lowering the number. The veterinarian may repeat the test or add a free T4 measurement before ruling the disease out.
  4. Baseline kidney assessment. Bloodwork that evaluates kidney function is standard before treatment, because kidney status influences treatment choice and follow-up (see the kidney section below).

Treatment options compared

The four standard approaches differ in whether they aim to cure or control the disease, and in how much ongoing work they require from the owner. The table summarizes the main trade-offs.

Radioiodine (I-131)

The American Animal Hospital Association (AAHA) describes radioiodine as the treatment of choice, reporting a cure in more than 95% of cases in its April 12, 2023 guidance on feline hyperthyroidism therapy. It avoids surgery and daily pills. The trade-offs are access and logistics: the cat must be treated at a facility licensed to handle radioactive material, and the cat is kept under restricted contact or isolation for a period set by that facility. Hypothyroidism, meaning too little thyroid hormone, can develop afterward, so thyroid levels need follow-up testing.

Antithyroid medication

Methimazole lowers thyroid hormone production. It does not remove the abnormal thyroid tissue; it keeps hormone levels in check while the medication is given. The U.S. Food and Drug Administration (FDA) notes that methimazole may be used for the cat’s lifetime, or as preparation for radioiodine or surgery, and that the FDA has approved two drugs for treating feline hyperthyroidism. Side effects are possible, and laboratory monitoring is needed to adjust the dose and catch problems early. For many owners, the difficulty is the consistency of giving medication over years, particularly with a cat that resists pills.

Thyroidectomy

Surgery to remove the abnormal thyroid tissue can be curative, but it requires anesthesia. Two complications are specifically noted: difficulty in cats with concurrent disease, and injury to the nearby parathyroid glands, which control calcium regulation. Because the thyroid sits close to these glands, the surgeon’s technique and the cat’s overall health shape the risk.

Iodine-restricted therapeutic diet

A prescription diet with restricted iodine can control the disease in some cats when it is fed exclusively. Hill’s Prescription Diet y/d is an example of this category, named by Cornell’s feline health resources. AAHA describes the diet as effective for many cats. Cornell notes concerns about long-term restriction and says ongoing research continues. The diet only works if it is the sole food source: a few treats, table scraps, or outdoor hunting can provide enough iodine to undo the effect.

Why kidney assessment and monitoring matter

Hyperthyroidism affects several organ systems, and the kidneys are a particular concern. Merck Veterinary Manual and FDA guidance both note that correcting excess thyroid hormone can reveal kidney dysfunction that was masked before treatment, even when earlier indicators looked reassuring. The increased blood flow and filtration that accompany hyperthyroidism can hide underlying kidney weakness, and once hormone levels fall, that weakness may become apparent.

For this reason, kidney testing before treatment and again after it is a standard part of the veterinary plan, not an optional extra. Follow-up thyroid testing is also important, because both over-treatment (leading to hypothyroidism) and under-treatment can occur, particularly with medication or radioiodine.

Questions to settle with your veterinarian

  • Has the cat had a total T4 test, and do the kidney values need to be checked before treatment is chosen?
  • Can you reliably give medication twice daily for life, or would a single treatment with follow-up testing suit your situation better?
  • Is a licensed radioiodine facility within reach, and can you arrange the isolation period it requires?
  • Would the cat accept a prescription diet as its only food, with no outdoor hunting or household treats?
  • Is the cat healthy enough for anesthesia if surgery is considered?
  • What is the schedule for follow-up thyroid and kidney testing after the chosen treatment?

Each of these answers changes which option is practical, and none of the four approaches is a one-size-fits-all choice. Your veterinarian can weigh them against the cat’s age, other health conditions, and your household.
