A thoracotomy is an operation in which the surgeon cuts through the chest wall to reach organs or tissue inside the chest. It is a major procedure, and it is meant to address a specific problem: a lung lobe that is diseased or twisted, a mass, an infection, a penetrating injury, a foreign body in the esophagus, certain lymphatic leaks, or selected conditions around the heart. The word alone does not tell you the diagnosis or the likely outcome. Those depend on what the veterinary team found, so the most useful first step is asking them to explain it in those terms.
Why a surgeon would open the chest
The chest holds the lungs, heart, esophagus, large blood vessels, and the thoracic duct, which carries lymph. Many problems in these areas cannot be fixed or even fully diagnosed from the outside. A thoracotomy gives the surgeon direct access. The incision is either intercostal or lateral, passing between the ribs, or a median sternotomy, which divides the breastbone. The surgeon chooses the approach based on where the problem sits and what needs to be done there.
Once the chest is open, the procedure might remove a lung lobe or a mass, repair damaged tissue, retrieve a foreign body, or operate on structures such as the thoracic duct. Because the operation is planned around the findings, the plan can change once the surgeon sees inside the chest. That is one reason to ask what the operation is intended to treat and whether the plan could shift.
Common reasons a dog may need one
Veterinary references group these indications into broad categories. They explain common patterns, but they do not identify the reason for any individual dog.

- Tumors and masses in the chest. Surgery may be used to remove the mass or obtain a biopsy. A mass may need further imaging such as CT before the team knows its extent. Surgery does not automatically cure a cancer, particularly if it is aggressive or has spread.
- Abscesses and other chest disease. Infection or inflammation in the chest may need direct drainage or removal of damaged tissue.
- Penetrating bite wounds. A bite that penetrates the chest wall can damage the lungs or other structures and may need surgical exploration and repair.
- Lung-lobe torsion. Here, one lobe twists around its airway and blood vessels, which compromises circulation and drainage. In the Vet Help Direct explainer by Emma Chandley, published August 19, 2026, treatment is usually removal of the affected lobe rather than untwisting it.
- Esophageal foreign bodies. Thoracotomy may be needed when an object cannot be removed safely through an endoscope or when the esophagus has perforated.
- Chylothorax. This is a leak of lymph into the chest. When it is persistent or has no identifiable cause, surgery on the thoracic duct may be considered.
- Selected heart and pericardial disease. Some congenital heart conditions and certain problems with the sac around the heart are treated through the chest.
Why not keyhole surgery?
Thoracoscopy uses a camera and instruments through small incisions. It is used more often than it once was and is suitable for some procedures. The University of Florida College of Veterinary Medicine Small Animal Hospital lists lung-lobe excision and pericardectomy among its minimally invasive procedures, and describes thoracoscopic pericardectomy as less painful than thoracotomy. Thoracoscopy is not suitable for every dog, and several factors decide the choice:
- The size and location of the lesion, and whether the surgeon can reach it.
- Adhesions or inflammation that make the tissue hard to see or move safely.
- How much tissue needs to be removed.
- Whether the surgeon can see and manipulate the target well enough to do the job safely.
An open approach may give safer or better access. If visibility or access turns out to be inadequate during a keyhole procedure, the surgeon may convert to an open thoracotomy, and that conversion can be the safer decision. Ask the surgeon whether thoracoscopy is suitable for your dog’s specific problem and, if not, what limits it.

What happens during surgery
General anesthesia is required. Opening the chest disrupts the pressure relationships that normally keep the lungs inflated, so the dog needs assisted ventilation during the operation. The anesthesia team monitors oxygenation, ventilation, and cardiovascular function throughout.
A chest drain is commonly placed at the end of surgery to remove residual air or fluid and help the lungs re-expand. The drain stays in until the veterinary team judges it is no longer needed.

Pain control
Thoracotomy is painful, and pain control matters for more than comfort. Pain that limits breathing or coughing can make lung complications more likely. Veterinary teams typically use several forms of pain relief, tailored to the dog and the procedure, rather than relying on a single medication.
Hospital stay and recovery at home
After surgery, the dog usually stays in hospital for monitoring of breathing, oxygen levels, pain, and chest drainage. The Purdue Veterinary Hospital soft tissue surgery service handout on thoracotomy (labeled 2023) describes this kind of intensive monitoring. How long the stay lasts varies. An uncomplicated case may go home sooner, while ongoing fluid drainage, infection, or other complications can extend it.

At home, expect restricted activity and instructions specific to the surgery, which may include medication, wound care, and follow-up appointments. Follow the discharge team’s written instructions for your dog rather than general guidance from another case.

Sources describe several categories of risk: anesthesia complications, bleeding, infection, problems at the surgical site, lung complications, persistent air leakage (pneumothorax), chest-drain problems, and postoperative breathing difficulty. The underlying disease and the dog’s other health problems affect how likely these are. When to call the vet urgently
Contact the treating veterinarian promptly after discharge if you notice increasing breathing difficulty, collapse, marked lethargy, worsening pain, significant swelling or discharge at the incision, or any other change that concerns you. Ask the team before you go home which symptoms should prompt a call and which should prompt an emergency visit.
Putting the survival figure in context
A frequently cited study gives a historical benchmark. Moores, Halfacree, Baines, and Lipscomb, writing in the Journal of Small Animal Practice in 2007, reported on a retrospective cohort of 70 dogs and 13 cats that had lateral thoracotomy. In that study, 91% of the dogs survived to discharge. The figure comes from a single 2007 cohort and is not a present-day success rate. Survival in the study varied with species, whether the disease was neoplastic or non-neoplastic, the dog’s age, and the procedure performed. The same study reported 62% survival to discharge among cats. That comparison is not relevant to a dog’s own prognosis, which depends on its diagnosis, its overall health, and how the surgery goes.
Questions to take to your veterinary team
- What specific finding or diagnosis is the operation meant to address?
- What procedure do you expect to perform once the chest is open, and could the plan change based on what you find?
- Is thoracoscopy suitable for this dog’s problem? If not, what limits it?
- What are the most relevant anesthesia, bleeding, breathing, infection, and chest-drain risks for this dog?
- What monitoring, drain care, pain relief, hospital stay, and home restrictions do you expect?
- Which changes after discharge should prompt an urgent call or return visit?
- If a mass is involved, what imaging or staging is needed, and what can surgery realistically achieve?
