A dog that suddenly staggers, tilts its head, or falls to one side needs prompt veterinary assessment. Vestibular disease and stroke can look almost identical at home, and an owner cannot tell from appearance which one is happening. Treat sudden balance loss as urgent until a veterinarian has examined your dog.
What to do right now
- Call your veterinarian immediately. If your dog is vomiting, distressed, or unable to stand, ask for a same-day appointment. The UK charity PDSA gives the same timing advice for vestibular signs in its vestibular disease guidance.
- Use an emergency service if the clinic is closed. Do not wait to see whether the signs settle on their own.
- Restrict movement. Keep your dog in a quiet, enclosed space, and block stairs and furniture it could jump from. Support its body when it needs to move.
- Write down the timeline. Note when the signs began, which way your dog leans or circles, whether the eyes move rapidly from side to side, and whether anything has changed since. A short video of the walking and eye movements is often useful to the clinician.
- Do not give any medication, including leftover prescriptions from another dog or from your dog’s previous illness, unless a veterinarian directs you to. Bring a complete list of current medications and supplements to the appointment.
How vestibular disease and stroke overlap
The vestibular system has peripheral parts in the inner ear and the nerves that connect to it, and central parts in the brain. A problem in either area can disturb balance and coordination. Because both pathways feed the same balance control, the signs of a vestibular problem and a stroke can be very similar.
The overlap is clearest in the following signs:
- Sudden loss of balance
- Falling or circling to one side
- Head tilt
- Nystagmus (rapid, involuntary eye movements)
- Nausea, vomiting, or reduced appetite
The American Animal Hospital Association describes stroke signs as sudden and dependent on the brain area affected. Its list includes head tilt, trouble walking, falling to one side, and abnormal eye movements, which also appear in vestibular disease. It also lists altered awareness or personality, blindness, and seizures. The sources reviewed do not present these three as typical features of uncomplicated vestibular disease, so their presence is a reason to escalate. AAHA’s guidance is direct: “If your cat or dog is showing any signs that potentially indicate a stroke, contact your veterinarian immediately.”
A sudden onset or a head tilt on its own does not establish a benign, idiopathic episode. Only a veterinary examination can separate the possibilities.

The possible causes behind the same signs
Older dogs often receive a diagnosis of “old dog vestibular disease,” which is usually idiopathic. Idiopathic means that no specific cause was identified after evaluation. It does not mean a cause has been ruled out. The 2023 specialist review in Frontiers in Veterinary Science calls idiopathic vestibular syndrome a diagnosis of exclusion, meaning other causes must be considered and reasonably excluded first (Mertens, Schenk & Volk, 2023).
Idiopathic vestibular syndrome
This is typically acute to peracute, improving, and non-painful peripheral vestibular dysfunction. Its course is covered in the recovery section below. It is a conclusion reached by exclusion, so it should not be assumed from the signs alone.
Middle- or inner-ear infection
Infection of the middle or inner ear can cause vestibular signs. Treatment is directed at the infection, and VCA Animal Hospitals notes that antibiotics or antifungals may be used.

Medication effects
Certain medications can damage the inner ear and produce vestibular signs. Any medicine your dog has taken recently should be reported to the veterinarian, along with the timing of doses.
Trauma and tumors
Head or ear trauma and tumors affecting the vestibular pathways are on the list of possible causes. These are typically identified through the history, examination, and imaging rather than by the signs alone.
Hypothyroidism and thiamine deficiency
Hypothyroidism is listed among possible causes. The specialist review also includes thiamine deficiency among the differential diagnoses. Blood testing is the usual way to assess both.

Central (brain) causes, including vascular events
Brain disease can produce the same balance signs. The specialist review lists cerebral ischemia, meningoencephalitis, and tumors among the differential diagnoses. Cerebral ischemia is a reduction of blood flow to the brain, which is the category a stroke belongs to. This is the group in which a stroke and idiopathic vestibular syndrome must be distinguished.
What your veterinarian will check
The diagnostic process starts with the onset and progression of signs, the medical history and medications, the physical and neurological examination, and an examination of the ears. Depending on the suspected cause, the veterinarian may recommend some or all of the following:

- Blood and urine tests, including thyroid values
- Blood pressure measurement
- Skull radiographs
- Otoscopic examination, with sampling when appropriate
- Cerebrospinal fluid analysis
- Infectious disease tests
- MRI or CT imaging
- BAER hearing testing in some dogs
Not every dog needs advanced imaging. The 2023 specialist survey describes a detailed neurological examination, comprehensive blood work including thyroid values, blood pressure, and otoscopy as core elements. It describes MRI and cerebrospinal fluid analysis as part of a thorough workup to rule out other causes. This reflects the opinions of specialists surveyed in that paper, not a universal diagnostic protocol.

Treatment depends on the cause
- Idiopathic cases: supportive nursing and anti-nausea treatment, with the course described below.
- Ear infection: cause-specific treatment, such as antibiotics or antifungals.
- Nausea: anti-nausea or motion-sickness medication may help, as VCA notes.
- Severe cases: hospitalization, IV fluids, and nursing support until the dog can eat, drink, and walk.
Drug choices and doses are for your veterinarian to decide, based on your dog’s examination and history.

Home care is appropriate only once a veterinarian has assessed your dog and agreed on a plan. In that setting, PDSA suggests the following:
- A small, safe area for resting, with food and water placed close by
- Help with toileting, since a dog may struggle to stand and walk to the usual spot
- Carpeted or non-slip flooring without raised edges
A low-profile, non-slip dog mat without raised edges is a practical aid for slipping risk on hard floors. It does not treat the condition and does not replace veterinary care. VCA adds that a dog may need help standing, walking, toileting, eating, drinking, and staying clean.

Signs that mean the plan needs to change
Contact your veterinarian again if any of the following occur:
- Signs worsen at any point after the first examination
- Your dog does not begin to improve within the timeframes above
- Your dog cannot eat, drink, stand, or walk
- New signs appear, such as altered alertness or personality, blindness, seizures, or collapse
VCA and PDSA both describe further investigation or referral as the next step when signs do not settle. The likely tests and outlook depend on the underlying cause.
What the evidence does and does not establish
No reliable population prevalence or incidence figure exists in the sources reviewed for this article, so this article does not give one. The recovery ranges come from clinical overviews and a specialist survey, and they describe groups of dogs, not the course your dog will follow. The PDSA guidance dates from January 2021; the VCA overview was updated September 4, 2026; and the AAHA stroke guidance was accessed October 7, 2026. Read each range with its date and source, and let your veterinary team interpret them for your dog.
